Despite widespread concern, ventricular tachycardia (VT) is rarely the actual cause of syncope, even in patients with structural heart disease. Indeed sustained VT occurs, it typically results in cardiac arrest rather than transient loss of consciousness, with the exception of torsades de pointes. On the other hand when syncope is arrhythmic, it is more commonly due to atrioventricular (AV) block or sinus node dysfunction, age-related conditions due to conduction system fibrosis. We argue that current guideline-based risk stratification may overestimate arrhythmic and sudden death risk in pati

A 66-year-old man presented with a 9-year history of chronic orthostatic intolerance and recurrent syncope. A review of systems also revealed several other symptoms including dry eyes and mouth, chronic nausea and bloating, constipation, urinary urgency and urge incontinence, and erectile dysfunction. He had no motor, sensory, or cognitive concerns. He took no medications that could cause autonomic dysfunction. His neurologic examination was unremarkable. The tilt table test revealed neurogenic orthostatic hypotension. His plasma norepinephrine level was low. Eventually, a treatable etiology o

A previously healthy 24-year-old man presented with acute-onset vertigo after a recent tick bite sustained during a hiking trip. The initial illness included fever, malaise, headache, and neck stiffness, all of which resolved without treatment within 1 week. Days later, he developed persistent vertigo accompanied by nausea, vomiting, and imbalance, but without hearing loss or other focal neurologic deficits. Neurologic examination localized the lesion, and imaging confirmed the suspected site of involvement. Additional diagnostic testing identified an unusual abnormality correlating with the c

OBJECTIVES: To give an overview of central aspects of pain medicine-specific clinical reasoning when assessing a pain patient. Clinical reasoning is the thinking and decision-making processes associated with clinical practice.
METHODS: Three core pain assessment areas that are crucial for clinical reasoning in the field of pain medicine are discussed, each of them consisting of three points.
RESULTS: First, it is important to distinguish acute, chronic non-cancer, and cancer-related pain conditions. This classical and very simple trichotomy still has important implications treatment-wise, e.…

A wide variety of diseases present with intracranial lesions. In this case report, a 67-year-old man initially presented to an outside hospital with nausea, headache, and ataxia and was found to have multiple intracranial lesions. Diagnostic workup was ultimately unrevealing, and his condition improved after a course of steroids and antibiotics. Unfortunately, symptoms returned 3 months later. MRI of the brain revealed progression of his intracranial lesions. This case highlights a diagnostic approach and general management strategy for patients presenting with undifferentiated intracranial pa

A 24-year-old Middle Eastern woman presented with a 2-month history of rapidly progressive asymmetric weakness and paresthesia that began in her left lower extremity and progressed to involve both legs and arms. It was associated with overflow urinary incontinence and significant weight loss. In addition, she complained of a constant occipital headache that worsened in the supine position and was associated with photophobia, tinnitus, nausea, vomiting, and horizontal binocular diplopia. She also had signs of meningismus, decreased left facial sensation, and right sensorineural hearing loss. Be

A 17-year-old girl presented with a long history of cognitive impairment, personality and behavioral changes, dysarthria, and paroxysmal lower-extremity weakness. She was initially suspected of having mitochondrial encephalomyopathy with lactic acidosis and stroke-like episodes because of stroke-like symptoms, such as episodic lower-extremity weakness, as well as abnormal brain MRI findings of generalized cerebral atrophy, extensive high-intensity lesions in the cortex and subcortical white matter on fluid-attenuated inversion recovery images, decreased N-acetyl aspartate/creatine ratio, and a

Background Situated cognition theory argues that thinking is inextricably situated in a context. In clinical reasoning, this can lead to context specificity: a physician arriving at two different diagnoses for two patients with the same symptoms, findings, and diagnosis but different contextual factors (something beyond case content potentially influencing reasoning). This paper experimentally investigates the presence of and mechanisms behind context specificity by measuring differences in clinical reasoning performance in cases with and without contextual factors. Methods An experimental stu…

BACKGROUND: To evaluate whether clinical features from the history, presentation, physical and neurological examination of dogs with cervical hyperaesthesia are statistically predictive of the underlying diagnosis.
METHODS: Two hundred and ninety-eight dogs presenting with cervical hyperaesthesia between January 2010 and October 2018 were investigated. Only neurologically normal dogs with cervical hyperaesthesia on examination were included, while those with concurrent neurological deficits including gait abnormalities and proprioceptive deficits were excluded. Univariate analysis of clinical
