A comparative study of the effects of different fucoidans on cefoperazone-induced gut microbiota disturbance and intestinal inflammation.

Antibiotic abuse can lead to gut microbiota disturbance and intestinal inflammation, which in turn may lead to serious inflammatory bowel disease and metabolic syndromes. To investigate the effect of fucoidan on alleviation of the side effects of antibiotics and its structure-activity relationship, we compared the effects of two fucoidan fractions with medium and low molecular weights (MF and LF) from Laminaria japonica on microbiota dysbiosis, colonic inflammation and intestinal mucosal damage in a cefoperazone-induced intestinal injury mouse model. Our results showed that oral administration…

The effect of liposomal cefoperazone against Pseudomonas aeruginosa in a granulocytopenic mouse model of acute lung infection.

The therapeutic efficacy of liposomal cefoperazone against Pseudomonas aeruginosa was investigated in a granulocytopenic mouse model of acute lung infection. Granulocytopenia was induced in mice by intraperitoneal (i.p.) injection of 200 mg/kg cyclophosphamide. Mice were challenged by exposure to an aerosol containing P. aeruginosa and were treated i.p. with liposomal cefoperazone prepared by the dehydration-rehydration method. The half-life of free cefoperazone in the lungs following i.p. administration of the liposomal drug was significantly lengthened (13 min vs. 261 min), and the cefoperaz…

Changing patterns of resistance to new beta-lactam antibiotics. In vitro efficacy of cefoperazone against bacterial pathogens.

No evidence for significant increase in resistance to cefoperazone was detected in susceptibility test surveillance programs or in the literature through 1983, but rare endemic resistance to cefoperazone and some other newer beta-lactams was found. Medical centers contemplating the use of a third-generation drug for cost-containment should be aware of the susceptibility of isolates in their hospitals, and the drug's ability to withstand beta-lactamase hydrolysis by local pathogens. The possible dangers of inducible cephalosporins in certain Enterobacteriaceae and strains of Pseudomonas aerugin…

Cefoperazone: a review of its antimicrobial spectrum, beta-lactamase stability, enzyme inhibition, and other in vitro characteristics.

The in vitro qualities of cefoperazone were reviewed on the basis of international medical literature and some new observations. Cefoperazone is highly active against the Enterobacteriaceae. Its activity against Staphylococcus aureus is comparable to that of the other newer cephem antibiotics. Cefoperazone is also active against all beta-hemolytic streptococci and Streptococcus pneumoniae and is relatively inactive against methicillin-resistant S. aureus and enterococci. Against Pseudomonas aeruginosa cefoperazone is at least fourfold more active than cefotaxime or moxalactam and is approximat…

In vitro and in situ evidence for the contribution of Labrasol and Gelucire 44/14 on transport of cephalexin and cefoperazone by rat intestine.

In vitro and in situ intestinal transport of beta-lactam antibiotics in the presence of two novel pharmaceutical excipients, caprylocaproyl and lauroyl macrogolglycerides (Labrasol and Gelucire 44/14), is described. The objective was to compare the effects of both macrogolglycerides on the intestinal transport of cephalexin, a substrate of oligopeptide transporters, and cefoperazone, a non-substrate of them. The in vitro transport studies were performed using a sheet of rat jejunum mounted in Ussing-type diffusion chambers. The in situ studies used an isolated internal loop model in the rat. L…

[Cefoperazone in the prevention and treatment of experimental plague caused by typical and fraction-free pathogen strains in white mice].

The high susceptibility of the plague microbe to cefoperazone (MIC of 0.1-0.25 microgram/ml) did not depend on the causative agent ability to produce fraction I. Cefoperazone, a 3rd generation cephalosporin, was highly active in the treatment of experimental plague caused by the plague microbe strain typical in the antigen composition: the drug daily dose of 250-500 mg/kg provided an 80-100 percent survival of the albino mice. The efficacy of cefoperazone lowered when the infection was caused by the strain defective in the capsule antigen. The use of the antibiotic for more prolonged periods p…

Comparison of effectiveness and safety of cefoperazone-sulbactam versus ceftriaxone and flomoxef in acute cholangitis: a multicenter retrospective study.

Acute cholangitis is a potentially life-threatening biliary tract infection requiring prompt antibiotic therapy. Cefoperazone-sulbactam is a recommended option for mild-to-moderate disease, but comparative evidence versus ceftriaxone and flomoxef is limited. In this multicenter retrospective study, 389 adults with acute cholangitis from nine Taiwanese centers (January 2022-July 2024) received cefoperazone-sulbactam (n = 249), ceftriaxone (n = 37), or flomoxef (n = 103). The primary outcome was clinical response (cure or improvement) on days 4, 7, and 14. Secondary outcomes were microbiological…

Development of novel cefoperazone-loaded glycerosomes for enhanced wound healing and antibacterial efficacy: In vitro and in vivo assessments.

