Non-specific effects of diphtheria tetanus pertussis vaccination on child mortality in Cebu, The Philippines.

BACKGROUND: To determine the non-specific effects of diphtheria, tetanus and pertussis (DTP) vaccination and sex on mortality before 30 months of age among those who received Bacille Calmette Guerin (BCG) vaccine in a high mortality area. METHODS: This analysis used a longitudinal study of child survival monitoring the use of primary care services, morbidity and mortality in Metro Cebu, The Philippines. Participants included 14 537 children under 30 months of age who received a BCG vaccination from July 1988 to January 1991. The main outcome measure was all-cause mortality. RESULTS: Mortalit

Under-five child mortality and associated factors among tribal populations from a South Indian district.

This paper reports child mortality and associated factors among tribal (indigenous) populations from a South Indian district. In India, 104 million people belonged to 705 tribal groups, constituting 8.6% of India's population. Of the 705 tribal groups, 75 were classified as particularly vulnerable tribal groups (PVTG). The present study aims to report the under-five child mortality among the tribal (both PVTG and non-PVTG) population in Visakhapatnam district, a district with a higher concentration of tribes in the Indian state of Andhra Pradesh. Four sub-districts were selected to conduct a c…

Respiratory syncytial virus-associated mortality in a healthy 3-year-old child: a case report.

BACKGROUND: Respiratory syncytial virus (RSV) is the most frequently identified pathogen in children with acute lower respiratory tract infection. Fatal cases have mainly been reported during the first 6 months of life or in the presence of comorbidity. CASE PRESENTATION: A 47-month-old girl was admitted to the pediatric intensive care unit following sudden cardiopulmonary arrest occurring at home. The electrocardiogram showed cardiac asystole, which was refractory to prolonged resuscitation efforts. Postmortem analyses detected RSV by polymerase chain reaction in an abundant, exudative peric

Child and Adolescent All-Cause Mortality in the United States vs Peer Nations: 1935-2023.

BACKGROUND: Children in the United States experience higher all-cause mortality than high-income peer nations, but the timing, durability, and age and sex contributions to this gap remain unclear. OBJECTIVES: To characterize the timing, persistence, and age- and sex-specific patterns of the US child and adolescent mortality disadvantage relative to peer nations. METHODS: Time-series comparison using vital statistics from the Human Mortality Database (1935-2023), comparing all-cause mortality among those aged 0 to 19 years in the United States vs those living in 13 high-income peer nations fr

Sociodemographic determinants of child mortality based on mothers' attitudes toward partner violence: Evidence from Bangladesh.

Bangladesh, a lower-middle-income country in South Asia, has achieved a significant reduction in child mortality over the last three decades from 151 to 40 per 1000 live births. However, child mortality is still considered high, which may be attributed to a lack of awareness among mothers regarding the risk factors, particularly their perceptions of intimate partner violence (IPV). To investigate the effect of demographic and socioeconomic factors of women on child mortality, this study extracted data from the cross-sectional survey of Bangladesh Multiple Indicator Cluster Survey (MICS) 2019. …

Assessment of a modified Child-Pugh-Turcotte score to predict early mortality after liver transplantation.

OBJECTIVE: The Model for End-Stage Liver Disease (MELD) predicts mortality on the transplant list; however, it has not been of much use to predict posttransplant outcomes. Several prognostic models have been tested among patients with cirrhosis; nevertheless, their predictive value has not been established in the posttransplant setting. We recently modified the Child-Pugh-Turcotte (CPT) score by adding creatinine levels (CPT + Cr), which has proven useful for patients with alcoholic cirrhosis. This retrospective analysis sought to predict early (1 month) mortality using CPT + Cr versus 5 other

Changes in maternal and child mortality rates in three isolated Gambian villages over ten years.

As an adjunct to the general nutrition research programme, the MRC Dunn Nutrition Unit has provided for the past 10 years a continuous medical service to three adjacent rural Gambian villages. There have been substantial reductions in annual childhood mortality rates. Perinatal mortality fell from 109.6 to 45.5 per 1000 births, infant mortality from 148.5 to 24.5 per 1000 live births, and mortality in children aged 1-4 years from 109.1 to 13.3 per 1000. There have been no pregnancy-related maternal deaths in the community for 8 years; 16 could have been expected given current estimates of mate

Child deaths caused by Klebsiella pneumoniae in sub-Saharan Africa and south Asia: a secondary analysis of Child Health and Mortality Prevention Surveillance (CHAMPS) data.

BACKGROUND: Klebsiella pneumoniae is an important cause of nosocomial and community-acquired pneumonia and sepsis in children, and antibiotic-resistant K pneumoniae is a growing public health threat. We aimed to characterise child mortality associated with this pathogen in seven high-mortality settings. METHODS: We analysed Child Health and Mortality Prevention Surveillance (CHAMPS) data on the causes of deaths in children younger than 5 years and stillbirths in sites located in seven countries across sub-Saharan Africa (Ethiopia, Kenya, Mali, Mozambique, Sierra Leone, and South Africa) and s…

Humility is critical in science communication: lessons from the UN's recent report on child mortality estimates.

