A comparative study of neurotics seen in a community mental health center and in private practice.

Differences that emerge in comparisons of persons applying for psychiatric care at a mental health center with those applying to the private sector are comfounded by marked diagnostic differences beyond obvious social class differences. To circumvent that problem and to ascertain what differences, if any, persist after diagnosis is held constant, the authors examined demographic and psychosocial characteristics of neurotics in a one-year adult applicant population, the only major diagnostic group treated in large numbers and similar proportions by the center and the private sector. Findings sh

Listening to many voices: qualitative perspectives of Black and Latino community health center patients on serious illness communication.

OBJECTIVES: To elucidate perspectives of Black and Hispanic/Latino patients to improve serious illness communication (SIC) delivery at a community health center. BACKGROUND: The benefits of serious illness communication (SIC) are well described, though access to effective and culturally appropriate SIC remains inequitable. Individuals from historically marginalized communities participate less in advance care planning, experience lower-quality communication, and feel less understood by clinicians. Making SIC more accessible for all patients is an important goal. METHODS: Focus groups were co

Adherence to Daily Oral TDF/FTC for PrEP in Community Health Center Populations: The Sustainable Health Center Implementation PrEP Pilot (SHIPP) Study.

The prevention effectiveness of oral preexposure prophylaxis (PrEP) is highly dependent on medication adherence but no validated longer term PrEP adherence measures are readily available for use by primary care clinicians caring for diverse populations. We compared two self-report measures (number of doses missed in past 7 days and day-by-day past week pill taking) to results of tenofovir concentrations in dried blood spot (DBS) samples at quarterly visits over the first 12 months of PrEP use. 1420 men and women in five US community health centers enrolled in a medication adherence substudy. A scoping review on community health care centers: Evidence in support of Italian policymakers to redesign the Italian primary care system.

BACKGROUND: The COVID-19 pandemic underscored the central role of primary health care in managing patients with complex and chronic health conditions. In response, the Italian government launched a reform of the National Health Service, introducing Community Health Care Centers ("Case della Comunità") as a key pillar of the new primary care model. Evidence is needed on which organizational features of such centers best support integrated, high-quality care. OBJECTIVE: This scoping review synthesized international evidence on community health care centers characterized by integrated care and c

Recommendations to Address Barriers to Patient Portal Use Among Persons With Diabetes Seeking Care at Community Health Centers: Interview Study With Patients and Health Care Providers.

BACKGROUND: Community health centers (CHCs) are safety-net health care facilities in the United States that provide care for a substantial number of low-income, non-English speaking adults with type 2 diabetes (T2D). Whereas patient portals have been shown to be associated with significant improvements in diabetes self-management and outcomes, they remain underused in CHCs. In addition, little is known about the specific barriers to and facilitators of patient portal use in CHCs and strategies to address the barriers. OBJECTIVE: The objectives of this qualitative study were to explore the bar…

Staff perspectives on a novel pediatric developmental screening and referral program in a community health center.

There are longstanding gaps in the surveillance, screening, and referral for developmental disorders among medically underserved children, which exacerbate existing health inequities. While innovative interventions to close these gaps are emerging, little is known about clinician and staff perspectives on implementing these novel screening and referral approaches for developmental disorders within safety net settings. This study examines safety net clinician and staff perspectives on implementing the Early Childhood Initiative, a project aimed at detecting developmental concerns early and faci

Screening for Hepatitis C Among Community Health Center Patients by Ethnicity and Language Preference.

INTRODUCTION: Hepatitis C virus is associated with high morbidity and mortality-chronic liver disease is a leading cause of death among Latinos in the U.S. Screening for hepatitis C virus in community health center settings, which serve a disproportionate percentage of Latinos, is essential to eradicating hepatitis C virus infection. We assessed hepatitis C virus screening disparities in adults served by community health centers by ethnicity and language preference. METHODS: This was an observational cohort study (spanning 2013-2017) of adults born in 1945-1965 in the Accelerating Data Value …

Community eye-health and vision center guidelines during COVID-19 pandemic in India.

The severe acute respiratory syndrome coronavirus 2 (COVID-19) pandemic has disrupted our society on an unprecedented scale since its inception in December 2019. As the health-care system is finally re-organizing to mitigate the impact of the pandemic, it was necessary to re-structure primary eye care (PEC) activities as well on the same lines. A consensus meeting was held with leading eye-care experts on 2nd May 2020 to prepare a roadmap for PEC in the days to come. Guidelines are needed for PEC activities like vision testing, refraction, optical dispensing, counseling, etc., Some of the acti

Patient Perspectives on Accessing Medication Abortion at a Family Medicine-Led Community Health Center: An Opportunity for Person-Centered Care.

OBJECTIVE: To explore how patients who received care for early pregnancy loss (EPL) at a family medicine-led federally qualified health center (FQHC) felt about hypothetical access to medication abortion at the clinic. STUDY DESIGN: We elicited patient perspectives on hypothetical access to medication abortion from their primary care provider via semistructured phone interviews with patients treated for EPL at one FQHC in Illinois that was using mifepristone for EPL but not abortion. The transcripts were coded using Atlas.ti and analyzed inductively. Participant demographics, including age, r

Nurse Standing Orders for Buprenorphine Follow-Up Care in a Community Health Center Network.

BACKGROUND: Less than 20% of individuals with opioid use disorder (OUD) are receiving a medication treatment for OUD in the United States. Though nurses can assume critical roles in outpatient models of OUD care, there are no published reports of buprenorphine standing orders for nurses that guide a nuanced response for patients returning as expected versus those re-engaging after a treatment lapse, without requiring real-time prescriber consultation. METHODS: Standing orders for buprenorphine were created with multiple stakeholders within an urban community health center that includes tradit
