Oral rehydration therapy of severe diarrheal dehydration.

In 1980, 104 infants with seven to 15 percent dehydration due to severe diarrhea and vomiting were hospitalized in Tehran and treated in two separate phases, deficit therapy and maintenance therapy, using two isotonic oral solutions. For deficit therapy, solution A (sodium 80, potassium 20 mmol/l) was administered at a rate of 40 ml/kg per hour until all signs of dehydration disappeared. For maintenance therapy, solution B (sodium 40, potassium 30 mmol/l) was given sip by sip at a rate of about 250 ml/kg per 24 hours until diarrhea stopped. Intravenous fluids were not used, even in severe dehy…

Voluntary dehydration in men working in heat.

An investigation was carried out on 77 steelworkers, divided into two groups, at their work places. Group I (30 subjects) drank ad lib. a sp … pectively. When the intake period of the drinks with salts was extended it was found that voluntary dehydration decreased and amounted to 10% of sweat rate on the last day of the study. The voluntary dehydration in group I was lower by 40%, the difference being highly significant in comparison with group II (P < 0.001). Data obtained from anamnesis and a ques …

Peripheral gangrene in hypernatraemic dehydration of infancy.

Gangrene of the extremities complicating diarrhoea and severe hypernatraemic dehydration occurred in 6 infants. This is a rare complication of gastroenteritis, and its association with hypernatraemia does not seem to have previously been emphasized. The increased blood viscosity resulting from serum hyperosmolarity may have been responsible for the gangrene, and studies in our patients suggested that disseminated intravascular coagulation was present. In addition to fluid and electrolyte replacement, the infants were treated with heparin with some recovery of the affected extremities.

[Beyond thirst: dehydration in the palliative phase].

Dehydration in patients in the final stage of life requires a multidimensional approach. The appropriateness of a particular intervention depends not only on clinical factors and medical-technical possibilities, but also on ethical, spiritual, and psychosocial considerations. These aspects all contribute to the decision whether or not to initiate diagnostic procedures or treatment for an individual patient. This clinical lesson presents a fictional case to illustrate the key considerations involved in decision-making, as outlined in the new guideline Dehydration in the Palliative Phase.

Vaping-Induced Proteolysis Causes Airway Surface Dehydration.

Proteases such as neutrophil elastase cleave and activate the epithelial sodium channel (ENaC), causing airway dehydration. Our current study explores the impact of increased protease levels in vapers' airways on ENaC activity and airway dehydration. Human bronchial epithelial cultures (HBECs) were exposed to bronchoalveolar lavage fluid (BALF) from non-smokers, smokers and vapers. Airway surface liquid (ASL) height was measured by confocal microscopy as a marker of hydration. ENaC cleavage was measured by Western blotting. Human peripheral blood neutrophils were treated with a menthol-flavore…

Dehydration in geriatrics: consequences and practical guidelines.

PURPOSE OF REVIEW: Geriatric dehydration is a widespread and under recognized problem. The purpose of this review was to examine the latest evidence regarding geriatric dehydration and provide practical guidance for health professionals. RECENT FINDINGS: This review covers evidence from the past 2 years and shows that geriatric dehydration is not benign and is associated with significant personal distress, as well as negative economic and health system consequences. New guidance on nutrition and hydration in the elderly recommend against the use of skin turgor, dry mouth, urine colour or spec…

Could neonatal hypernatremia dehydration influence hearing status?

INTRODUCTION: Neonatal hypernatremia dehydration (NHD) is a dangerous condition in neonates, which is accompanied by acute complications (renal failure, cerebral edema, and cerebral hemorrhage) and chronic complications (developmental delay). Children begin learning language from birth, and hearing impairment interferes with this process. We assessed the hearing status of infants with hypernatremia dehydration. MATERIALS AND METHODS: In a case-control study in 110 infants presenting at the Ghaem Hospital (Mashhad, Iran) between 2007 and 2011, we examined the incidence of hearing impairment in…

Physiologic basis for understanding quantitative dehydration assessment.

Dehydration (body water deficit) is a physiologic state that can have profound implications for human health and performance. Unfortunately, dehydration can be difficult to assess, and there is no single, universal gold standard for decision making. In this article, we review the physiologic basis for understanding quantitative dehydration assessment. We highlight how phenomenologic interpretations of dehydration depend critically on the type (dehydration compared with volume depletion) and magnitude (moderate compared with severe) of dehydration, which in turn influence the osmotic (plasma os…

Five-year neurodevelopmental outcome of neonatal dehydration.

OBJECTIVE: To determine the long-term outcome of neonatal dehydration. STUDY DESIGN: We identified 182 newborns who were rehospitalized with dehydration (weight loss > or =12% of birth weight and/or serum sodium > or =150 mEq/L) and 419 randomly selected controls from a cohort of 106,627 term and near-term infants with birth weight > or =2000 g born between 1995 and 1998 in northern California Kaiser Permanente hospitals. Outcomes data were obtained from electronic records, interviews, questionnaire responses, and neurodevelopmental evaluations performed in a masked fashion. RESULTS: Follow-…

[Dehydration due to gastro-enteritis in children].

Gastroenteritis is the commonest cause of dehydration in children. Infants and young children dehydrate more easily than adults if fluid intake is insufficient or fluid loss too high because of the combination of a large extracellular volume, a large insensible loss and a mediocre concentrating capacity of the kidney. Fluid loss due to gastroenteritis is often accompanied by electrolyte and acid-base disturbances. Oral rehydration with oral rehydration salts (ORS) is nearly always possible. Re-evaluation after 6 hours is advised especially in young children. Early (< 6-24 hours) resumption of …

Severe hypernatremic dehydration in a newborn infant.

A female infant presented with severe hypernatremic dehydration and failure to thrive two weeks after birth. The diagnosis and management of the infant are described, highlighting the need for follow-up and support after early discharge of newborns.

Dehydration deaths in infants and young children.

Dehydration in developed countries is an uncommon but important mechanism resulting in the death of infants and children. The clinicopathological features of a series of 37 fatal dehydration cases autopsied at the Adelaide Children's Hospital over a 33-year period (1961-1993) are presented. Causative factors for dehydration included gastroenteritis (21 cases), gastroenteritis with high environmental temperature (one case), high environmental temperatures (six cases), neglect/failure to thrive (four cases), mental retardation/chromosomal abnormality (three cases), congenital adrenal hyperplasia…

[Therapeutic guides in hypertonic dehydration (author's transl)].

Physiopathology of hypertonic dehydration is revised. The bases of treatment are discussed; they are based on: 1. Basal requirements of fluids. 2. Slow correction of deficit. 3. Replacement of maintaining losses. Total fluids support will carry to rehydration in 2 or 3 days. In the most severe cases peritoneal dialysis is associated. The indications, technique and realization of peritoneal dialysis during hypertonic dehydration are discussed.
