A Rare Case of Deglutition-Induced Atrial Fibrillation Secondary to Esophageal Dilation.

Atrial fibrillation (AF) is the most common arrhythmia in the world. However, deglutition-induced tachyarrhythmias are exceptionally rare. Diagnosis relies on a documented history, Holter monitoring and echocardiograms. The mechanism underlying deglutition-induced tachycardia remains uncertain, with leading hypotheses suggesting mechanical left atrial stimulation after esophageal distention or activation of the vagus nerve due to increased intra-esophageal pressure. Lifestyle changes, medications (e.g., beta-blockers and antiarrhythmics), and radiofrequency catheter ablation are viable treatme

Cross-System Integration of Respiration and Deglutition: Function, Treatment, and Future Directions.

Swallowing occurs preferentially in the expiratory phase of the quiet breathing cycle and at mid-to-low tidal volume. This coordinative pattern imparts important biomechanical advantages to swallowing and airway protection and facilitate laryngeal elevation, laryngeal vestibular and vocal fold closure, and cricopharyngeal sphincter opening. This preferred coordinative relationship between breathing and swallowing is impaired in a variety of patient populations, including head and neck cancer survivors with dysphagia. We developed a training protocol to re-establish more optimal phasing of swal

Cervicofacial and Pharyngolaryngeal Lymphedema and Deglutition After Head and Neck Cancer Treatment.

One of the sequelae of head and neck cancer treatment is secondary lymphedema, with important impact on breathing, swallowing and vocal functions. The aim of the study was to assess the presence, staging characteristics and relationship of external and internal lymphedema and dysphagia after head and neck cancer treatment. The MDACC Lymphedema Rating Scale in Head and Neck Cancer was employed for the assessment and staging of face and neck lymphedema; the Radiotherapy Edema Scale for internal lymphedema; and a fiberoptic endoscopic evaluation of swallowing (FEES) for swallowing. The sample con

[Dysphagia caused by neurogenic deglutition disorders and diffuse idiopathic skeletal hyperostosis (DISH)].

BACKGROUND: Diffuse idiopathic skeletal hyperostosis (DISH) may lead to dysphagia caused by osteophytes of the cervical spine. Osteophytes can be resected transorally or transcervically, but operative ablation should not be indicated generously because of the threat of severe complications. PATIENT: A fifty-year-old man with dysphagia and loss of weight of 15 kg in the last three months is presented. He also suffered from a brain damage during infancy which caused grand-mal-seizures. One seizure lead to cardiac arrest which required cardio-pulmonary resuscitation and subsequent tracheostomy.

The "FEEDS (FEeding Eating Deglutition Skills)" over Time Study in Cardiofaciocutaneous Syndrome.

Feeding, eating and deglutition difficulties are key concerns in patients with cardiofaciocutaneous syndrome (CFCS). This study intends to quantify the development of feeding skills from birth to adulthood in patients with CFCS. Twenty-seven patients (eight males; mean age: 16.7 ± 8.3 years; median age: 15 years, age range: 1.5-38 years) with molecularly confirmed clinical diagnosis of CFCS were prospectively recruited from the Rare Disease Unit, Paediatrics Department, Fondazione Policlinico Agostino Gemelli-IRCCS, Rome, Italy, over a one-year period. Pathogenic variants along with key inform

Speech, deglutition and life quality after intraoral tumour resection. A prospective study.

The aim of the present prospective study was to evaluate the functional sequelae and postoperative development of quality of life after intraoral tumour surgery in 40 consecutive patients with a squamous cell carcinoma of the floor of the mouth. Immediate reconstruction of intraoral soft tissues after tumour resection was accomplished by local flaps (cheek and tongue) (n=27) and by revascularized transfer of small bowel grafts (n=5) and myocutaneous/fasciocutaneous flaps (n=8). Mobility of the tongue was assessed by ultrasound. Quality of speech was analysed using the Freiburg speech intelligi

Coordination between respiration and deglutition in a preterm infant mammal, Sus scrofa.

In adult mammals, the path of a swallowed bolus of solid food crosses the laryngeal opening, so that coordination between respiration and deglutition is critical for airway protection. The nature of such coordination in preterm, low-birth-weight infants with immature nervous systems, is not clear. Using preterm pigs as a model, two measures of respiration were recorded and then coordinated with a high-resolution cineradiographic record of swallowing. Swallows, divided into three distinct events, began before inhalation ended, but expiration did not start until after the milk had passed around

Laryngeal reflex mechanism during deglutition--observation of subglottal pressure and afferent discharge.

In this investigation, particular attention was paid to elucidate the laryngeal reflex mechanism of protective closure and the sensory function of the larynx during deglutition. For this purpose, three different experimental procedures were adopted: (1) subglottal pressure of felines was measured during deglutition using a pressure transducer; (2) subglottal pressure of human beings was measured during deglutition using a pressure transducer; and (3) afferent discharges from superior and recurrent laryngeal nerves of felines were recorded. The following conclusions appear justified. (1) Feline

Involvement of sensory input from anterior teeth in deglutitive tongue function.

