Cesarean section rates and outcomes in Fiji have not been previously reported in the literature. Between 1986 and 1996, Fiji's cesarean section rates rose 2.5%, from 9.4% to 11.9%. Labor dystocia (33%), repeat cesarean (18%), and "fetal distress" (17%) were the most common indications for performing c-sections. A retrospective case-control study covering the period 1986-1996 in Fiji's three referral hospitals found that cesarean deliveries were three times more likely to involve child mortality (O.R. = 3.01, 95% c.l. = 1.19 < OR < 8.08), 26 times more likely to involve maternal morbidity (O.R.

The incidence of cesarean scar rupture varies from 0.64 to .24 per cent, with some institutions reporting no ruptures. Associated maternal mortality is extremely low, especially in low segment transverse incisions, so as to be almost nonexistent. Maternal mortality from cesarean section is higher than from vaginal delivery. Fetal mortality as a result of low segment scar rupture is at most 12.5 per cent. With modern methods of monitoring this should be reduced to zero. Several authors already report no fetal mortalities. Patients who have had a previous cesarean section should be allowed an at

From January 1992 to December 1997, 13 parturients with cerebrovascular diseases had childbirth at our institution. Among them, 8 patients received anesthesia for delivery. Five patients had a history of ruptured arteriovenous malformation (AVM), cerebral aneurysm, or intraventricular bleeding due to moyamoya disease, and they had radical operations. Of these 5 patients after radical operations, three had a repeat cesarean section under spinal anesthesia, and two had a vaginal delivery under epidural anesthesia to avoid excessive hypertension and hyperventilation. There were two patients with

Objective: We compared the extraperitoneal cesarean section to transperitoneal cesarean on fetal delivery time.Material and methods: This randomized study included 210 pregnant women undergoing cesarean section for elective reasons, repeat cesarean (< four), or dystocia. Patients who required an urgent cesarean section, who were at high risk for obstetric or maternal bleeding, who had a uterine or adnexal mass, or who requested tubal ligation were excluded from the study. The primary outcome of the study was the skin incision-to-delivery time. The sample size was set to detect of 1-minute diff

The indications for all cesarean sections done at Charleston Area Medical Center from 1972 to 1979 were reviewed. The cesarean section rate rose from 4.4% in 1972 to 8.8% in 1975, to 17.2% in 1979. Repeat cesarean sections accounted for 30.3% of all cesarean sections. The increase in the rate of primary cesarean sections was due to more cesarean sections being done for old indications as well as for the additional indications. From 1972 to 1979, cesarean sections performed for indication of dystocia increased threefold, for fetal distress ninefold, and for breech presentation elevenfold. Dysto

OBJECTIVE: Our goal is to determine whether a trial of labor in women with suspected fetal macrosomia would be a valuable alternative to elective repeat cesarean.
MATERIAL: and methods: Based on retrospective analysis of 355 women with previous cesarean section who delivered macrosomic infants (> or =4,000g), we tried to determine the impact of fetal weight on a trial of labor. The outcomes of trial of labor with fetal macrosomia were compared on the one hand to those of elective repeat cesarean and on the other hand to those of trial of labor with normal birth weight (<4,000g).
RESULTS: The

The records of 1319 patients undergoing cesarean section were evaluated to determine the relationship between postoperative febrile morbidity and fertility during the five years after the operation. Infection was significantly more common among patients undergoing primary as compared with repeat cesarean section and was three times more frequent among patients with membranes ruptured for 12 hours or more compared with those with membranes ruptured less than 12 hours. After correction of the data for voluntary infertility, neither postcesarean section endometritis nor pelvic cellulitis had a de

Florida legislation implemented in the fall of 1992, unique in the nation, mandated that practice guidelines regarding cesarean section deliveries be disseminated to obstetric physicians. The law also required that peer review boards at hospitals be established to review cesarean deliveries and that the exact dates of implementation of the guidelines be reported to a state agency. To determine the impact of the legislation, we conducted a retrospective analysis of 366,246 total live births occurring in Florida hospitals during 1992 and 1993, before and after formal hospital certification of th

In spite of the recognized occurrence of cesarean-attributable adhesions, its clinical significance is uncertain. The presence of adhesions during a repeat cesarean section can make fetal extraction lengthy and the procedure challenging and may increase the risk of injury to adjacent organs. Two methods for adhesion prevention are discussed, peritoneal closure and use of adhesion barriers. Peritoneal closure appears to be safe in the short term. In the long term, conflicting evidence arise from reviewing the literature for possible adhesion reduction benefits. A systematic review of the litera

