Open Versus Closed Reduction of Maxillary Fractures: Complications and Resource Utilization.

INTRODUCTION: Maxillary fractures are frequently managed depending on the surgeon's preferences, nature of the injury, presence of associated injuries, and comorbidities. Current literature advocates open reduction with plating versus closed techniques. However, data defining associated costs and complications comparing the 2 approaches remains lacking. METHODS: National Inpatient Sample (2006-2011) was examined for patients undergoing closed or open (76.73-76.74) reduction of maxillary fractures. Treatment-related complications were regarded as re-exploration of surgical site, hemorrhage, he

Supracondylar humerus fractures in children treated with closed reduction and percutaneous pinning.

Supracondylar fractures of the humerus in children are important for frequency and type of associated serious complications. The management of this kind of fractures is still controversial (Skaggs et al. in J Bone Joint Surg Am 86:702-707, 2004; Kalllio et al. in J Pediatr Orthop 12:11-15, 1992). We are going to present our experience in the treatment of supracondylar humeral fracture in children. In the Orthopedic Department of Pisa, we treated 150 cases from 1989 to 2006. We are used to perform, emergency or within 12 h, reduction and two lateral-entry percutaneous pins fixation. The mean ag

Closed treatment of overriding distal radial fractures without reduction in children.

BACKGROUND: Traditionally, distal radial fractures with marked displacement and angulation have been treated with closed or open reduction techniques. Reduction maneuvers generally require analgesia and sedation, which increase hospital time, cost, patient risk, and the surgeon's time. In our study, a treatment protocol for pediatric distal radial fractures was used in which the fracture was left shortened in an overriding position and a cast was applied without an attempt at anatomic fracture reduction. METHODS: Consecutive patients three to ten years of age presenting between 2004 and 2009

A new closed reduction technique for the treatment of radial neck fractures in children.

Displaced radial neck fractures in the skeletally immature patient are uncommon and can be difficult to reduce. We describe a new technique for the closed reduction of radial neck fractures in children. One of the authors (R.M.) has used this technique in treating 6 children with displaced radial neck fractures. This was a retrospective review carried out at a tertiary care hospital in Winnipeg, Manitoba, Canada. All charts and radiographs were reviewed. One patient underwent an ultrasound assessment to assist reduction (case 4). Average age of the patients was 9 years and 6 months. Average fo

Closed reduction and percutaneous stabilization of pediatric T-condylar fractures of the humerus.

T-condylar fractures of the humerus are a rare fracture pattern in children. The usual recommendation is to treat them with open reduction and internal fixation, in accordance to the usual practice in adult fracture patterns. This involves extensive surgical approach to the elbow to allow anatomic reduction and placement of hardware for rigid fixation. The authors present a technique of closed reduction of the intra-articular component of the T-condylar fracture that is stabilized with partially threaded pins that afford interfragmentary compression, followed by the use of two elastic titanium

Closed reduction of mallet fractures using extension-block Kirschner wire.

Fourteen cases of mallet fingers with a displaced large fracture fragment and/or a subluxated distal phalanx were treated by closed reduction using extension-block Kirschner wire. The follow-up evaluations took place after a mean of 9 months. The results according to Crawford's criteria were eight excellent, four good, one fair, and one poor. This technique is simple, is easier than other techniques for reduction of mallet fractures, and is associated with a low morbidity.

Closed reduction of acute femoral neck fractures-a simple and reproducible technique.

OBJECTIVE: Surgical technique of closed reduction followed by percutaneous fixation of acute femoral neck fractures. INDICATIONS: Displaced femoral neck fractures. CONTRAINDICATIONS: Open femoral neck fractures and concomitant neurovascular injuries. SURGICAL TECHNIQUE: The patient is positioned supine on a traction table, ensuring optimal fluoroscopic visualization. Closed reduction is attempted before draping to assess reducibility. Gentle axial traction restores length and valgus alignment, followed by internal rotation to correct anterior angulation and restore alignment, guided by the

Closed Reduction Technique for Severely Displaced Radial Neck Fractures in Children.

BACKGROUND: The described technique is useful for achieving closed reduction of severely displaced (i.e., Judet Type-III and IV) pediatric radial neck fractures. It is widely agreed that radial neck fractures with angulation of >30° should be reduced. Various maneuvers have been described, but none uniformly achieves complete reduction in severely displaced radial neck fractures (Types III and IV)1-4. The aim of the present technique is to achieve closed reduction in these severely displaced radial neck fractures without surgical instrumentation. DESCRIPTION: A stepwise approach is described.

