BACKGROUND: This study aimed to assess the outcome of laparoscopic cholecystectomy (LC) in patients 80 years old or older.
METHODS: All consecutive patients 80 years old or older who underwent LC for symptomatic gallstone disease were evaluated. Data analysis included patients' age, gender, indication for surgery, comorbid condition, American Society of Anesthesiology (ASA) score, preoperative endoscopic retrograde cholangio pancreatography (ERCP), intraoperative cholangiogram, operative time, conversion to open surgery, morbidity, mortality, and length of stay.
RESULTS: In this study, 67 pa…

Laparoscopic cholecystectomy is nowadays the method of choice in treatment of symptomatic cholecystolithiasis, despite the fact that some operations cannot be completed laparoscopically and are converted. In a group of 431 patients operated during the last two years the authors evaluate the causes and predispositions of conversion which was performed in 7.4%. A significantly higher risk was found in subjects above 65 years of age, in those with ultrasonographic signs of thickening of the gallbladder wall and patients with acute cholecystitis. The risk of conversion is not significantly influen…

OBJECTIVE: Retrospective evaluation of laparoscopic cholecystectomy in children. The authors deal with the asset of the method in child surgery and emphasize the differences of treatment of cholelithiasis in children by a laparoscopic approach.
METHOD: During 1994 to 1998 the authors made 54 laparoscopic cholecystectomies in children 5 to 18 years old (mean age 14.0 years). The indication for surgery was in 48 patients cholecystolithiasis and in six instances confirmed chronic cholecystitis without concrements. Four times cholecystolithiasis associated with congenital haemorrhagic disease was…

BACKGROUND: The many advantages and extreme versatility made laparoscopic cholecystectomy (L.C.) the gold standard for symptomatic cholelithiasis. The aim of this research is a retrospective analysis of personal experience with laparoscopic cholecystectomy in a peripheral hospital compared with the literature on the subject.
METHODS: Since June 1982, a total of 989 L.C. have been performed. The patients were: 691 (68.6%) women and 298 (29.4%) men with a mean age of 56 years (range 17-84). The indications were: 791 symptomatic cholelithiasis, 142 chronic cholecystitis, 45 empyema-hydrops and 1…

Laparoscopic cholecystectomy has been performed in the United States since 1989 and currently is the procedure of choice for the management of symptomatic cholelithiasis. Its utility in the pregnant patient has been controversial. Concerns have been expressed for a number of potential problems, including trocar injury to uterus and fetus, effect of pneumoperitoneum on both mother and fetus, induction of preterm labor, teratogenic effects on the fetus, and long-term effects on fetal and neonatal development. We describe the Greenville Hospital System experience with laparoscopic cholecystectomy…

UNLABELLED: Several recent literature reviews have shown that Laparoscopic cholecystectomy can be performed safely in pregnant females with symptomatic gallbladder disease. We have performed a retrospective analysis of all patients who underwent a cholecystectomy (6,080 patients) from January 1, 1990 until December 31, 1996 at San Pablo Medical Center. Lapa-cholecystecomy was performed in 4,252 (64%) and in the remaining 1,828 (36%) patients, an open cholecystectomy performed. Of the Laparoscopic cases, 5(0.1%) were performed in pregnant females with complicated gallbladder disease(GBD). The d…

Acute calculus cholecystitis during pregnancy can be a difficult management problem. Two pregnant patients with cholecystitis refractory to conservative management underwent laparoscopic cholecystectomy at a community hospital. One patient was treated under epidural anesthesia. Pregnancy should not be an absolute contraindication to laparoscopic cholecystectomy, and epidural anesthesia should be considered.

