Surgical burn secondary to the use of alcoholic chlorhexidine.

Surgical wound infection is one of the main sources of postoperative morbidity/mortality. Skin preparation with an antiseptic agent such as chlorhexidine alcohol solution is an effective method to reduce the incidence of surgical wound infections. Nevertheless, the use of alcohol solutions is associated with some risk as they are inflammable products. We report a case of a 51-year-old male who suffered an extensive burn associated with concurrent use of electrocautery and clorhexidine alcohol solution.

Making a choice of catheterisation gel and the role of chlorhexidine.

It is estimated that over 90 000 people in the UK community live with an indwelling catheter ( Wilson, 2016 ). It is essential that a lubricating gel is used during catheterisation, as this helps to reduce the risk of pain, trauma and infection ( Loveday et al, 2014 ; National Institute for Health and Care Excellence, 2012 ). Chlorhexidine is an antiseptic contained in many catheterisation gels. Evidence demonstrates that increasing numbers of patients are experiencing hypersensitivity reactions to this ingredient ( Australian and New Zealand College of Anaesthetists, 2014 ). This article cons…

Is cetrimide-chlorhexidine risky for secondary sclerosing cholangitis?

BACKGROUND: The liver is the most frequent organ for placement of hydatid cyst disease. All known protoscolicidals that are used for echinococcus degeneration have a risk of caustic secondary sclerosing cholangitis. The cetrimide-chlorhexidine combination is an effective protoscolicidal agent for treatment of hydatid liver cysts. OBJECTIVES: The aim of this experimental study was to examine this combination for potential risks of caustic secondary sclerosing cholangitis. MATERIAL AND METHODS: Thirty rats were enrolled and divided into two groups. In the study group, 0.15 mL of a cetrimide (0…

Chlorhexidine urticaria: a rare occurrence with a common mouthwash.

Chlorhexidine is a widely used antiseptic and disinfectant in medical and nonmedical environments. Compared to its ubiquitous use, allergic contact dermatitis from chlorhexidine has rarely been reported and so its sensitization rate seems to be low. The prevalence of contact urticaria and anaphylaxis due to chlorhexidine remains to be unknown. This case report presents a case of urticaria due to oral use of chlorhexidine. The adverse reaction was confirmed by skin prick test.

Anaphylaxis to the chlorhexidine component of Instillagel: a case series.

Anaphylaxis to chlorhexidine is rare. We report three cases of anaphylaxis attributed to the chlorhexidine component of Instillagel, presenting after urological surgery, while the patients were in the recovery room. In these cases, the cause of the collapse was not immediately obvious as the presentation was delayed. Anaesthetists should be aware that urethral lubricants may contain chlorhexidine that can trigger anaphylaxis in susceptible individuals. Anaphylaxis should be considered a possible diagnosis when a patient collapses in the recovery room. Investigation of suspected anaphylactic re…

Anaphylaxis to an urethral lubricant: chlorhexidine as the "hidden" allergen.

We report on a severe allergic reaction during urethral catheterisation, initially erroneously attributed to natural rubber latex (NRL). However, the negative investigations for latex, the uneventful further applications of NRL, the positive skin test and basophil activation test for chlorhexidine strongly support diagnosis of chlorhexidine anaphylaxis. This case illustrates that it is not widely appreciated that "innocent" products such as urethral lubricants may elicit serious life-threatening reactions. Careful inspection of the medical records is once again re-emphasised to be mandatory in…

Chlorhexidine anaphylaxis: case report and review of the literature.

Chlorhexidine is a widely used antiseptic and disinfectant. Compared to its ubiquitous use in medical and non-medical environments, the sensitization rate seems to be low. Multivarious hypersensitivity reactions to the agent have been reported, including delayed hypersensitivity reactions such as contact dermatitis, fixed drug eruptions and photosensitivity reactions. An increasing number of immediate-type allergies such as contact urticaria, occupational asthma and anaphylactic shock have been reported. In the case report, we describe anaphylaxis due to topical skin application of chlorhexidi…

Clinical and microbiological effects of local chlorhexidine applications.

AIM: To study the effects of an application of chlorhexidine varnish (40%) on dental plaque. DESIGN: Randomised controlled clinical trial. SUBJECTS: 40 patients (50% females), aged 25-34 years after undergoing any necessary restorative treatment. Exactly one half of these individuals practiced good oral hygiene, the efforts of the others were poor. METHOD: Test groups received a chlorhexidine varnish application, the control group received a placebo varnish. Initially and after 2 and 6 weeks, a modified Dentocult SM-test, bleeding on probing, and a plaque index (Quigley-Hein) were recorded.…

Trial of chlorhexidine gluconate for fungal corneal ulcers.

