The Thai HIV/AIDS epidemic at 15 years: sustained needle sharing among southern Thai drug injectors.

To explore perceptions and attitudes towards needle sharing among clinic-based injecting drug abusers (IDUs) at a drug-treatment clinic in Hat Yai City, Songkla Province, Southern Thailand. Qualitative methods were used to gather data, including: in-depth interviews with 17 active IDUs and with three nurses, participant observation, review of the IDUs' files, and validation after interview completion to ensure data triangulation. A form of comparative content analysis, including thematic analysis, was used for data analysis. After 15 years of the Thai HIV/AIDS epidemic, most southern Thai IDUs

Evaluation of a risk-sharing agreement for atezolizumab treatment in patients with non-small cell lung cancer: a strategy to improve access in low-income countries.

BACKGROUND: Using immune checkpoint inhibitors (IO) is a promising approach to maximize clinical benefits for patients with non-small cell lung cancer (NSCLC). PD-L1 expression serves as a predictive factor for treatment outcomes with IO. However, the high cost of this treatment creates significant barriers to access. Substantial evidence demonstrates the sustained clinical benefits experienced by patients who respond to immunotherapy. While IOs show promise in NSCLC treatment, their high cost poses access barriers. AIM: This study focused on a prospective cost analysis conducted at a high-sp

Insurance Coverage for Chronic Diseases and Healthcare of Low-Income People: Evidence From Chinese Administrative Data.

Patient cost-sharing can lead to delays in necessary care, especially among low-income populations. In this study, we examine the impact of health insurance coverage for chronic disease treatments in outpatient care, using recent administrative insurance claims dataset from a low-income population in an underdeveloped city in China. Employing a propensity score matching and difference-in-differences approach, we find that outpatient coverage for certain chronic diseases increased outpatient utilization and expenses among patients with these conditions. Interestingly, these patients also increa

Evaluation of telepharmacy and the use of a gravimetric technology-assisted workflow system for remote sterile product pharmacist checks.

PURPOSE: To evaluate the impact of remote sterile product pharmacist checks when used with a gravimetric-based technology-assisted workflow (TAWF) system on product checking accuracy, pharmacist review time, workload sharing, cost savings, and staff perceptions. METHODS: A double-arm, prospective study was conducted at 4 pharmacy locations for a 90-day period. Each compounded sterile product (CSP) checked by a remote pharmacist was also checked by a local pharmacist at the site of CSP preparation. An anonymous, online survey was emailed to staff before and after implementation to evaluate per

Impact of a Novel Cost-Saving Pharmacy Program on Pregabalin Use and Health Care Costs.

BACKGROUND: Pharmacy cost-saving programs often aim to reduce costs for members and payers by encouraging use of lower-tier or generic medications and lower-cost sales channels. In 2010, a national U.S. health plan began a novel pharmacy program directed at reducing pharmacy expenditures for targeted medications, including pregabalin. The program provided multiple options to avoid higher cost sharing: use mail order pharmacy or switch to a lower-cost alternative medication via mail order or retail. Members who did not choose any option eventually paid the full retail cost of pregabalin. OBJEC…

Identifying Dual-Eligible Beneficiaries With Long-Term Services and Supports Use in Medicare Enrollment Data.

Identifying dual-eligible beneficiaries who use Medicaid-funded long-term services and supports (LTSS) is difficult, hindering efforts to monitor use and improve quality. We demonstrate a strategy that uses only Medicare data to identify nursing home (NH) users and Medicaid home-and-community-based-service (HCBS) waiver participants by leveraging the fact that these groups exclusively qualify for zero cost-sharing in Part D. In a multistate cohort of low-income older adults, we identified Part D LTSS beneficiaries (dual-eligible beneficiaries with zero Part D cost-sharing) in Medicare enrollme

Immunization programs and their costs.

The Expanded Program on Immunization (EPI) has made considerable progress towards immunizing the world's women and children, preventing 3.2 million child death episodes per year from measles, neonatal tetanus, and pertussis, as well as 440,000 cases of paralytic poliomyelitis. Vaccinations provided through the EPI are believed to be one of the most cost-effective child survival interventions at a cost between $5 and $10 per child. However, variation exists in the average cost per fully immunized child, depending upon the type of vaccine technology and delivery strategy utilized, the scale of o

Multi task learning based early prediction model for antibiotic resistance using multi institutional cohort data.

