Pelvic actinomycosis: A confusing diagnosis.

INTRODUCTION AND IMPORTANCE: Actinomycosis is a rare chronic and suppurative infection caused by anerobic Gram Positive bacteria: actinomyces. Pelvic location is extremely rare, usually associated with history of IUD contraception and doesn't have specific signs. Pelvic actinomycosis diagnosis may be confused with pelvic gynecologic malignancies or abscess. We present a retrospective and descriptive study of twelve patients with pelvic actinomycosis diagnosed and managed in our department from January 2000 to December 2011. CASES PRESENTATION: The patients' mean age was 47 years. 75% of them

[Confusion in the diagnosis of endobronchial tumor].

Bronchopulmonary cancer is the leading cause of death in men and the second in women. Some endoscopic or radiological features may guide histological diagnosis and thus facilitate therapeutic management. We here report the case of a 54-year old man, with a history of smoking and recent coronary stent implantation, presenting with haemoptysis and worsening of dyspnea which had evolved over the last month. Chest x-ray showed left pulmonary hemifield lucency with signs of retraction. Bronchial fibroscopy objectified raspberry bud formation spontaneously bleeding, originating from the left main br

Concussion is confusing us all.

It is time to stop using the term concussion as it has no clear definition and no pathological meaning. This confusion is increasingly problematic as the management of 'concussed' individuals is a pressing concern. Historically, it has been used to describe patients briefly disabled following a head injury, with the assumption that this was due to a transient disorder of brain function without long-term sequelae. However, the symptoms of concussion are highly variable in duration, and can persist for many years with no reliable early predictors of outcome. Using vague terminology for post-trau…

Management of acute confusion in the elderly.

Acute confusion in the elderly as a presenting symptom in an accident and emergency department requires just the same energy devoted to diagnosis as does, say for example, acute coma, epilepsy or haematemesis. Doctors in accident and emergency departments are reminded not to succumb to the pitfalls of assuming that acute confusion is merely part of a progressive dementia in an elderly person and therefore incapable of treatment. In this paper a number of clinical examples are given where treatment of the cause of the acute confusion has led to restoration of the patient's independent existence…

Medical admissions: a confusing disease.

It is strictly for admission to undergraduate medical courses. (i) 50% of seats should be reserved for local candidate on merit of HSC, with academic year starting after the HSC results. (ii) 20% of seats should be reserved for candidates coming from rural areas, on the basis of merit at HSC, with academic year starting after HSC results. (iii) 30% of seats to be filled by an all-India competitive examination. The academic year to start 6 months after the HSC results. A ceiling of minimum number of marks and a minimum number of attempts for eligibility to the all-India PMT would make the condu

[A woman in her 60s with confusion].

BACKGROUND: Delirium is common in patients admitted to hospital, and may sometimes be the first sign of a serious underlying condition. Identifying a specific cause can be complex, especially in patients without clear precipitating factors. CASE PRESENTATION: A woman in her sixties was admitted to hospital with neurological symptoms and acute confusion. Delirium of unknown origin was suspected. Blood tests revealed elevated troponins and ECG showed subtle ST changes, but she reported no chest pain. CT revealed infarctions in the kidney and spleen, and brain MRI showed multiple embolic infarct…

Psychopathy: a confusing clinical construct.

Although psychopathy has traditionally been cited as a disorder of personality, confusion arises as the term is used interchangeably with the terms antisocial personality disorder and dissocial personality disorder, both of which are largely behaviorally based. This paper aims to provide an overview of the literature on the topic of psychopathy, which examines this conundrum. Included in the discussion are definitions of psychopathy, incidence, approaches to diagnosis, and the debates that surround causes, manifestations, and treatability. Experimental studies and theoretical papers have been …

A case of breathlessness, confusion and diarrhoea.

A 61-year-old woman presented to the emergency department with acute-onset breathlessness, fever, sore throat and confusion. Her initial investigations revealed hyponatremia and middle lobe consolidation. In view of the atypical symptoms and signs, erythromycin was commenced. Urinary legionella antigen was requested and that tested positive. She was one of the first few patients whose findings alerted us to a possible outbreak of legionnaire' disease. We drew the following conclusions from our experience with this and other cases that we saw during the legionnaires' outbreak: an atypical prese…
