INTRODUCTION: Persistent cloaca is a rare congenital malformation that can present with significant phenotypic variability. A subset of female cloaca patients presents with significant abdominal distension and deficient abdominal musculature, representing a distinct "prune belly-like" phenotype with unique challenges for neonatal care and surgical management.

We present a simple surgical technique aiming to improve urine outflow through the common urogenital sinus in cloaca and facilitate drainage of existing hydrocolpos. The study included three cases of cloaca with associated hydrocolpos that were operated during the period 2022 through 2023. A 40-year-old pregnant woman presented with a fetal abdominal cyst and oligohydramnios. Color Doppler scan revealed a single blood vessel from the fetal aorta into a single umbilical artery.
