Congenital clubfoot. Month of conception.

The medical records of 330 children who were born with uncomplicated congenital clubfoot were reviewed retrospectively. To determine their months of conception, the duration of gestation was extrapolated and those which were less than 40 weeks were noted. The years of conception for the studied children were from 1956 to 1994. Months of the year were ascribed angle values and the distribution of conceptions per month were grouped in radial manner. Using a circular analysis for variance a lack of uniform circular distribution was found for the grouped months of conception. There is a significan…

Clubfoot deformity in Down's syndrome.

The association of Down's syndrome (trisomy 21) with clubfeet has not previously been elaborated. Eight patients with a total of 15 clubfeet were identified for review. Five of these had trisomy 21 noted by chromosomal analysis, and 1 had a mosaic pattern. Two patients did not have chromosomal documentation, but had characteristic features of Down's syndrome. Interestingly, 2 patients had evidence of arthrogryposis as well as Down's syndrome. Four of the 8 patients had other orthopedic anomalies, including scoliosis, atlantoaxial instability, brachydactyly, and coxa valga. All 8 patients had a

Conservative treatment of idiopathic clubfoot.

Operation in the early stage of clubfoot is still a controversial issue. Twenty-five patients treated conservatively were studied at the age of 4-16 years. In 95%, the outcome was either excellent or good according to Laaveg and Ponseti's rating system. Operation appears not to be indicated when the residual varus, adductus, or intoeing is passively correctable at least to the neutral position. Residual equinus without fixed varus was correctable by serial casting in childhood. Muscle imbalances, which make maintenance of the corrected position difficult in infancy, tend to improve with growth…

Part 1. Congenital idiopathic clubfoot.

Although not life-threatening, clubfoot, a non-traumatic deviation of the foot, makes a strong impact on the infant, parents, and family. Serial casting with or without surgical intervention make up the medical plan. Nursing care needs to focus on emotional support of the family and education regarding the diagnosis, treatment plan, and daily care of an infant in a cast.

Congenital clubfoot: pathoanatomy and treatment.

A better understanding of the pathoanatomy of clubfoot, meticulous attention to detail during surgery, and orthotic maintenance of correction to facilitate remodeling should make it possible to improve the results of treatment.

Clubfoot deformity and tibiofibular diastasis.

Four children with distal tibiofibular diastasis associated to talipes equinovarus deformity are described. Tibial hypoplasia resulting in t … nkle disarticulation rather than reconstructive surgery. Radiographs should be made of all apparent clubfoot deformities in order to rule out the more serious distal tibiofibular diastasis deformity.

Congenital clubfoot: an electromyographic study.

Twenty-five patients with congenital idiopathic clubfoot, aged 1 day to 22 months, were studied electromyographically. No myopathic or neurogenic changes were found in tibialis anterior, peroneus, gastrocnemius, extensor digitorum brevis, and abductor hallucis brevis muscles of the leg and foot. Maximum motor nerve conduction velocities for the peroneal and tibial nerves were normal. It is concluded that conventional electromyographic techniques were unable to demonstrate abnormalities suggesting neuropathic or myopathic changes in idiopathic clubfoot.

The Treatment of Recurrent Congenital Clubfoot.

The Ponseti method for treatment of congenital clubfoot is well established and has been introduced in most pediatric orthopedic centers worldwide. However, reported rates of recurrence are largely variable and open joint surgery is still performed frequently, even in the age group younger than 6 years of age. Preventing recurrence and residual deformity can be achieved by strict adherence to the Ponseti method, ensuring and enforcing brace compliance, frequent follow-up, and early treatment of recurrence. This review discusses reasons for clubfoot recurrence, prevention of clubfoot recurrence…

The genetics of isolated and syndromic clubfoot.

PURPOSE: Congenital clubfoot is a serious birth defect that affects nearly 0.1% of all births. Though there is strong evidence for a genetic basis of isolated clubfoot, aside from a handful of associations, much of the heritability remains unexplained. METHODS: By systematically examining the genes involved in syndromic clubfoot, we may find new candidate genes and pathways to investigate in isolated clubfoot. RESULTS: In addition to the expected enrichment of extracellular matrix and transforming growth factor beta (TGF-β) signalling genes, we find many genes involved in syndromic clubfoot …

Smoking, the xenobiotic pathway, and clubfoot.

BACKGROUND: Isolated clubfoot is a common orthopedic birth defect that affects approximately 135,000 newborns worldwide. It is characterized by ankle equinus, hindfoot varus, and forefoot adductus. Although numerous studies suggest a multifactorial etiology, the specific genetic and environmental components have yet to be delineated. Maternal smoking during pregnancy is the only common environmental factor consistently shown to increase the risk for clubfoot. Moreover, a positive family history of clubfoot, in conjunction with maternal smoking, increases the risk 20-fold. These findings sugges…

Update on clubfoot: etiology and treatment.

UNLABELLED: Although clubfoot is one of the most common congenital abnormalities affecting the lower limb, it remains a challenge not only to understand its genetic origins but also to provide effective long-term treatment. This review provides an update on the etiology of clubfoot as well as current treatment strategies. Understanding the exact genetic etiology of clubfoot may eventually be helpful in determining both prognosis and the selection of appropriate treatment methods in individual patients. The primary treatment goal is to provide long-term correction with a foot that is fully func…

Treatment of the complex idiopathic clubfoot.

Treatment with the Ponseti method corrects congenital idiopathic clubfeet in the majority of patients. However, some feet do not respond to the standard treatment protocol. We describe the characteristics and treatment results of these complex idiopathic clubfeet. We identified 50 patients (75 clubfeet) from 762 consecutive patients treated at five institutions. Clinically, complex clubfeet were defined as having rigid equinus, severe plantar flexion of all metatarsals, a deep crease above the heel, a transverse crease in the sole of the foot, and a short and hyperextended first toe. The Achil