The United Nations announced a historic milestone in global child mortality in March 2024, with deaths among children less than 5 years falling below 5 million in 2022. While this news is welcome, the headline news is too definitive and masks uncertainties in the results. The UN's projections rely heavily on modeled estimates based on historical data, with only 5% of the 2022 estimate derived from countries with actual data for that year. The model also fails to account for the potential temporal discontinuity caused by the COVID-19 pandemic and is inconsistent with related data on declining v…

Causes of Death Among Infants and Children in the Child Health and Mortality Prevention Surveillance (CHAMPS) Network.

IMPORTANCE: The number of deaths of children younger than 5 years has been steadily decreasing worldwide, from more than 17 million annual d … rtain conditions. CONCLUSIONS AND RELEVANCE: In this cross-sectional study investigating causes of child mortality in the CHAMPS Network, results indicate that, in these high-mortality settings, infectious diseases continue to cause most deaths in infants and children, often in conjunction with malnutrition. Decline in Child Hospitalization and Mortality After the Introduction of the 7-Valent Pneumococcal Conjugative Vaccine in Rwanda.

Pneumonia is a public health problem in the tropics, and the 7-valent pneumococcal conjugative vaccine (PCV-7) has been introduced in an effort to prevent the disease and therefore reduce childhood mortality. In Rwanda, PCV-7 was introduced in 2009, and we aimed to determine its impact on the rate of child hospitalization/mortality due to pneumonia. A retrospective survey was conducted on hospitalization rates and pediatric deaths between two periods, that is, before the introduction of PCV-7 (2007-2009) and after the introduction of PCV-7 (2010-2013) in Kabutare District Hospital. There was a

Urbanisation and child health in resource poor settings with special reference to under-five mortality in Africa.

The health of children improved dramatically worldwide during the 20th century, although with major contrasts between developed and developing countries, and urban and rural areas. The quantitative evidence on urban child health from a broad historical and comparative perspective is briefly reviewed here. Before the sanitary revolution, urban mortality tended to be higher than rural mortality. However, after World War I, improvements in water, sanitation, hygiene, nutrition and child care resulted in lower urban child mortality in Europe. Despite a similar mortality decline, urban mortality in…

Strategies and causes of reduced infant and young child diarrheal disease mortality in Cuba, 1962-1993.

This article describes the basic strategies employed by Cuba's Diarrheal Disease Control Program (DDCP) to reduce acute diarrheal disease (ADD) mortality among infants and young children from 1962 through 1993, together with the diarrheal disease trends recorded in these years. An initial control effort, the Program to Combat Gastroenteritis, began operating in 1963. Since then, in one form or another, increasingly effective efforts have consistently lessened ADD mortality. Among other things, these efforts have concentrated on providing improved sanitation, effective health education, proper

Survival analysis of maternal, household, environmental, and child-level determinants of under-five mortality in Nigeria.

BACKGROUND: In Africa, Nigeria bears the highest burden of under-five mortality, with an estimated 850,000 under-five deaths from preventable causes reported in 2024. Existing evidence has not applied a time to event approach using recent nationwide data to examine factors associated with child mortality in this high-burden setting. The current study employed a time to event approach to examine factors affecting under-five mortality across a 59-month life course. METHODS: Retrospective data from the 2024 Nigeria Demographic and Health Survey was used. We analyzed data for 27,604 under-fives. …

Child Mortality and Nutritional Risks in Rural Chad: A Community-Based Cross-Sectional Study in Béré.

Chad, a Sub-Saharan country, has some of the worst child mortality and health indicators. A lack of recent and accurate health records in Béré, rural Chad, due to decades of strife compromises the development of relevant health interventions by Project 21, a community health program. This study investigates child mortality, weight gain ability, and related factors in 0-2-year-olds through a cross-sectional study completed by trained community health workers using a survey questionnaire. Most household heads are Nangtchéré Christian males with secondary-level education. Male infants have the hi…

The introduction of diphtheria-tetanus-pertussis vaccine and child mortality in rural Guinea-Bissau: an observational study.

BACKGROUND: and objective Previous studies from areas with high mortality in West Africa have not found diphtheria-tetanus-pertussis (DTP) vaccine to be associated with the expected reduction in mortality, a few studies suggesting increased mortality. We therefore examined mortality when DTP was first introduced in rural areas of Guinea-Bissau in 1984-1987. Setting Twenty villages in four regions have been followed with bi-annual examinations since 1979. SUBJECTS: In all, 1657 children aged 2-8 months. Design Children were weighed when attending the bi-annual examinations and they were vaccin

Impact of spatial distribution of permethrin-impregnated bed nets on child mortality in rural northern Ghana.

Effects of the distribution in space of permethrin (insecticide)-impregnated bed nets (IIBNS) on child mortality were studied in a randomized controlled trial of IIBNs in a an area highly endemic for Plasmodium falciparum malaria in rural northern Ghana. Eight hundred sixty-two deaths occurred among children 6-59 months of age during 16,841 child-years-at-risk. Mortality increased with the distance from health facilities but not with proximity to identifiable anopheline breeding sites (reservoirs). The efficacy of IIBNs was independent of these distances. Mortality in users of IIBNs was indepe…