On the basis of literature and of 475 laparoscopic cholecystectomies of the authors, some pitfalls are reviewed. The circumstances, the mechanism and the prevention of injuries were detailed together with the connected problem of postoperative bile leakage. Among the cholangiographic pitfalls the importance of detection of congenital and acquired anomalies of the biliary tree by means of preoperative ERCP or intraoperative trans-cystic cholangiograms was emphasized. A particular study was made of 3 pictures: Mirizzi syndrome; stone impaction in Vater's papilla; no retrograde flow of the common

This study analysed 90 cases of symptomatic cholelithiasis treated with laparoscopic cholecystectomy. 27% of the cases studied suffered from microlithiasis, 44% from multiple calculosis with calculi measuring 1 to 2 cm in diameter, 25% from single calculi measuring up to 4 cm in diameter, and 4% from a benign proliferative pathology of the cholecystic wall. Calculosis of the main biliary tract was also found in 3 patients and was treated by preoperative endoscopic papillosphincterotomy. In addition to routine tests and ultrasonography, endovenous cholangiography, or retrograde cholangiography …

Laparoscopic cholecystectomy is a safe and effective treatment of cholelithiasis in experienced hands. Mortality is rare. The Southern Surgeons Club data and several other recent large series indicate that major complications occur in less than 3% of patients. The most significant common complication is injury to the bile duct, for which the greatest risk factor is inexperience. Major biliary injury usually requires reoperations. Roux-en-Y hepaticojejunostomies, often multiple, are usually necessary for repair. The popularity of this technique continues, and further efforts should be focused o…

The positive experience with laparoscopic cholecystectomy (LC) in the adult surgical community encouraged us to perform LC in our last nine adolescent patients requiring cholecystectomy. There were no operative or postoperative complications, and the average hospital stay was less than 3 days. All the teenagers resumed their normal activities 1 week after surgery and were pleased with the small operative scars. Once the technique has been mastered and adequate experience gained with the new instrumentation, laparoscopic cholecystectomy would seem to offer many advantages in the teenage patient…

Laparoscopic cholecystectomy (LCHE) is a safe and effective way to treat gall stones. The procedure was carried out for the first time in France in 1987 and since then it has spread rapidly all over the world, in view of the excellent results achieved. The major benefits are the low morbidity, the very short hospitalization, the early return to daily life and work and its excellent cosmetic results. LCHE is indicated in symptomatic non-complicated gall-bladder disease. The operative technique is standardized, the complication rate is low. Compared with other treatments such as litholysis, extr…

As laparoscopic cholecystectomy has become more widely practiced, the full spectrum of complications associated with this technique is being realized. We have performed 283 consecutive laparoscopic cholecystectomies with no deaths and a morbidity rate of 5.3% (15 of 283 patients; six major complications, nine minor complications). Major complications included one bile duct injury requiring laparotomy and t-tube insertion and two patients with retained stones. Symptomatic bile leakage occurred in three patients (1%). Two of these bile leaks were from accessory ducts entering the gallbladder bed…

Laparoscopic cholecystectomy was performed on 65 unselected and consecutive patients, regardless of age, weight, history of abdominal surgery or presence of acute cholecystitis. All procedures were completed successfully, with only two patients converted to an open cholecystectomy. There were no intra-abdominal intraoperative complications; n o intraoperative transfusions were required. There were no intra-abdominal injuries, and no patient required repeat surgery for postoperative complications. Hospital stays averaged 30 hours, and the average time until patients resumed normal activities wa…

There were 14,232 patients operated on for non-malignant biliary tract disease at a single medical center from 1932 through 1984. During this period, there were 237 postoperative deaths. Of the total number of patients, 10,749 underwent cholecystectomy with 60 postoperative deaths. Cholecystostomy was performed in 599 patients with 60 deaths. Cholecystectomy or cholecystostomy in conjunction with common duct exploration was performed in 2,226 patients with 89 deaths. Choledochotomy alone as a secondary procedure to search for stones was performed in 374 patients with 21 deaths. Procedures for …

We are of the opinion that the investigation of the upper gastrointestinal tract must precede an elective cholecystectomy. In a prospective study the value of preoperative oesophagogastroduodenoscopy preceding elective cholecystectomy is considered. 589 gastroscopies were performed in our endoscopy unit on 960 patients coming to elective gallbladder surgery. Only in 56% of patients we detected no abnormalities. Our plan of therapy was changed in 113 patients (11.7%). 11 patients were discharged after conservative medical therapy (1.1%), in 31 cases simultaneous procedures were performed (3.2%)…

Some controlled clinical trials have demonstrated that early or immediate operation for acute cholecystitis produces better results than delayed operation or conservative treatment. From 1969 to 1983, 397 patients have been operated for acute cholecystitis at the Surgical University Hospital Würzburg, the results are discussed here.
