Analgesic and hemodynamic effects of two injection approaches of dexmedetomidine in elderly cholecystolithiasis patients undergoing laparoscopic cholecystectomy: a retrospective study of 150 patients.

This retrospective study investigated the effects of three dexmedetomidine (Dex) injection approaches on analgesic and hemodynamics in elderly cholecystolithiasis patients undergoing laparoscopic cholecystectomy. The clinical data of 150 elderly patients with cholecystolithiasis were collected, and they were divided into the Dex A (n=50), Dex B (n=50), and Dex C (n=50) cohorts. Patient's heart rate (HR) and mean arterial pressure (MAP) were collected at T0, T1, and T2 for blood gas analysis. The difference in oxygen content between cerebral arterial and venous blood (Da-jvO2) was calculated. T

Impacts of the zero mark-up policy on hospitalization expenses of T2DM and cholecystolithiasis inpatients in SC province, western China: an interrupted time series analysis.

BACKGROUND: Since 2009, a series of ambitious health system reforms have been launched in China, including the zero mark-up drug policy (ZMDP); the policy was intended to reduce substantial medicine expenses for patients by abolishing the 15% mark-up on drugs. This study aims to evaluate the impacts of ZMDP on medical expenditures from the perspective of disease burden disparities in western China. METHOD: Two typical diseases including Type 2 diabetes mellitus (T2DM) in internal medicine and cholecystolithiasis (CS) in surgery were selected from medical records in a large tertiary level-A ho

The clinical effect of a strategy called transcystic gallbladder-preserving cholecystolithotomy based on endoscopic retrograde cholangiopancreatography for cholecystolithiasis: A retrospective study from a single center.

BACKGROUND: Choledocholithiasis complicated with cholecystolithiasis is a common disease. This study explores a novel strategy, called ERCP-based transcystic gallbladder-preserving cholecystolithotomy, for the simultaneous removal of common bile duct stones and gallbladder stones. METHODS: From December 2018 to June 2021, all patients with cholecystolithiasis and common bile duct stones who met the criteria for gallbladder preservation in our hospital were included in the study and prospectively followed up. RESULTS: We included 48 patients, including 20 patients with acute biliary pancreati

Development and validation of a clinical decision tool for predicting long-term pain reduction following laparoscopic cholecystectomy in patients with symptomatic cholecystolithiasis: a prospective cohort study.

BACKGROUND: The applicability of the recently reported predictors to predict pain reduction after laparoscopic cholecystectomy (LC) is limited by only predicting short-term outcomes. The aim of this study was to develop and externally validate a model to predict clinically relevant pain reduction 5 years after LC. MATERIALS AND METHODS: This study reports on 5-year follow-up data of two multicenter prospective trials in 25 hospitals. Only patients who underwent LC were included. The model was developed using data from the SUCCESS cohort ( n = 494) and externally validated using data from the

[Preoperative and postoperative lengths of extrahepatic biliary duct measured by direct cholangiography].

The lengths of extrahepatic duct of 110 cases of cholecystolithiasis and 108 cases of choledocholithiasis were compared by direct cholangiography. An average length of the former was 11.0 +/- 1.45 cm, while that of the latter was 12.5 +/- 1.50 cm. There was remarkable change in length between cholecystolithiasis and choledocholithiasis. The length of 75 postoperative choledocholithiasis was 10.4 +/- 1.06 cm. The postoperative length of extrahepatic duct of choledocholithiasis was shorten significantly. There was no significant postoperative change in length in cholecystolithiasis and post-oper

The role of juxtapapillary duodenal diverticulum in the formation of gallbladder stones.

BACKGROUND/AIMS: Juxtapapillary diverticula (JPD) are considered to be associated with choledocholithiasis but not with cholecystolithiasis. However, there have been few comparative studies on the relationship between JPD and cholecystolithiasis under strict matching for sex and age. METHODOLOGY: Among 4542 consecutive ERCPs at Tokyo Metropolitan Komagome Hospital, 549 patients who were 63 years of age or older were enrolled in this study and were matched for sex and age. They were divided into two groups: with and without JPD. Firstly, the frequency of cholecystolithiasis was compared betwee

Changes in plasma bile acids are associated with gallbladder stones and polyps.