The rate of cesarean delivery has risen steadily over the past fifteen years from a rate of 17% in 1980 to 24% in 1995. In an effort to curb medical expenses and reduce the risk associated with operative delivery, many suggestions have been proposed to reduce the rate of cesarean sections. The most promising of these is to reduce the number of repeat cesarean deliveries through vaginal birth after cesarean section (VBAC). For fifteen years the American College of Obstetrics and Gynecology has proposed VBAC as a safe alternative to repeat C-section. VBAC carries a 1% to 2% risk of uterine ruptu

PURPOSE: This study aimed to assess the impact of delivery mode combinations after the second birth on maternal pelvic floor differences using transperineal ultrasound.
METHODS: A total of 266 women were enrolled in this study. According to the different delivery modes of two births, women were categorized as repeat vaginal birth (RVB), vaginal birth after cesarean section (VBAC), cesarean section after vaginal birth (CAVB), and repeat cesarean section (RCS) groups. Univariate analysis was performed to compare levator hiatus area (LHA-V), bladder neck-symphyseal distance on Valsalva (BSD-V),

OBJECTIVE: We aimed to validate a widely used US prediction model for vaginal birth after cesarean (Grobman et al. [8]) and modify it to suit Swedish conditions.
STUDY DESIGN: Women having experienced one cesarean section and at least one subsequent delivery (n=49,472) in the Swedish Medical Birth Registry 1992-2011 were randomly divided into two data sets. In the development data set, variables associated with successful trial of labor were identified using multiple logistic regression. The predictive ability of the estimates previously published by Grobman et al., and of our modified and ne

OBJECTIVE: We sought to test the hypothesis that an extraperitoneal cesarean section (ECS) technique reduces postoperative pain without increasing intraoperative and postoperative complications.
STUDY DESIGN: In a single-center, single-blinded prospective trial we randomized 54 patients with an indication for primary or first repeat cesarean section at term pregnancy to an ECS (n = 27) or transperitoneal cesarean section (TCS) (n = 27) procedure. Patients with suspected abnormal placentation, a history of >1 cesarean section, or major abdominal surgery were excluded. The primary endpoint of t

BACKGROUND: We evaluated the predictive value of risk factors for repeat cesarean delivery identified in retrospective studies.
STUDY DESIGN: We identified 175 consecutive patients who underwent trial of labor (TOL) and compared detailed admission, intrapartum, and postpartum characteristics of those who required repeat cesarean delivery with those who had vaginal births. We calculated relative risks, positive predictive values, and sensitivities for potentially predictive admission characteristics. We also performed multiple logistic regression and classification analyses.
RESULTS: Ninety-f

Although the current literature attests to the merits of a trial of labor in the patient with a prior cesarean section, some controversies remain. For example, can women with two or more sections be allowed a trial of labor and can patients who undergo a trial of labor receive oxytocin or prostaglandins for induction? Also, do certain indications for previous cesarean section such as relative cephalopelvic disproportion/failure to progress or the diagnosis of breech or twins in the present pregnancy constitute an indication for elective repeat cesarean delivery? These questions along with some

A study of 112 cesarean hysterectomies is presented. They were performed in a 220-bed community hospital by six obstetricians in private practice. Eighty-nine cases were planned during the prenatal course of uncomplicated pregnancies. Twenty-three were unplanned and were performed because of severe medical, surgical, or obstetric problems. Postoperative performance and rate of complications were more favorable in the planned group. True indications for cesarean hysterectomy are discussed. In selected cases, sterilization by cesarean hysterectopmy may be the procedure of choice at the time of r

OBJECTIVE: To report a case of cesarean scar molar pregnancy.
CASE REPORT: A 31-year-old, gravida 8, para 1, female patient underwent suction curettage for a diagnosis of molar pregnancy. However, she had persistent vaginal spotting and residual decidual tissue in a cesarean scar. She was successfully treated with repeat suction curettage.
CONCLUSION: Cesarean scar pregnancy is difficult to diagnose and must be considered in the patient with a history of cesarean section who has persistent vaginal bleeding after suction curettage. To our knowledge, this is the first report of molar pregnancy

Gravid uterine torsion less than 45 degrees is a common phenomenon of the third trimester. Torsion greater than 45 degrees represents a rare, pathologic, and obstetric emergency. The rotation of the uterus on a longitudinal plane can result in vascular compromise, and it has potential for catastrophic maternal-fetal complications. We report the case of a 22-year-old G3P1011, third pregnancy with history of one full-term live newborn, one spontaneous abortion, and presented at 38 weeks gestation with complaints of abdominal pressure and recurrent transverse fetal presentation. She underwent an
