[Cholera--once and nowadays].

HISTORY: Cholera is an acute intestinal infection which raged in India in the nineteenth century and it broke out in six great pandemics out in Europe. ETIOLOGY: Cholera is caused by bacteria Vibrio cholerae, which produce an enterotoxin causing massive diarrhoea. There are two biotypes--classic and El Tor. EPIDEMIOLOGY: The source of Vibrio cholerae is a man. Cholera is a typical faecally-oral infection. PATHOGENESIS: Ingested by contaminated water and food, Vibrio cholerae comes to the intestinal wall. The tightness is intermediated with toxin-coregulated pilus. There it produces enteroto…

Vaccines for preventing cholera.

BACKGROUND: Oral cholera vaccines (either killed whole cell or live recombinant vaccines) are newer alternatives to the parenteral vaccines which have been thought to confer only moderate and short-term immunity. OBJECTIVES: The objective of this review was to assess the effect of cholera vaccines in preventing cases of cholera and preventing deaths. SEARCH STRATEGY: We searched the Cochrane Infectious Diseases Group trials register, Medline, Embase and reference lists of articles. We handsearched the journal Vaccine, contacted researchers in the field and manufacturers. SELECTION CRITERIA:…

Cholera reveals helminthiasis problem.

Cholera has now lasted for about 6 months in our hilly region in Mwanza, Tanzania. The heavy rains coupled with the poor sanitary conditions and sewage disposal system have worsened the situation. The pit latrines used by the majority of low-income people are either not adequately and properly used for excreta disposal or they overflow. At our regional referral and consultancy hospital we were, for a period, receiving cholera cases which should have been managed in isolation at other centers. (This situation has now been rectified.) Some of the children with cholera either vomited or passed As…

Oral Cholera Vaccines as Transmission-Interrupting Tools for Cholera Control: A Narrative Review.

BACKGROUND: Cholera remains a significant global health challenge, with an estimated 1.3 billion people at risk in endemic regions and recurrent outbreaks increasingly reported in fragile, conflict-affected, and humanitarian settings. Persistent transmission continues despite advances in case management and water, sanitation, and hygiene (WASH) interventions. OBJECTIVE: To synthesize current evidence on oral cholera vaccines (OCVs) as transmission-interrupting tools and to examine their implications for integrated cholera control strategies. METHODS: This narrative review synthesized publish…

Impact of oral cholera vaccines in cholera-endemic countries: A mathematical modeling study.

BACKGROUND: Impact evaluation of vaccination programs is necessary for making decisions to introduce oral cholera vaccines (OCVs) in cholera-endemic countries. METHODS: We analyzed data to forecast the future global burden of cholera. We developed a mathematical model of cholera transmission in three countries as examples: Nigeria, Uganda, and Indonesia. After fitting the model, we evaluated the impact of OCVs delivered in four vaccination strategies varying by target age group and frequency of vaccination over the period of 2015-2030. RESULTS: Data suggest that the global annual incidence o…

Delayed second dose of oral cholera vaccine administered before high-risk period for cholera transmission: Cholera control strategy in Lusaka, 2016.

BACKGROUND: In April 2016, an emergency vaccination campaign using one dose of Oral Cholera Vaccine (OCV) was organized in response to a cholera outbreak that started in Lusaka in February 2016. In December 2016, a second round of vaccination was conducted, with the objective of increasing the duration of protection, before the high-risk period for cholera transmission. We assessed vaccination coverage for the first and second rounds of the OCV campaign. METHODS: Vaccination coverage was estimated after each round from a sample selected from targeted-areas for vaccination using a cross-sectio…

In a time of cholera.

INTRODUCTION: Dr. Nathaniel Alcock in his book A treatise on cholera described 22 cases of cholera that he treated in 1832. Blood-letting, either by leeches or venesection, was an essential part of the treatment. RATIONALE: The belief was that reducing the blood volume would relieve stress on the heart and lungs allowing for better function. The receipts of the Townsend Street Cholera Hospital where Dr. Alcock worked show how extensive the practice was. Outside Dublin, local Boards of Health dealt with the cholera epidemic. Various public measures such as street cleaning and removal of patien…
