Itch in Atopic Dermatitis Management.

Patients with atopic dermatitis (AD) suffer from chronic inflammatory dermatitis and antihistamine-resistant itch. The management of intractable pruritus in AD is important, requiring the development of new therapeutic approaches. At present, the standard treatments for AD include topical anti-inflammatory drugs such as calcineurin inhibitors and corticosteroids. Topical emollient treatment is recommended to moisten the skin and to restore and maintain barrier function. Phototherapy is also effective in reducing the number of epidermal nerve fibers, normalizing imbalances in the levels of expr

Flexural eczema versus atopic dermatitis.

Flexural eczema and atopic dermatitis are frequently synonymized. As respiratory atopy is rarely tested for and found in these patients, systematically equating a flexural distribution of dermatitis with atopic dermatitis may too frequently result in misclassified diagnoses and potentially missed opportunity for intervention toward improving patients' symptoms and quality of life. We present a critical review of the available evidence for the atopic dermatitis diagnosis and discuss the similarities between atopic dermatitis and allergic contact dermatitis. Because neither flexural predilection

Topical tacrolimus for atopic dermatitis.

BACKGROUND: Atopic dermatitis (AD) (or atopic eczema) is a chronic inflammatory skin condition that affects children and adults and has an important impact on quality of life. Topical corticosteroids (TCS) are the first-line therapy for this condition; however, they can be associated with significant adverse effects when used chronically. Tacrolimus ointment (in its 2 manufactured strengths of 0.1% and 0.03%) might be an alternative treatment. Tacrolimus, together with pimecrolimus, are drugs called topical calcineurin inhibitors (TCIs). OBJECTIVES: To assess the efficacy and safety of topica

Atopic dermatitis phenotypes in childhood.

BACKGROUND: Atopic dermatitis (AD) is a chronic inflammatory skin disease and can be the first step of the atopic march. OBJECTIVE: In this retrospective study, we analysed the immunological and clinical patterns of AD in a group of children affected by the disease since their first years of life, in order to evaluate if and how these patterns can change over time, and to identify biomarkers that can possibly correlate with the clinical phenotype. METHODS: We enrolled Caucasian children with diagnosis of AD performed by a specialist on the basis of Hanifin and Rajka's criteria and with a fir

Contact dermatitis in atopic individuals.

PURPOSE OF REVIEW: Both atopy and diseases from the spectrum of dermatitis and eczema are among the most frequent clinical problems worldwide; nevertheless, they are still poorly defined and too frequently misdiagnosed. In the present review, studies pertinent to this topic were systematized and critically assessed with particular attention to definitions of relevant diseases. RECENT FINDINGS: The overall message from the research done to date is that various types of dermatitis frequently coexist. Atopy and contact allergy seem independent, while there is insufficient data to state upon the

Innate immunity in atopic dermatitis.

Atopic dermatitis (AD) is a clinically defined, highly pruritic, chronic inflammatory skin disease. In AD patients, the combination of a genetic predisposition for skin barrier dysfunction and dysfunctional innate and adaptive immune responses leads to a higher frequency of bacterial and viral skin infections. The innate immune system quickly mobilizes an unspecific, standardized first-line defense against different pathogens. Defects in this system lead to barrier dysfunction which results in increased protein allergen penetration through the epidermis and predisposes to secondary skin infect

Inpatient management of atopic dermatitis.

Atopic dermatitis (AD) is a common chronic inflammatory skin disease that is generally managed on an outpatient basis. However, a significant percentage of patients may develop complications severe enough to require inpatient treatment. The most common complications of AD that may require hospital admission include erythroderma, eczema herpeticum, and systemic bacterial infection. Hospital admission can also be useful for chronic and severe disease that has not responded to standard therapy or in situations where nonadherence is suspected as the cause of treatment failure. In these cases, inpa

Immunologic aspects of atopic dermatitis.

Atopic dermatitis/eczema is a chronically relapsing, pruritic, and inflammatory skin disease. The term "atopic eczema/dermatitis syndrome" covers the "extrinsic" and "intrinsic" atopic dermatitis. The pathogenesis of atopic dermatitis includes complex interaction between the genetic background, skin barrier defects, abnormalities in innate and adaptive immunity, abnormalities of humoral and cellular immunity, and environmental influences. Understanding the immunopathogenesis of atopic dermatitis leads to new diagnostic and therapeutic approaches. The targets in atopic dermatitis are innate imm

Atopic dermatitis and cancer risk.

