Very high fructose intake increases serum LDL-cholesterol and total cholesterol: a meta-analysis of controlled feeding trials.

Fructose is widely used as a sweetener in the production of many foods, yet the relation between fructose intake and cholesterol remains uncertain. In this study, we performed a systematic review and meta-analysis of human, controlled, feeding trials involving isocaloric fructose exchange for other carbohydrates to quantify the effects of fructose on serum total cholesterol (TC), LDL cholesterol (LDL-C), and HDL cholesterol (HDL-C) in adult humans. Weighted mean differences were calculated to determine changes from baseline cholesterol concentrations by means of generic, inverse variance, rand

Beyond early LDL cholesterol lowering to prevent coronary atherosclerosis in familial hypercholesterolaemia.

AIMS: Familial hypercholesterolaemia (FH) patients are subjected to a high lifetime exposure to low density lipoprotein cholesterol (LDL-C), despite use of lipid-lowering therapy (LLT). This study aimed to quantify the extent of subclinical atherosclerosis and to evaluate the association between lifetime cumulative LDL-C exposure and coronary atherosclerosis in young FH patients. METHODS AND RESULTS: Familial hypercholesterolaemia patients, divided into a subgroup of early treated (LLT initiated <25 years) and late treated (LLT initiated ≥25 years) patients, and an age- and sex-matched unaffe

Elevated LDL Cholesterol Increases Microvascular Endothelial VWF and Thromboinflammation After Myocardial Infarction.

BACKGROUND: In reperfused myocardial infarction, VWF (von Willebrand factor)-mediated platelet adhesion contributes to impaired microvascular reflow and possibly also to postmyocardial infarction inflammation. We hypothesized that postischemic thromboinflammatory processes are worsened by elevated LDL (low-density lipoprotein) cholesterol. METHODS: Myocardial ischemia-reperfusion or sham procedure was performed in wild-type mice and hyperlipidemic mice deficient for the LDL receptor and Apobec-1 (apolipoprotein-B mRNA editing enzyme catalytic polypeptide-1; DKO [double knockout]). DKO subgrou

Prognostic Implication of Small Dense LDL-Cholesterol Levels following Acute Coronary Syndrome.

Background and Objectives: Small dense LDL cholesterol is a strong risk factor for atherosclerosis. However, few studies have investigated the impacts of this specific lipid profile on the incident risk of adverse cardiovascular events in patients with acute coronary syndrome. Materials and Methods: Patients with acute coronary syndrome, who underwent revascularization, were included and followed for 2 years. The levels of small dense LDL cholesterol were measured at index discharge (day 0) in the setting of newly administered therapies for secondary prevention, including aspirin and statins,

Phenome-wide association analysis of LDL-cholesterol lowering genetic variants in PCSK9.

BACKGROUND: We characterised the phenotypic consequence of genetic variation at the PCSK9 locus and compared findings with recent trials of pharmacological inhibitors of PCSK9. METHODS: Published and individual participant level data (300,000+ participants) were combined to construct a weighted PCSK9 gene-centric score (GS). Seventeen randomized placebo controlled PCSK9 inhibitor trials were included, providing data on 79,578 participants. Results were scaled to a one mmol/L lower LDL-C concentration. RESULTS: The PCSK9 GS (comprising 4 SNPs) associations with plasma lipid and apolipoprotein

Unfavourable blood pressure and LDL-cholesterol levels in obese non-diabetic individuals.

BACKGROUND: Early diagnosis and treatment of high blood pressure (BP) and cholesterol is important to reduce cardiovascular risk. We compared BP and LDL-cholesterol (LDL-C) as well as the quality of treatment between obese subjects and normal weight and overweight individuals. METHODS: 87,648 participants of the Lifelines study were categorised according to obesity (normal weight/ overweight/obesity) and age. Mean systolic BP and LDL-C were calculated depending on treatment, BMI, age and sex. RESULTS: In all age groups, except those aged 70-80 years, women had a significantly lower BP than m

Flaxseed reduces total and LDL cholesterol concentrations in Native American postmenopausal women.

OBJECTIVE: The objective of the study was to investigate the extent to which the daily incorporation of approximately 30 g of flaxseed, a rich source of lignans, omega-3 fatty acids, and fiber, for a period of 3 months into the diet of Native American postmenopausal women positively affects their lipid profiles. METHODS: Fifty-five mild to moderately hypercholesterolemic (> or =5.1 to < or =9.8 mmol/L) Native American postmenopausal women were randomly assigned to control (A), flaxseed (B) or flaxseed + additional oat bran fiber (C) groups. Overnight fasting venous blood was collected at base

Decreasing LDL cholesterol and medication cost with every-other-day statin therapy.

Statins are administered daily, but because of their long half-lives, atorvastatin and rosuvastatin can be given every other day (q.o.d.). We examined the effect of q.o.d. statin therapy in 25 patients treated with either atorvastatin (mean dose, 18.8 mg q.o.d.) or rosuvastatin (mean dose, 9.7 mg q.o.d.). Low-density lipoprotein cholesterol decreased 43%, triglycerides (mean +/- SEM) decreased 22%+/-6%, and high-density lipoprotein cholesterol increased 9%+/-6% with q.o.d. atorvastatin (n=9; p<0.05 for all). Low-density lipoprotein cholesterol decreased 28%, triglycerides decreased 15%+/-6%, a

Small dense LDL cholesterol in human subjects with different chronic inflammatory diseases.

