Fracture of the anterior arch of atlas after minor trauma of the immature spine postlaminectomy.

Torticollis is a common complaint in the pediatric emergency department. Here, we report what we believe to be the first example in a young child of a fracture of the anterior arch of the atlas associated with an acquired, postsurgical defect of the posterior arch. A brief review of pediatric cervical spine injuries and fractures is presented. Atlas laminectomy may predispose patients to isolated atlas fractures even with minor trauma. Those fractures, however, are stable and treated by hard cervical orthosis.

Occipitocervical reconstruction through direct lateral and posterior approach for the treatment of primary osteosarcoma in the atlas: a case report.

STUDY DESIGN: Case report. OBJECTIVE: To report a new surgical technique of the primary cervical osteosarcoma in the atlas. SUMMARY OF BACKGROUND DATA: Primary osteosarcoma of the atlas is extremely rare. This is the first report that describes a surgical treatment of the primary osteosarcoma in the atlas. Resection of an osteosarcoma in the atlas is very difficult because of the complex and important anatomic structures that surround it, and secure reconstruction of the atlas is difficult as well. METHODS: A 48-year-old man was referred to our institute with a 10-month history of a palpabl

C1 Stenosis - An Easily Missed Cause for Cervical Myelopathy.

C1 stenosis is often an easily missed cause for cervical myelopathy. The vast majority of cervical myelopathy occurs in the subaxial cervical spine. The cervical canal is generally largest at C1/2, explaining the relatively rare incidence of neurological deficits in patients with odontoid fractures. However, some subjects have anatomical anomalies of the atlas, which may cause stenosis and result in clinical symptoms similar to subaxial cord compression. Isolated pure atlas hypoplasia leading to stenosis is quite rare and may be associated with other anomalies, such as atlas clefts or transver

Functional morphology of the first cervical vertebra in humans and nonhuman primates.

The cervical vertebral column bears or balances the weight of the head supported by the nuchal muscles that partly originate from the cervical vertebrae. The position of the head relative to the vertebral column, and consequently locomotion and posture behavior, could thus be associated with the form of the cervical vertebrae. In spite of this assumption and some empirical indications along these lines, primate vertebral morphologies have been reported to be very similar and not clearly related to locomotion. We therefore study the relationship between the morphology of the first cervical vert

Atlas fractures: treatment and long-term results.

Fractures of the first cervical vertebra (C1) represent approximately 7% of all acute cervical spine fractures. Frequently, the atlas will be fractured in combination with the axis (44% of cases). Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute traumatic atlas fractures are reviewed. The long-term outcome from an atlas fracture is good (median follow-up, 40 months).

Traumatic neuropathy of second cervical spinal nerves.

The second cervical spinal nerves are unduly vulnerable to forcible approximation of the arches of the atlas and axis and to excessive rotation of the atlas on the axis. Sequelae of such injury include sensory aberrations ranging from loss of feeling to severe neuralgia and disorders of balance. Diagnosis of second cervical neuropathy may be difficult when there are multiple injuries to the cervical spine, but most cases clear up spontaneously within one to three years.

A single fracture in the atlas ring: report of two cases and a review of the literature.

Atlas fractures commonly present more than two breaks in its ring structure because of the unique anatomy and the mechanism of injury. The incidence of a single break in the atlas ring is exceedingly rare. However, we encountered two cases of a single fracture of the atlas. One patient was struck by a falling tree on the right side of the head and the other was involved in a motor vehicle accident. Radiographs of the cervical spine failed to show a single fracture or other abnormalities. Computed tomography demonstrated the single fracture. The patients were treated conservatively with a cervi

Isolated atlas-duplication as a manifestation of persistent proatlas: a case report.

PURPOSE: Atlas-duplication is an exceedingly rare dysplasia of the craniocervical junction. To the best of our knowledge, only two cases of atlas-duplication have been reported and these were associated with complete anterior rachischisis and os odontoideum. We aimed to report a case of isolated atlas-duplication of incidental finding and without attributable symptoms which makes it unique. METHODS: Following a normal coronarography for a suspected myocardial infarction, a 60-year-old-man with no significant medical history developed a transient ischemic attack that justified brain computed-t

Acute traumatic atlas fractures: management and long term outcome.

Fractures of the 1st cervical vertebra (C1) represent 7% of all acute cervical spine fractures. Isolated atlas fractures are most commonly bilateral or multiple fractures through the ring of C1. Frequently (44% of cases), the atlas will be fractured in combination with the axis. Treatment of isolated C1 fractures should be governed by the rules of Spence. The treatment of combination C1-C2 fractures is dictated by the type and severity of the C2 fracture. Experience with 57 cases of acute atlas fractures is reviewed. Nonoperative external immobilization was used in 53 patients (with 1 failure)

Surgical Management of Unstable Atlas Fractures: A Single Institutional Retrospective Review with Comparison of Occipitocervical and Atlantoaxial Fusion Outcomes.

BACKGROUND: Atlas fractures represent 25% of craniocervical injuries, most common in elders. Unstable first cervical vertebra fractures with lateral mass displacement require surgical fixation. Options include atlantoaxial fusion (AAF) or occiput-cervical fusion (OCF), each have their respective merits and detractors. Here, we compare the clinical and neurological outcomes of patients with unstable atlas fractures treated with AAF and OCF. METHODS: A retrospective review of patients with unstable atlas fractures that underwent either AAF or OCF between 2005 and 2023 was performed. Demographic

Unilateral arcuate foramen with a posteriorly projecting vertebral artery loop between C1-C2: a rare anatomical variation.

PURPOSE: To describe the morphological features of this rare anatomical variation and to evaluate its significance in relation to embryological development, potential pathological consequences, and surgical considerations. METHODS: A detailed anatomical dissection of the craniovertebral junction was conducted. Morphometric analysis was conducted to document the characteristics of the arcuate foramen and the course of the vertebral artery, with emphasis on osseous bridging and vascular loop. RESULTS: On the right side, two osseous bridges (ponticulus posticus and ponticulus lateralis) were iden

Cervical injury related to mandibular fractures. A retrospective study of 315 cases.

Objective To define the association between mandibular fractures and alterations of the cranio-atlo-axial joint (CAAJ). Methods 315 cases of displaced mandibular fractures were retrospectively evaluated by CT scan and cervical X-ray for associated alterations of the CAAJ. Statistical analysis employed the chi-square and Cohen's kappa coefficient. Results Single fractures amounted to 69.84% and multiple fractures to 30.16% of total fractures. CT scans showed a rotation of the atlas on the same side of single fractures and subluxation of the CAAJ. Vertical derangement of the joint was observed i