Swallowing is a reflex that receives sensory information from the peripheral nerves and from the cerebral cortex. The aim of the present study was to investigate whether the sensory input from anterior teeth affects the functional characteristics of tongue pressure applied against the hard palate during swallowing. Subjects were eight healthy volunteers. Tongue pressure against the hard palate during swallowing 10 ml of water was measured under two conditions: preanesthesia and postanesthesia of anterior teeth. The sensory deprivation of anterior teeth was performed by periodontal anesthesia.

A cineradiographic study of deglutitive tongue movement in patients with anterior open bite.

The purpose of this study was to use cineradiographic images to investigate tongue movement during deglutition in anterior open bite patients with tongue thrust. Each subject had semi-spherical lead markers attached to the tip and dorsal surface of the tongue and was asked to swallow 5 ml of diluted liquid barium. Tongue movement during deglutition was recorded in the mid-sagittal plane with an X-ray VTR system. The deglutition process was divided into 6 stages to analyze the movements of the tip and dorsal surface of the tongue in each stage. In open bite patients, both the tip and dorsum of

Severe craniofacial malformations and deglutition dysfunction in a brother and sister: new syndrome?

We report seemingly unique craniofacial malformations and deglutition dysfunction in a sib pair. The boy had right maxillomandibular alveolar synechae, ankylosis of right temporomandibular joint, hypoplasia of the zygomatico-maxillary region, nasal deviation to the left, choanal stenosis, and exophthalmos due to shallow orbita. His ears were apparently low-set with prominent lobules. He had severe gastroesophageal reflux and increasing respiratory problems and died at age 11 months. Psychomotor development was normal. His 10-year-old sister had similar craniofacial malformations and a cleft so

Piecemeal deglutition and dysphagia limit in normal subjects and in patients with swallowing disorders.

OBJECTIVE: Before the advanced evaluation of deglutition and selection of a treatment method, objective screening methods are necessary for patients with dysphagia. In this study a new electroclinical test was established to evaluate patients with dysphagia. METHODS: This test is based on determining piecemeal deglutition; which is a physiological phenomenon occurring when a bolus of a large volume is divided into two or more parts which are swallowed successively. The combined electrophysiological and mechanical method used to record laryngeal movements detected by a piezoelectric transducer

[The role of the hyoid bone in the act of deglutition after horizontal epiglottic laryngectomy].

Results of ++roentgeno-cinematographic studies of the deglutition act in patients after horizontal supraglottic laryngectomy are presented. The analysis of the material has been expected to provide an answer to what an extent the removal or the preservation of hyoid bone does influence the disorders that involve the swallowing. The entire elaborated material comprises 51 deglutition acts, having been divided into 3 groups. In the I group of studied subjects it was observed that there was a more sizable shortening of the pharynx, as well as a greater upward and forward movement of the remaining

Role of fetal deglutition and micturition in the production and turnover of amniotic fluid in the monkey.

In order to determine the precise role played by the absence of fetal swallowing or urine contribution in the disposal and production of amniotic fluid, we surgically occluded the fetal esophagus in 5 monkeys and the urethra in 2 monkeys. The amniotic fluid volume was then measured daily with an indicator dilution technic. Sham operations were performed on 3 monkeys of similar gestational age. It was found that in late pregnancy failure of fetal deglutition resulted in transient hydramnios which appeared to be followed by a readjustment in water exchange rates in other pathways by which the am

The physiological experiments of Constantin von Economo on the central pathways of mastication and deglutition.

The first study of Constantin von Economo on the mammalian brain was published in 1902. Experiments were carried out in rabbits at the Physiological Institute headed by Siegmund von Exner-Ewarten in Vienna to investigate the central pathways of chewing and swallowing. After placing cortical lesions, Economo applied cortical and subcortical electrical stimulation to obtain masticatory movements, and tracked degenerated fibers by means of the Marchi method. He traced fibers through the internal capsule, ventral nucleus of the thalamus, subthalamic nucleus, substantia nigra and its connections wi

Alleviating speech and deglutition: Role of a prosthodontist in multidisciplinary management of velopharyngeal insufficiency.

Surgical resection of soft palate due to cancer affects the effective functioning of the velopharyngeal mechanism (speech and deglutition). With the loss of speech intelligibility, hyper resonance in voice and impaired function of swallowing (due to nasal regurgitation), there is a depreciation in the quality of life of such an individual. In a multidisciplinary setup, the role of a prosthodontist has been described to rehabilitate such patients by fabrication of speech aid prosthesis. The design and method of fabrication of the prosthesis are simple and easy to perform. The use of prosthesis,