Development of a 3D printed simulator for closed reduction of distal radius fractures.

BACKGROUND: The use of simulators in medical education is critical for developing procedural competence prior to treating patients. Current training of emergency physicians to perform distal radius fracture reduction is inconsistent and inadequate. APPROACH: We developed a 3D printed distal radius fracture simulation training model that is easy to assemble and relatively inexpensive. We present step-by-step instructions to reproduce the model. EVALUATION: The model was found to have high fidelity for training by both instructors and participants in a simulation-based mastery learning course.

Closed Reduction Techniques for Distal Radius Fractures and Appropriate Casting Methods.

Distal radius fractures are one of the most common orthopedic injuries. After history, physical examination, and diagnostic imaging, treatment begins with closed reduction and immobilization to restore an upper extremity that has both acceptable alignment and stability. Whether for definitive nonoperative management or in preparation for surgical fixation, it is important to understand the principles of closed reduction and immobilization. This article presents a brief review of indications and a technical guide to successful closed reduction and casting for the orthopedic surgeon.

Is Surgical Navigation Useful During Closed Reduction of Nasal Bone Fractures?

OBJECTIVE: To report the case of a 42-year-old woman with a nasal bone fracture that was easily treated using a surgical navigation system. METHODS: In this clinical report, the authors suggest that intraoperative surgical navigation systems are useful diagnostically and for localizing sites of nasal bone fractures exactly. RESULTS: The patient underwent successful closed reduction of the nasal bone fracture. CONCLUSIONS: Surgical navigation is a useful tool for identifying nasal bone fracture locations and for guiding closed reduction. Surgical navigation is recommended when nasal bone fra

Predicting alignment after closed reduction and casting of distal radius fractures.

PURPOSE: We sought to independently validate the McQueen equation and LaFontaine's criteria as predictors of instability in a large series of distal radius fractures treated nonsurgically. In addition, we hypothesized that restoring the volar cortical integrity (ie, volar hook) would be another factor that would independently predict the maintenance of a closed reduction in a cast. METHODS: We screened 546 consecutive distal radius fractures with 168 meeting all inclusion criteria. Dorsal tilt, radial height, radial inclination, ulnar variance, and carpal malalignment were measured on initial

Closed reduction of distal radius fractures: does instability mean irreducibility?

INTRODUCTION: The belief that not all distal radius fractures can be initially anatomically reduced with conservative means is rising. The aim of this study was to examine whether adequate reduction with a closed reduction technique is possible and to assess the importance of each step. MATERIALS AND METHODS: We prospectively enrolled 63 distal radius fractures (62 patients). A standardized reduction technique was implemented. Reduction was radiologically evaluated in hanging traction, after reduction, and in plaster. Subgroup analysis was performed for fracture-dependent and fracture-indepen

Optimizing closed reduction of nasal and zygomatic arch fractures with a mobile fluoroscan.

BACKGROUND: Although closed reductions of nasal fractures and zygomatic arch fractures are considered minor procedures, improper reductions are not uncommon. Objectively evaluating reduction adequacy with imaging assistance during surgery is crucial. The authors used mobile Fluoroscan intraoperatively to assess the adequacy of closed reduction for nasal fractures and zygomatic arch fractures. METHODS: Patients with nasal fractures or zygomatic arch fractures who underwent surgery between 2000 and 2004 were enrolled. Results were reviewed according to postoperative photographs and radiography.

Technique and timing for closed reduction of isolated nasal fractures: a retrospective study.

Isolated fractures of the nasal pyramid are among the most common facial injuries. Nevertheless, studies of therapeutic results following closed reduction of nasal fractures are rare. We conducted a retrospective clinical review of 187 patients who were evaluated for nasal trauma (including nondislocated fractures, dislocated fractures, and contusions) at our otolaryngology department during 1997 and 1998. Of this group, 96 fractures were treated with closed reduction--either under local anesthesia (n = 68), under general anesthesia (n = 21), or with concomitant septoplasty under general anest

Wrist position in closed reduction of fractured carpal scaphoid. An experimental observation.

Experimental observation on the relation between the apposition of scaphoid fracture surfaces and wrist position showed that fracture reduction could be achieved when the wrist was placed in a position from 15 degrees radial deviation to 15 degrees ulnar deviation provided that the wrist assumed certain amount of extension. The principle was that the more the wrist was radially deviated, the more the extension was needed. Fracture displacement occurred regularly and could be classified into three types, which were related to different loading patterns on the distal scaphoid.