BACKGROUND AND AIMS: Chronic inflammatory diseases (CID) are associated with a profound increase in cardiovascular (CV) risk resulting in reduced life expectancy. However, LDL-cholesterol is reported to be low in CID patients which is referred to as the "LDL paradoxon". The aim of the present study was to investigate whether LDL-particles in CID exhibit an increased content of the highly atherogenic small-dense LDL subfraction (sdLDL). METHODS AND RESULTS: In this prospective, single center, observational study we enrolled 141 patients with CID (RA n = 59, inflammatory bowel disease (IBD) n =

Non-HDL cholesterol and remnant cholesterol are better indicators of atherosclerotic cardiovascular disease risk than LDL cholesterol in patients with chronic kidney disease.

BACKGROUND AND AIMS: Low-density lipoprotein cholesterol (LDL-C) is the primary therapeutic target for lipid lowering, yet it may incompletely reflect atherosclerotic cardiovascular disease (ASCVD) risk in patients with chronic kidney disease (CKD). Non-high-density lipoprotein cholesterol (non-HDL-C) and remnant cholesterol (RC) capture cholesterol carried by a broader spectrum of atherogenic lipoproteins and may provide complementary information for risk stratification in this population. This study aimed to compare the associations of LDL-C, non-HDL-C, and RC with ASCVD risk and to evaluate

Safety and Efficacy of Achieving Very Low LDL Cholesterol Concentrations with PCSK9 Inhibitors.

Background: The advent of newer pharmacological agents, particularly proprotein convertase subtilisin/kexin type 9 (PCSK-9) inhibitors, in combination with conventional lipid-lowering treatments, has allowed for the significant lowering of low-density lipoprotein cholesterol (LDL-C). However, it is unclear if very low LDL-C levels achieved with the use of PCSK-9 inhibitors are associated with increased adverse events that may outweigh potential benefits. Methods: A systematic search of PubMed, Medline, and Cochrane databases was conducted from their inception to 21 February 2025, for randomize

Real-world exploration of LDL-cholesterol management in patients with atherosclerotic cardiovascular disease.

BACKGROUND: Although guidelines recommend low-density lipoprotein cholesterol (LDL-C) to be < 70 mg/dL in patients with atherosclerotic cardiovascular disease (ASCVD), the rate of achieving this goal remains suboptimal. We sought to understand real world contemporary practice patterns of LDL-C management in patients with ASCVD, and whether LDL-C testing influenced management across US health systems. METHODS: A retrospective cohort study utilizing electronic medical record data from five health systems participating in the CardioHealth Alliance was performed on patients with an LDL-C measurem

Rare primary dyslipidaemias associated with low LDL and HDL cholesterol values in Portugal.

Background: Dyslipidaemia represents a group of disorders of lipid metabolism, characterized by either an increase or decrease in lipid particles, usually associated with triglycerides, LDL cholesterol (LDL-C) and/or HDL cholesterol (HDL-C). Most hyperlipidaemias and HDL deficiencies confer an increased cardiovascular risk, while hypolipidaemia, such as abeta or hypobetalipoproteinemia, may present different manifestations ranging from poor weight progression to neurological manifestations. The aim of this study is to present 7 cases with rare dyslipidaemias associated with low LDL or low HDL

Prevalence, Awareness, and Treatment of Elevated LDL Cholesterol in US Adults, 1999-2020.

PURPOSE: The photoreceptor outer segment is a highly specialized ciliary structure essential for phototransduction, rendering photoreceptors especially vulnerable to ciliary dysfunction. WDR34, a key component of the retrograde intraflagellar transport machinery, has been implicated in rod-cone dystrophy. However, the pathogenic mechanisms linking WDR34 deficiency to photoreceptor degeneration remain elusive. In this study we aim to investigate the in vivo function of Wdr34 in the photoreceptor cells using conditional knockout allele of Wdr34. METHODS: We generated retina-specific Wdr34 knock

Acquired low cholesterol: diagnosis and relevance to safety of low LDL therapeutic targets.

PURPOSE OF REVIEW: Acquired hypocholesterolaemia occurs more commonly than inherited hypocholesterolaemia but has received little attention in the literature. In this review, we discuss the causes and underlying mechanisms of acquired hypocholesterolaemia and its relevance to safety of therapeutically induced decreased LDL cholesterol levels. RECENT FINDINGS: Hypocholesterolaemia is increasingly identified as cholesterol testing becomes more widespread in the assessment of cardiovascular risk. Lower therapeutic targets for LDL cholesterol are also being achieved more regularly with the introd

Circulating microRNA-126 in patients with coronary artery disease: correlation with LDL cholesterol.

BACKGROUND: Coronary artery disease (CAD) is a major problem worldwide. Atherosclerosis and thrombosis underlying CAD involve multiple cell types. New and useful diagnostic markers are required. MicroRNAs (miRNAs) are a class of noncoding RNAs that posttranscriptionally regulate the gene expressions involved in various cellular processes. Endothelial dysfunction is implicated in early processes of athero-thrombosis. Thus, it was hypothesized that the level of vascular endothelium-enriched miRNAs would be altered in plasma samples of CAD patients. METHODS: Vascular endothelium-enriched miRNA (

Hepatocyte-Targeted Cyclic Peptide LYTACs for PCSK9 Degradation and LDL Cholesterol Metabolic Regulation.

Proprotein convertase subtilisin/kexin type 9 (PCSK9) promotes the degradation of the LDL receptor (LDLR), thereby increasing LDL cholesterol (LDL-C) levels and elevating the risk of cardiovascular disease. Current antibodies and siRNA target and inhibit secreted PCSK9 but are clinically costly, noncatalytic in action, and induce feedback upregulation. High-affinity cyclic peptides (CP1/CP2) developed by Merck and Novartis block the PCSK9-LDLR interaction but induce compensatory PCSK9 elevation. Herein, we report cyclic peptide-based lysosome-targeting chimeras (LYTACs), LY1 and LY2, construct
