Streptococcus bovis causing perforating corneal ulcer.

PURPOSE: The purpose of this study was to report the first case of Streptococcus bovis causing a perforating corneal ulcer in a healthy adult. METHODS: Observational case report. RESULTS: A healthy 77-year-old man presented with rapidly progressing corneal ulcer, resulting in perforation within 4 days. Preoperative and operative cultures isolated the same organism, S. bovis, susceptible only to vancomycin and ceftriaxone. The patient underwent a successful corneal transplantation, with histopathology revealing full-thickness necrosis and scattered bacteria. CONCLUSIONS: Although a rare path

Penetrating keratoplasty for corneal perforations in fungal keratitis.

PURPOSE: To evaluate the complications and therapeutic effects of penetrating keratoplasty (PKP) in the treatment of corneal perforations in fungal keratitis. METHODS: The medical records of patients who underwent PKP for corneal perforations in fungal keratitis at Shandong Eye Institute from January 1999 to December 2004 were retrospectively reviewed. RESULTS: Fifty-two patients (52 eyes) were included in this study. Twenty eyes (38.5%) had graft rejection after primary PKP, 12 of which were medically treated, and 8 underwent secondary PKP with 4 acquiring clear grafts. Fungal infection rec…

A fibrin sealant for perforated and preperforated corneal ulcers.

Fibrin sealant is used to close perforated or preperforated corneal ulcers. In addition to the usual advantages of cyanoacrylates it is degraded physiologically and provides a good support for corneal healing. Corneal grafting can be avoided when contraindicated or postponed when conditions allow. This technique has been successful in nine eyes of eight patients.

Medical and surgical management of corneal thinnings and perforations.

The primary objective of treatment of a thin cornea is to maintain the integrity of the anterior chamber and to control the ocular inflammation. Available methods of therapy range from simple medical means, through soft contact lenses and tarsorrhaphy, to conjunctival flaps and keratoplasty. Once perforation has occurred, if the anterior chamber has not reformed in two to three days despite patching, soft contact lenses or adhesives should be tried; if not successful, the more complicated procedures such as glued-on hard contact lens, a blow-out patch graft, or penetrating keratoplasty must be

Surgical outcomes of perforated and unperforated corneal descemetocele.

OBJECTIVE: To compare the surgical outcomes of amniotic membrane transplantation (AMT) and penetrating keratoplasty (PK) in perforated and non-perforated corneal descemetocele. METHOD: In this study, we retrospectively examined 48 eyes of 48 patients operated with AMT, and 32 eyes of 32 patients operated with PK for perforated and non-perforated descemetocele. RESULTS: There were no significant differences between the two groups with regard to age, sex, laterality, follow-up duration, cause of descemetocele, and localization (p > 0.05). Thirty-six (75%) eyes in the AMT group and 17 (53.1%) e

[Corneal ulcer with hypopion in a patient with perforant keratoplasty].

After corneal transplantation, the patients' problems are far from being definitively resolved. The transplant pathology requires an attentive follow-up, as there is always a danger of corneal graft failure. We present here the case of young patient who had a corneal transplantation after an eye injury and who has developed a corneal ulcer on the grafted cornea, with subsequent risk of graft failure and consequent loss of the eye.

Corneal perforation with extrusion of lens in a burn patient.

The incidence of ophthalmologic injuries has been reported to be between 20 and 25% in all the burn patients. As most of the eye injuries are associated with severe burns, attention is often directed towards the life-threatening events and eye injuries may be overlooked and undertreated. This case report presents a patient who had 40% flame burn and had recently been transferred from another hospital. He was examined by an ophthalmologist at the referring hospital and it was documented that both of his eyes were normal. He was transferred into the Burns Unit at the Prince of Wales Hospital 8 h

[Lamellar autografting in the treatment of small corneal perforations].

BACKGROUND: The treatment of corneal perforations due to corneal ulceration is a serious challenge in countries where there are no eye banks. The author presents an original surgical procedure for treating these cases: lamellar autografting. METHODS: The corneal transplant was taken at 12 h; size was 2 to 4 mm and thickness 2/3 of the stroma. Twelve patients were treated. RESULTS: The results are satisfactory and the grafting successful; no complication of significant importance has been noted. CONCLUSION: Lamellar autografting is an effective method for treating small corneal perforations.…

Bilateral Corneal Perforation in a Patient Under Anti-PD1 Therapy.

Immune checkpoint inhibition has improved the clinical outcomes for numerous patients with cancer. However, the downside is a whole new spectrum of immune-related adverse events. We report a 68-year-old man with a history of nonsmall cell lung cancer presenting with a spontaneous corneal perforation in the right eye after 22 cycles of pembrolizumab. In addition, a chronic central nonhealing epithelial defect developed after performing a penetrating keratoplasty. Treatment with autologous serum drops resulted in complete healing of the corneal ulcer, where other conventional therapies had no ef…
