Difference in clinical characteristics and outcomes between hospital-acquired and community-acquired pleural infection.

Data on the clinical characteristics and outcomes of patients with pleural infection are limited. This retrospective study aimed to describe the clinical characteristics and outcomes associated with community-acquired pleural infection (CAPI) and hospital-acquired pleural infection (HAPI) in a large Chinese tertiary-care hospital in Beijing over 11 years. The study included 832 patients, of whom 84.9% had HAPI. The most common cause of CAPI was pneumonia and pleurisy, whereas the most common cause of HAPI was trauma or chest surgery. The predisposing factors with statistically significant diff

[Targeted bacteremia surveillance throughout a year--comparison of community-acquired and hospital-acquired infection].

To keep an eye on severe nosocomial infection and to evaluate the clinical difference of blood-stream infection between community-acquired and hospital-acquired infection, a survey of blood culture was performed in National Tokyo Medical Center from the period between November 2000 and October 2001. There were 252 episodes detected in 219 patients (80 community-acquired episodes in 80 patients and 172 hospital-acquired episodes in 139 patients). The three most common foci of infection/pathogens were as follows: in the community-acquired cases; urinary tract, pneumonia, infective endocarditis/E

Fluoroquinolone Resistance in Escherichia coli Causing Community-Acquired Urinary Tract Infections: A Systematic Review.

Community-acquired urinary tract infections account for 15% of all outpatient use of antibiotics, and women are primarily affected; the major causative microorganism is uropathogenic Escherichia coli (E. coli). Treatment is indicated for cystitis and pyelonephritis and includes B-lactams (amoxicillin-clavulanic acid or third-generation cephalosporins), fluoroquinolones (ciprofloxacin or levofloxacin), nitrofurantoin, fosfomycin, and trimethoprim-sulfamethoxazole. Resistance to antibiotic treatment is of concern; several mechanisms have been associated with the acquisition of genes that confer

Drivers of Prolonged Outpatient Antibiotic Therapy for Urinary Tract Infections and Community-Acquired Pneumonia.

BACKGROUND: Variability exists in treatment duration for community-acquired pneumonia (CAP) and urinary tract infection (UTI) in children and may be associated with non-clinical factors. METHODS: A retrospective study was conducted of patients treated for outpatient CAP and UTI in a children's hospital network from 2016 to 2019. Multivariable logistic regression was performed to identify predictors of long antibiotic duration (≥10 days). Hospitalization within 30 days was determined. RESULTS: Overall, 2124 prescriptions for CAP and 1116 prescriptions for UTI were included. Prescriptions were

Understanding etiology of community-acquired central nervous system infections using metagenomic next-generation sequencing.

BACKGROUND: Community-acquired central nervous system infections (CA-CNS infections) have the characteristics of acute onset and rapid progression, and are associated with high levels of morbidity and mortality worldwide. However, there have been only limited studies on the etiology of this infections. Here, metagenomic next-generation sequencing (mNGS), a comprehensive diagnosis method, facilitated us to better understand the etiology of CA-CNS infections. METHODS: We conducted a single-center retrospective study between September 2018 and July 2021 in which 606 cerebrospinal fluid (CSF) sam

The burden and epidemiology of community-acquired central nervous system infections: a multinational study.

Risk assessment of central nervous system (CNS) infection patients is of key importance in predicting likely pathogens. However, data are lacking on the epidemiology globally. We performed a multicenter study to understand the burden of community-acquired CNS (CA-CNS) infections between 2012 and 2014. A total of 2583 patients with CA-CNS infections were included from 37 referral centers in 20 countries. Of these, 477 (18.5%) patients survived with sequelae and 227 (8.8%) died, and 1879 (72.7%) patients were discharged with complete cure. The most frequent infecting pathogens in this study were

Common community-acquired infections and subsequent risk of multiple myeloma: a population-based study.

The role of bacteria and viruses as aetiological agents in the pathogenesis of cancer has been well established for several sites, including a number of haematological malignancies. Less clear is the impact of such exposures on the subsequent development of multiple myeloma (MM). Using the population-based U.S. Surveillance Epidemiology and End Results-Medicare dataset, 15,318 elderly MM and 200,000 controls were identified to investigate the impact of 14 common community-acquired infections and risk of MM. Odds ratios (ORs) and associated 95% confidence intervals (CIs) were adjusted for sex,

[Comparison of community-acquired pneumonia in relation to influenza A and RS virus infections].

We experienced 142 cases with community-acquired pneumonia between April 1998 and March 2000. By measuring the titers of respiratory viruses for these cases, we were able to identify acute phase infections of influenza A virus in 10 cases and RS virus in 6 cases and determined that there was an increase in community-acquired pneumonia during both winter seasons. Thereafter we compared the clinical features of community-acquired pneumonia with regard to these two types of virus infection by dividing the patients into two groups, both of which frequently included in the elderly. In the influenza

New antibiotics for community-acquired lower respiratory tract infections: improved activity at a cost?

Opinions are changing as to the continued validity of traditional beta-lactam and macrolide therapy of community acquired LRTI, partly because of resistance and partly because of early evidence that suggests that some new agents may be more effective. Guidelines are altering to reflect this view, although there is conflicting evidence on their effects on outcome. Fluoroquinolones are becoming accepted in the treatment of community acquired pneumonia and are established choices for acute exacerbations of chronic bronchitis. The 8-methoxy fluoroquinolones, moxifloxacin and gatifloxacin, have exc

Antibiotic resistance patterns of community-acquired urinary tract infections caused by Enterobacterales in Albania.

BACKGROUND: The escalating rates of antibiotic resistance, especially through Extended-Spectrum Beta-Lactamase (ESBL) production within Enterobacterales represents a critical challenge in the effective management and treatment of urinary tract infections (UTIs). This study aims to examine the distribution of pathogens, assess the prevalence of ESBL production, and analyze the resistance patterns of Enterobacterales isolated from outpatient urocultures in Albania. METHODS: Midstream urine specimens from 406 patients were cultured on MacConkey agar and analyzed using the VITEK 2 automated syste