Antibiotic resistance poses a significant global health challenge, with its rapid emergence driven by inappropriate antibiotic use. This study aimed to develop and compare machine learning models to predict resistance to nine antibiotic classes in hospitalized patients, addressing the high-cost labeling challenge of medical data. We conducted a retrospective study using electronic medical records from three Korean tertiary care institutions (n = 59,551). Single-task learning models (Logistic Regression, XGBoost, LightGBM, CatBoost, Multi-Layer Perceptron) and multi-task learning models (Hard P

The Impact of Redistricting Proposals on Health Care Expenditures for Liver Transplant Candidates and Recipients.

Redistricting, which means sharing organs in novel districts developed through mathematical optimization, has been proposed to reduce pervasive geographic disparities in access to liver transplantation. The economic impact of redistricting was evaluated with two distinct data sources, Medicare claims and the University HealthSystem Consortium (UHC). We estimated total Medicare payments under (i) the current allocation system (Share 35), (ii) full regional sharing, (iii) an eight-district plan, and (iv) a four-district plan for a simulated population of patients listed for liver transplant over

Delivering a primary-level non-communicable disease programme for Syrian refugees and the host population in Jordan: a descriptive costing study.

The Syrian conflict has caused enormous displacement of a population with a high non-communicable disease (NCD) burden into surrounding countries, overwhelming health systems' NCD care capacity. Médecins sans Frontières (MSF) developed a primary-level NCD programme, serving Syrian refugees and the host population in Irbid, Jordan, to assist the response. Cost data, which are currently lacking, may support programme adaptation and system scale up of such NCD services. This descriptive costing study from the provider perspective explored financial costs of the MSF NCD programme. We estimated ann

Beyond Medicare ACOs: Pediatric Alternative Payment Models and Financial Risk-Sharing.

Pediatric clinicians are increasingly asked to participate in alternative payment models (APMs) that hold clinicians financially accountable for population-level spending. Drawing heavily on evidence from Medicare, policymakers promote accountable care organizations and other risk-sharing arrangements as pathways to lower costs and higher quality. Yet the pediatric context differs in fundamental ways that limit the direct applicability of adult models. Child health care has fewer short-term opportunities for cost savings, and small numbers of children with medical complexity drive much spendin

Why some disabled adults in Medicaid face large out-of-pocket expenses.

Using national survey data, we found that the Medicaid program effectively limits out-of-pocket health care spending for the majority of Medicaid enrollees who are disabled but are not in institutions, such as nursing homes. However, we also found that for 10 percent of these enrollees, annual out-of-pocket spending is $1,200 or higher--a significant sum for people who are low-income. Other research has shown that such cost barriers can deprive Medicaid enrollees of cost-effective care in favor of cost-ineffective care or no care at all. As state policy makers contemplate benefit reductions th

Sharing Overdose Data Across State Agencies to Inform Public Health Strategies: A Case Study.

Data sharing and analysis are important components of coordinated and cost-effective public health strategies. However, legal and policy barriers have made data from different agencies difficult to share and analyze for policy development. To address a rise in overdose deaths, Maryland used an innovative and focused approach to bring together data on overdose decedents across multiple agencies. The effort was focused on developing discrete intervention points based on information yielded on decedents' lives, such as vulnerability upon release from incarceration. Key aspects of this approach in

Tiered diagnosis and value-based healthcare: can the outpatient reimbursement policy for medical costs in retail pharmacies promote both?

BACKGROUND: Amid growing health demands and tight welfare budgets, guiding rational healthcare-seeking behavior to improve health while controlling costs has become a global challenge. This study aims to examine whether the reform of medical insurance on the cost-sharing of pharmacy can promote tiered diagnosis and value-based healthcare. METHODS: Using 2011-2018 China Health and Retirement Longitudinal Study data and the Staggered DID method, this paper estimates the effects of the Retail Pharmacies Outpatient Reimbursement (RPOR) reform in the Urban Employee Basic Medical Insurance (UEBMI).