AIMS: Suppurative corneal ulcers due to filamentous fungi are a serious and intractable problem in many tropical developing countries. In vitro studies and a small pilot study have shown that chlorhexidine gluconate is effective. The aim was to establish the optimum concentration which would be appropriate to use in a larger randomized clinical trial. METHODS: A masked randomized clinical trial of three concentrations of chlorhexidine compared with natamycin 5% was carried out in consecutive patients with established corneal ulcers shown by microscopy to contain fungal hyphae and later proven…

[Two cases of anaphylactic shock induced by chlorhexidine].

We reported two patients who developed skin eruptions and severe hypotension immediately after scrubbing their wound in the leg using 4% chlorhexidine solution. Both patients were successfully treated by epinephrine administration. Patient-1 (a 42-year-old man) had his wound scrubbed using this antiseptic several times before the operation. He showed a positive skin scratch test for chlorhexidine. Patient-2 (a 74-year-old man) had no prior treatment with chlorhexidine. Positive lymphocyte transformation test was not demonstrated in these patients. It has been reported that more than 10% of pat…

The effect of local application of chlorhexidine on plaque and gingivitis.

This study compared the effects on plaque accumulation and gingival inflammation, of a 0.2 percent chlorhexidine solution applied with an interdental brush, with the effects of the interdental brush with control solution. Thirty-three subjects with at least one tooth-bounded edentulous space were chosen. The trial was of a double-blind, cross-over design with two, 28-day active periods separated by a 21-day rest period. The experimental solution was 0.2 percent aqueous chelorhexidine, and 0.1 percent quinine solution was used as the control. Prior to the commencement of the trial, plaque accum…

Chlorhexidine rinsing in physically handicapped pupils in Katlehong.

Poor oral hygiene and a high prevalence of marginal gingivitis are characteristic findings in handicapped persons. The present double-blind, cross-over study was to evaluate a twice daily mouth rinse of 10ml 0.2 per cent chlorhexidine on plaque and gingivitis in physically handicapped pupils aged 6 to 21 years. The plaque index (PI) and gingival index (GI) at baselines were relatively lower than in other published studies. For PI the mean score did not exceed 1.0 at any time; for GI the same was true except for tooth 16 which was 1.1. Only mild gingival inflammation was present. The chlorhexid…

Chlorhexidine solutions, gels and varnishes in caries prevention.

To combat dental caries, a chemical has been sought that possesses stronger antimicrobial properties than fluoride in support of its physicochemical tooth-protecting properties. These searches have led to several agents, of which chlorhexidine (CH) appears most effective. To reduce local side effects of the well-known 0.2% CH mouthwash, a 0.05% CH gluconate + 0.04% NaF solution, pH 5.9, has been developed. Use of this combination over a 2-year period resulted in a 53% reduction in caries increment and a 75% reduction in gingival bleeding, i.e. a clear duality of prevention of oral disease (Luo…

Antiseptic chitosan acetate-chlorhexidine spray for wound cleaning.

The presence of microbial biofilms in acute and chronic wound infections creates a significant dermal complication as these biofilms interfere with wound healing and complicate the regenerative process. Effective cleaning of wound bed remains essential because it eliminates necrotic materials, biofilms, and contaminants leading to faster treatment and decreased secondary infections. A novel antiseptic spray formulation using chitosan acetate (CA) and chlorhexidine (CHX) was developed aiming infection control. In the present study, an antiseptic CA-CHX spray, with poly (vinyl alcohol) (PVA) as …

Propionibacterium acnes Has Low Susceptibility to Chlorhexidine Digluconate.

BACKGROUND: The skin bacterium Propionibacterium acnes has been reported to be responsible for surgical site infections (SSIs). Skin disinfection before surgery therefore is of the utmost importance in the prevention of SSIs caused by skin bacteria. METHODS: We assessed the susceptibility of clinical isolates of two skin bacteria, P. acnes and Staphylococcus epidermidis, to disinfectants. RESULTS: The range of chlorhexidine digluconate (CHG) minimum inhibitory concentrations (MICs) for P. acnes isolates was 0.25-1 mcg/mL. Furthermore, the minimum bactericidal concentrations (MBCs) for a rang…
