OBJECTIVE: To analyse deliveries after a previous cesarean section.
STUDY DESIGN: The way of birth of 262 patients between 1993-1995 in the Ist Department of Obstetrics and Gynaecology of Warsaw University Hospital was analysed. Material was divided into two groups: an elective cesarean section operation was performed on 117 women, while 145 women underwent a trial of labour.

Of 711 patients who were delivered after one or more previous cesarean sections, 17 (2.4%) had an extremely serious complication. Uterine rupture and placenta previa or placenta accreta with accompanying hemorrhage were the major contributors to mortality and major morbidity. This prospective investigation was undertaken to determine whether routine ultrasound visualization of the gravid uterus shortly before cesarean section would provide useful information in determining the site for uterine incision. Patients delivered by cesarean section are at risk for postoperative infectious morbidity, especially those patients who have labored with ruptured membranes for a long period of time. The bacteria involved in these infections are predominantly those of the patient's lower genital tract, both aerobes and anaerobes. 55 deaths following a Caesarean section occurred in Austria during 1975-1982 (41,7% of all maternal deaths). 45 deaths (82%) were directly related to the Caesarean section, in 9 cases (16%) a severe underlying disease was found which had contributed to the fatal outcome (indirect deaths). A very rare complication after cesarean section is reported. The spontaneous cecal perforation observed caused an acute abdomen. The re-laparotomy required clarified the situation and led to the correct therapy. The various possibilities by which this clinical picture may arise are dealt with on the basis of the available literature. Extreme overstretching of the cecum is evidently the decisive etiological factor.

In this case report we describe the fourth finnish woman with toxic-shock-syndrome (TSS). The illness begun seven weeks after caesarean section abruptly with high fever, abdominal pain, erythematous rash and sore throat. She was discharged after two weeks of hospitalization in a good condition and there was in a follow up time of four years no recurrence.

A Clostridium perfringens infection is described in an 18-year-old primigravida following cesarean section at 43 weeks' gestation. Despite the ubiquitous distribution of these organisms, such infections are rarely encountered in obstetrics. Characteristics of these bacteria and appropriate diagnostic and therapeutic procedures are presented. Despite the infrequency of this potentially lethal complication, survival depends on early diagnosis and aggressive treatment.

Management of 226 patients with previous low transverse cesarean section in a special obstetric clinic for high-risk pregnancies significantly and safely reduced the incidence of purely elective cesarean section to less than 10%. A substantial incidence (25%) of other high-risk factors dictating antepartum indicated cesarean section were found in this population. Pyogenic spondylodiscitis is an uncommon bacterial infection of the intervertebral disc and the vertebral endplates.
