The effect of domperidone, a peripheral antidopaminergic drug, on sigmoid motor activity in the irritable bowel syndrome, has been evaluated by measuring pressures in 3 opentipped tubes perfused with distilled water at a constant flow rate of 0.636 ml/min and inserted into the sigmoid colon. Domperidone 20 mg i.v. in 10 patients, did not induce any significant change in basal motility, but prevented the increase in motor activity produced by the infusion of dopamine 5 micrograms/kg/min for 10 min. Sixty-six patients who had a jejunoileal bypass with ileosigmoidostomy for intractable obesity were reviewed. Thirty-three patients had sudden, severe, upper pain develop in the abdomen with distention from one to four years after the original operation. Functional bowel disorders are the most frequent cause of chronic diarrhoea. Functional disorders may present as typical "diarrhée motrice" (colonic hypermotility) or as watery diarrhoea. The diagnosis cannot be made unless an organic cause has been ruled out. Although colonic motor abnormalities have been described in some patients, its physiopathology remains obscure. Treatment is the same as for functional bowel disorders, i.e. fibres and antidiarrhoeal drugs.

Patterns of gut motility, such as colonic motor complexes, are controlled by central pattern generators (CPG) in the enteric nervous system; however, the mechanisms that co-ordinate enteric neural networks underlying this behaviour remain unclear.