BACKGROUND: The development of gallbladder disease (GBD) is related to bile acid (BA) metabolism, and the rate of BA circulation increases the risk of biliary cancer. However, it is unclear whether patterns of circulating bile acids (BAs) change in patients with benign GBDs such as gallbladder stones and polyps. Herein, we compared and characterised plasma BA profiles in patients with cholecystolithiasis and non-neoplastic polyps with healthy controls, and explored relationships between plasma BA profiles, demographics, and laboratory test indices. METHODS: A total of 330 subjects (13 healthy

[How safe is laparoscopic cholecystectomy?].

INTRODUCTION: Laparoscopic cholecystectomy (LCE) is the standard treatment for symptomatic non-complicated cholecystolithiasis. However, there is still an ongoing debate about the appropriate surgical approach in cases with complicated cholecystolithiasis. As a result, an initial decision whether to use the open or laparoscopic approach must be primarily based on patient safety. Additional indications favouring the use of LCE must not lead to an increase in the rate of serious complications. METHODS: All patients who underwent operations for cholecystolithiasis performed in a single surgical

Tailoring diagnosis and treatment in symptomatic gallstone disease.

BACKGROUND: There is a lack of consensus in selecting patients who do or do not benefit from surgery when patients present with abdominal pain and gallbladder stones are present. This review aimed to give an overview of results from recent trials and available literature to improve treatment decisions in patients with uncomplicated cholecystolithiasis. METHODS: First, an overview of different symptom criteria for laparoscopic cholecystectomy in patients with uncomplicated cholecystolithiasis is given, based on national and international guidelines. Second, treatment outcomes (absence of bilia

[Problematic treatment of cholecystitis in a gastric bypass patient after abdominoplasty].

Gastric bypass patients are at risk of late procedure-related complications, e.g. internal hernia or cholecystolithiasis. These complications may be important to identify before other surgical procedures are performed. The treatment of cholecystitis based on cholecystolithiasis in a patient who had recently undergone abdominoplasty may be highly problematic and may lead to an exacerbated situation.

Biliobiliary fistulas manifested by worsening liver function--a case report.

We report a case of cholecystolithiasis with biliobiliary fistulas from gallbladder to hepatic ducts, which were manifested by worsening liver dysfunction. Although it was not diagnosed preoperatively, it was successfully treated by cholecystectomy with closure of fistulas by the gallbladder wall. This case suggests that an internal biliary fistula may be possible, when the gallbladder wall is thickened and shrunken in the case of cholecystolithiasis, accompanied with liver dysfunction despite no dilatation of the common bile duct.

[Aerobilia as an initial sign of a cholecystoduodenal fistula--a case report].

An 82 year old female patient has the following medical history: repeated chemotherapy and radiation therapy for non-Hodgkin's lymphoma, a radiation-induced osteoblastic osteosarcoma and a longstanding cholecystolithiasis with sclero-atrophic cholecystitis. She appeared at the practice and complained about a dull food-dependent mesogastric and epigastric pain as well as a discrete scleral jaundice. Sonography and CT showed air inclusions in the bile ducts. Subsequently, a cholecystoduodenal fistula was found intraoperatively. The cause could be the long-existing cholecystolithiasis.

Single-Incision Laparoscopic Cholecystectomy in Situs Inversus Totalis: A Case Report.

BACKGROUND Situs inversus totalis (SIT) is a rare developmental anomaly. For many years, laparoscopic cholecystectomy has been a well-established and effective treatment for patients with cholecystolithiasis. However, when it comes to SIT patients with cholecystolithiasis, this surgery presents unique challenges. Due to its complete mirror inversion of viscera, it is uncomfortable to carry out standard laparoscopic cholecystectomy for some surgeons. Single-incision laparoscopic cholecystectomy (SILC) offers a minimally invasive alternative to traditional laparoscopic cholecystectomy, potential