Bacterial burn wound infections can disrupt the natural wound-healing process, leading to life-threatening diseases. This study aimed to investigate the use of glycerosomes as a novel system for the topical delivery of cefoperazone to promote its burn wound healing action. Cefoperazone-loaded glycerosomes (CEFO-GLY) were optimized using a 23 full-factorial design and prepared via the thin film hydration method. The optimized CEFO-GLY formulation was evaluated for its antibacterial activity using a minimum inhibitory concentration (MIC) assessment. The wound healing efficacy of the optimized CE…

In vitro activities of piperacillin or cefoperazone alone and in combination with beta-lactamase inhibitors against gram-negative bacilli.

We investigated in vitro activities of piperacillin or cefoperazone alone and in combination with beta-lactamase inhibitors against Gram-negative bacilli. Piperacillin/tazobactam 8:1 lowered resistance for Escherichia coli, Serratia marcescens, Enterobactaer cloacae, Klebsiella pneumoniae and imipenem-susceptible Acinetobacter baumannii. When piperacillin was combined with sulbactam 2:1 or 4:1, resistance against E. coli, S. marcescens, E. cloacae, extended spectrum beta-lactamase (ESBL)-K. pneumoniae and A. baumannii were reduced. MIC90 of cefoperazone against S. marcescens, E. cloacae, ESBL-…

In vitro antimicrobial spectrum, occurrence of synergy, and recommendations for dilution susceptibility testing concentrations of the cefoperazone-sulbactam combination.

Broth microdilution tests and an antimicrobial interaction (synergy) studies using various combinations of cefoperazone and sulbactam were performed in an effort to determine the most appropriate in vitro dilution test system. The test results with cefoperazone and sulbactam were categorized as synergistic (complete or partial) for nearly 80% of the strains isolated from clinical trial patients. The results indicate that the cefoperazone-sulbactam fixed ratio (2:1) maximized the cefoperazone spectrum of activity and best approximated the parenteral formulation of the drug. The cefoperazone-sul…

[Combined effect of sulbactam/cefoperazone and other antibiotics against clinical isolates of multi-resistant strains. I. Effect of sulbactam against beta-lactams resistant strains and in vitro combined effect of sulbactam/cefoperazone with each of piperacillin, latamoxef, ceftazidime, fosfomycin and doxycycline].

We evaluated relationships between production of beta-lactamase and their resistances to beta-lactams, effect of sulbactam (SBT), a beta-lactamase inhibitor, against beta-lactam resistant strains, and combined effect of sulbactam/cefoperazone (SBT/CPZ) with other antibiotics against multi-resistant strains. Through these studies, we obtained the following results. 1. Most of the strains resistant to beta-lactams were beta-lactamase producers. 2. Relationships between the production of beta-lactamase and their resistances to beta-lactams indicate that their resistances generally were the highes…

Accidental alcohol ingestion triggering severe disulfiram-like reaction in a child receiving cefoperazone-sulbactam: a rare case report.

BACKGROUND: The disulfiram-like reaction (DLR) is a potentially life-threatening adverse drug reaction triggered by concomitant alcohol exposure during treatment with certain medications, particularly cephalosporins containing a methyltetrazolethiol (MTT) side chain such as cefoperazone-sulbactam. It is well-documented in adults, however pediatric cases are rarely reported. CASE PRESENTATION: We report the case of a 4-year-old boy hospitalized with bronchopneumonia who received intravenous cefoperazone-sulbactam for 7 days with good clinical response. On the 8th day of hospitalization, approx…

Nitrite Promotes ROS Production to Potentiate Cefoperazone-Sulbactam-Mediated Elimination to Lab-Evolved and Clinical-Evolved Pseudomonas aeruginosa.

Cefoperazone-sulbactam (SCF)-resistant Pseudomonas aeruginosa poses a big challenge in the use of SCF to treat infection caused by the pathogen. We have recently shown exogenous nitrite-enabled killing of naturally and artificially evolved Pseudomonas aeruginosa strains (AP-RCLIN-EVO and AP-RLAB-EVO, respectively) by SCF. However, the underlying mechanism is unknown. Here, reprogramming metabolomics was adopted to investigate how nitrite enhanced the SCF-mediated killing efficacy. Nitrite-reprogrammed metabolome displayed an activated pyruvate cycle (P cycle), which was confirmed by elevated a…

Comparative in-vitro activity of cefoperazone/sulbactam and 11 other agents against multidrug resistant Bacteroides fragilis group species.

The activity of cefoperazone/sulbactam was compared with eleven other antimicrobial agents against 91 strains of the Bacteroides fragilis group known to be resistant to clindamycin and/or beta-lactam agents. Sulbactam alone possessed moderate activity against most strains and improved the activity of cefoperazone and ampicillin. Bacteroides distasonis strains were more resistant than other species. There was no correlation between resistance of an isolate to either clindamycin or cefoxitin with resistance to cefoperazone/sulbactam.
