[Choice of treatment strategy for corneal opacities in patients with mucopolysaccharidosis type VI].

Mucopolysaccharidoses (MPSs) are a group of rare disorders caused by inherited defects of lysosomal enzymes, leading to widespread intra- and extracellular accumulation of glycosaminoglycans (GAGs). The use of enzyme replacement therapy (ERT) and hematopoietic stem cell transplantation has significantly reduced the systemic manifestations of MPS but has little effect on the severity of corneal opacity in ophthalmic involvement. Therefore, the choice of the management strategy for patients with these disorders remains a complex and relevant issue. Only a limited number of studies have addressed…

Phacoemulsification in patients with corneal opacities using slit illumination of the surgical microscope.

Performing phacoemulsification in eyes with corneal opacities is challenging even with expert surgeons. Several techniques have been described to improve intraoperative visualization through opacified corneas. This retrospective interventional case series included 10 eyes of 10 patients with coexisting senile cataract and corneal opacity who underwent phacoemulsification with intraocular lens (IOL) implantation under slit illumination of the surgical microscope. Uneventful phacoemulsification with IOL implantation was achieved in all eyes without intraoperative complications. Slit illumination…

Corneal Opacity in Domestic Ducks Experimentally Infected With H5N1 Highly Pathogenic Avian Influenza Virus.

Domestic ducks can be a key factor in the regional spread of H5N1 highly pathogenic avian influenza (HPAI) virus in Asia. The authors performed experimental infections to examine the relationship between corneal opacity and H5N1 HPAI virus infection in domestic ducks (Anas platyrhyncha var domestica). A total of 99 domestic ducks, including 3 control birds, were used in the study. In experiment 1, when domestic ducks were inoculated intranasally with 2 H5N1 HPAI viruses, corneal opacity appeared more frequently than neurologic signs and mortality. Corneal ulceration and exophthalmos were rare …

Chandelier retroillumination-assisted torsional oscillation for cataract surgery in patients with severe corneal opacity.

We describe a technique that uses 25-gauge transconjunctival chandelier endoillumination in combination with a torsional oscillation system for cataract surgery in cases with severe bullous keratopathy. Because of the hands-free and self-retaining nature of the chandelier fiber, continuous curvilinear capsulorhexis and subsequent bimanual intraocular manipulation can be performed easily. Torsional oscillation efficiently emulsifies lens particles along with the fluidics, preventing posterior capsule rupture and endothelial cell damage. After the chandelier fiber is removed at the end of surger

Overcoming Opacity: The Role of Intraoperative OCT in Complex Corneal and Anterior Segment Surgery.

Intraoperative optical coherence tomography (iOCT) has emerged as a pivotal technology in anterior segment surgery, particularly in cases limited by corneal opacity, edema, or altered anatomy. By providing real-time, cross-sectional imaging, iOCT enables surgeons to visualize otherwise hidden structures and to perform critical intraocular maneuvers with greater precision and safety. Its integration into the surgical microscope allows continuous monitoring of tissue-instrument interaction, transforming traditionally "blind" procedures into image-guided interventions. This review highlights the …
