Breathing Through a Straw: A Case of Adult Croup at High Altitude.

Croup in adults is a rare but potentially life-threatening condition. We describe a case of croup in a 40-y-old female climber, likely precipitated by exposure to cold, dry air at high altitude. To our knowledge, this is the first documented case of croup in an adult associated with exposure to high altitude. This case highlights the importance of early recognition and timely intervention in managing this uncommon presentation in an adult of a common childhood condition.

Esophageal pathology and the aerodigestive triple endoscopy for pediatric recurrent croup.

Recurrent croup (RC) is characterized by recurrent episodes of stridor, barking cough, and hoarseness. While viral infections are the primary cause in croup, RC is thought to be caused by non-infectious etiologies such as structural airway abnormalities, allergic disorders, airway hyperresponsiveness and gastroesophageal reflux (GERD). Recent studies have suggested a potential link between esophageal diseases, particularly GERD/reflux esophagitis and eosinophilic esophagitis (EoE), and RC. This retrospective study aims to explore the association between esophageal disorders and RC through a mu…

Croup and COVID-19 in a child: a case report and literature review.

Croup (laryngotracheitis) is frequently encountered in the emergency department in a young child presenting with stridor. We describe a rare case of croup secondary to SARS-CoV-2 in an 18-month-old child who presented with stridor and respiratory distress and required urgent intubation. Subsequently, the child developed multisystem inflammatory syndrome in children (MIS-C). The child was monitored in paediatric intensive care unit. We would like to highlight that COVID-19 croup in children may be an indicator for MIS-C, and close monitoring is warranted as MIS-C is a life-threatening condition…

Predictors of Hospitalization for Children With Croup, a Population-Based Cohort Study.

OBJECTIVES: We sought to determine predictors of hospitalization for children presenting with croup to emergency departments (EDs), as well as predictors of repeat ED presentation and of hospital readmissions within 18 months of index admission. We also aimed to develop a practical tool to predict hospitalization risk upon ED presentation. METHODS: Multiple deterministically linked health administrative data sets from Ontario, Canada, were used to conduct this population-based cohort study between April 1, 2006 and March 31, 2017. Children born between April 1, 2006, and March 31, 2011, were …

Safety and utility of direct laryngoscopy and bronchoscopy in patients hospitalized with croup.

Acute croup is a common admitting diagnosis for pediatric patients. If a patient is not responding to medical management for presumed croup, the otolaryngology team is occasionally consulted for direct laryngoscopy and bronchoscopy (DLB) to rule out tracheitis or another airway pathology. We conducted a study to determine if inpatient DLB in acute croup is safe and efficacious and to correlate preoperative vital signs with intraoperative findings. We reviewed the charts of 521 patients with an admitting diagnosis of acute tracheitis, acute laryngotracheitis, or croup. Of this group, 18 patient…

A Narrowing Diagnosis: A Rare Cause of Adult Croup and Literature Review.

Croup or laryngotracheitis is rare in adults. We present a case of an otherwise healthy young woman that presented in the winter with 3 days of increasing dyspnea, cough, and fever. She was hemodynamically stable but was found to have a barking cough, paradoxical abdominal breathing, and stridor. Chest radiograph revealed subglottic narrowing. Respiratory viral nucleic acid amplification testing was positive for respiratory syncytial virus. The patient was treated with nebulized epinephrine, dexamethasone, and a helium-oxygen mixture. Stridor resolved immediately after starting the helium-oxyg…

[Acute dyspnea in childhood. Croup or foreign body--bronchiolitis or asthma?].

The diagnosis and treatment of acute respiratory distress or dyspnea in children needs to consider the age-related major causes, but also the rare causes. In neonates, congenital abnormalities, immaturity of the lungs, perinatal complications and infections are major causes, in infants bronchiolitis/obstructive bronchitis and croup, while in older children and adolescents acute asthmatic attacks and pneumonia predominate. Of critical importance is a clear diagnostic classification, assessment of severity, securement of adequate oxygenation and a carefully taken decision for hospital or ambulat…

The cost-effective use of nebulized racemic epinephrine in the treatment of croup.

Recent studies have shown that discharging to home an emergency department (ED) patient with croup if the patient is clinically stable 3 to 4 hours after being treated with nebulized racemic epinephrine (NRE) is safe and cost-effective. The objective of this study was to determine if EDs in our geographic area are using NRE cost-effectively in the management of croup. A survey was mailed to the ED medical directors of 23 hospitals in Ohio, Kentucky, and Indiana within a 150-mile radius of our teaching/referral children's hospital. All the hospitals surveyed were community hospitals with EDs an…

Is admission to hospital with stridor and acute viral croup influenced by time?

Study of the records of 100 children admitted sequentially to hospital with viral croup suggested that they were more likely to be admitted at night than during the day when compared with other common conditions. The actual day of the week was unimportant, suggesting that medical availability was not a factor.

Risk factors associated with severe viral croup in hospitalised Malaysian children.

OBJECTIVE: To determine the risk factors associated with severe viral croup in hospitalised Malaysian children. METHODOLOGY: The medical records of children aged less than five years admitted with a diagnosis of viral croup between 1994 and 1999 were reviewed. Severe viral croup was diagnosed in children who had stridor at rest with marked recession associated with central cyanosis or altered level of consciousness. Multivariate logistic regression analysis was performed to identify risk factors associated with severe viral croup. RESULTS: Eighteen (14.7%) of 122 children with viral croup we…

Outpatient treatment of moderate croup with dexamethasone: intramuscular versus oral dosing.

OBJECTIVE: Steroid use for the treatment of croup has been supported by several studies, although few have addressed the use of oral dexamethasone for outpatient management. The efficacy of oral (PO) versus intramuscular (IM) dosing of dexamethasone in the outpatient treatment of moderate croup are compared in this study. METHODS: Patients between the ages of 3 months and 12 years with moderate croup (history or presence of stridor, cyanosis, or retractions) were eligible for enrollment in this single-blind, prospective study. Patients were randomized to receive a single dose (0.6 mg/kg, maxi…

The Relationship between Croup and Gastroesophageal Reflux: A Systematic Review and Meta-Analysis.

OBJECTIVE: The mechanism by which recurrent croup occurs is unknown. Gastroesophageal reflux is commonly implicated, although this relationship is only loosely documented. We conducted a systematic review with a meta-analysis component to evaluate the relationship between recurrent croup and gastroesophageal reflux disease (GERD), and to assess for evidence of improvement in croup symptoms when treated. STYLE DESIGN: Systematic Review and Meta Analysis. METHODS: We searched five separate databases. Studies were included if they discussed the relationship between croup and GERD in children, >…

Management of pediatric laryngeal webs secondary to severe croup and traumatic intubation.

Laryngotracheobronchitis is a common, typically self-limiting viral infection. However, severe laryngotracheobronchitis can require urgent intubation to prevent imminent airway obstruction. The concurrent inflammation and urgency make laryngeal trauma more likely. We report two cases of children who underwent emergent intubation for acute respiratory distress due to viral laryngotracheobronchitis and subsequently developed anterior laryngeal webs. Both underwent laryngoplasty with keel placement, with resolution of their laryngeal webs. These cases describe pediatric laryngeal web formation as

Multiple pulmonary abscesses caused by Legionella pneumophila infection in an infant with croup.

Legionella infections are uncommon in childhood. We report the case of an infant who developed multiple pulmonary abscesses caused by Legionella pneumophila after receiving prolonged corticosteroid treatment of severe croup. Diagnosis was not suspected because immunosuppression was not initially considered. Caution should be used when prolonging high-dose corticosteroids in children with respiratory diseases.