BACKGROUND: Epidemiological studies have provided growing evidence of a link between atopy and cancer risk. OBJECTIVES: To review the evidence from case-control studies and cohort studies on a possible association between atopic dermatitis (AD) and cancer risk, with particular attention to the case definition of AD. METHODS: Studies with quantitative data on the association between AD (eczematous disease) and cancer risk were obtained from MEDLINE in combination with a review of cited references. RESULTS: In 23 publications, AD was implicated in the risk of haematological [childhood leukaem

[Systemic therapy of atopic dermatitis].

The optimal treatment of atopic dermatitis requires regular medical supervision. The course of this chronic skin disease is influenced by multiple triggers which are relevant for the treatment. The mainstays of topical therapy include regular use of emollients coupled with antimicrobial substances, corticosteroids and immune modulators as required. Ultraviolet radiation and immunosuppressive regimens represent further options for the treatment of severe exacerbations and may lead to long term improvement. Data from experimental studies provide insight into possible future treatment methods.

Clinical aspects of atopic dermatitis.

Atopic dermatitis (AD), or atopic eczema, is the most common, chronic inflammatory disease among children in industrialized countries. We still lack knowledge of its pathophysiology and in particular the role of allergy as both an eliciting factor for disease expression and for disease activity. This article describes the clinical symptoms of the disease and its qualitatively different aspects. AD cannot be understood as being induced by one factor only, e.g. allergy, and this is important when planning treatment strategies. It is also important to realize the very wide range of disease intens

[Ocular complications of atopic dermatitis].

We examined the frequency and types of ocular complications that developed in 560 eyes of 280 patients with atopic dermatitis at the Department of Dermatology, Tokyo Medical College Hospital. Mild ocular involvement included blepharitis in 294 eyes (52.5%), conjunctivitis in 221 eyes (39.5%), and superficial punctate keratopathy in 65 eyes (11.6%). More serious ocular complications leading to decreased visual function occurred in 133 eyes (23.8%) that developed lens opacities, and 34 eyes (6.0%) that developed retinal break or retinal detachment. Because of the presence of abnormalities in the

[From atopic dermatitis to asthma].

Atopic dermatitis (AD) is the most common chronic skin disorder in infancy and childhood and is the main hallmark of atopic constitution. The disease is multifactorial, and although genetic predisposition is certainly a prerequisite, a number of environmental factors modulate the phenotypic expression of AD. The majority of affected children shows IgE sensitisation towards a large variety of foods and aeroallergens. Since at least 1600, it has been recognized that patients with AD have a high predisposition to develop asthma. Recent epidemiological studies show that AD is commonly seen in indi

Candida albicans and atopic dermatitis.

The role of sensitization and exposure to Candida albicans in atopic dermatitis (AD) was studied with skin-prick tests, yeast cultures and immunoblotting in 156 young adults with AD attending the Department of Dermatology, University of Turku, during 1983-89. Eighteen patients with allergic rhinitis without eczema and 39 non-atopics were included as controls. Parameters associated with severe AD were simultaneous anti-C. albicans IgE and saprophytic C. albicans growth. A statistically significant correlation between C. albicans sensitization (specific IgE antibodies) and AD symptoms was observ

Atopic dermatitis: recent therapeutic advances.

New treatments were recently proposed for the management of severe atopic dermatitis (AD). They all act on some component of the immune reaction. Oral cyclosporine reduces the number of CD4+ cells, the secretion of interleukins, and the function of Langerhans cells. Although the action of oral cyclosporine at moderately high dosages is regular and rapid, the risk of serious side effects and the reappearance of progressive disease after stopping treatment limits the use of this drug in AD. Thymic hormone extracts in patients with severe AD affect the deficit of cellular immunity. gamma-Interfer

Viral infections in atopic dermatitis.

In a study of almost 1000 patients with past or present atopic dermatitis (AD) it was found that histories of recurrent (greater than 5 episodes/year) cold sores and upper respiratory infections, as well as histories of zoster were significantly more common in AD patients than in non-atopic controls. Serological studies revealed that AD patients have clearly elevated titers of antibodies against Epstein-Barr virus. These findings suggest that the increased susceptibility to viral infections in AD is due to immune dysfunction rather than to cutaneous alterations which are associated with the di
