Athletes face numerous hazards in their daily activities. An athlete's skin, in particular, endures repeated exposure to trauma, heat, moisture, and numerous allergens and chemicals. These factors combine with other unique and less well-defined genetically predisposing factors in the athlete's skin to cause both allergic contact dermatitis (ACD) and irritant contact dermatitis (ICD). As with other cases of contact dermatitis, these eruptions in athletes present as a spectrum of acute to subacute to chronic dermatitis. Recognizing the unique environmental irritants and allergens encountered by

Metals are in close contact with skin and mucous membranes on a repeated, if not constant, basis. Nickel and mercury, well-recognized causes of contact dermatitis; gold and palladium, recently gaining acceptance as patch test allergens on standard screening trays; and cobalt are reviewed in this article. Sensitization to nickel, the most frequently identified allergen on patch testing, is associated with ear piercing. Contact with this potential allergen is ubiquitous. Mercury may be encountered as organic mercury in thimerosal, used as an antiseptic and a preservative in topical medications a

Proflavine allergy is uncommon, occurring in approximately 6% of patients attending contact dermatitis clinics. Proflavine wool is used by many surgeons in the UK as a dressing that can be moulded to conform to the contours of a corrected prominent ear. It may have bacteriostatic properties. We present a case where contact dermatitis in response to proflavine developed after pinnaplasty. This caused diagnostic confusion, a lengthened hospital stay and an unsightly hypertrophic scar.

A complete work-up of most patients with cosmetic allergies can be performed satisfactorily in a dermatologist's office without expensive equipment. After taking a complete history and performing a thorough examination, a biopsy is seldom required. Examine all the patient's cosmetics, eliminate the strong irritants, and proceed with product patch testing. Patch testing with the AAD allergen test tray at the same time is frequently helpful. When results are doubtful or do not fit with the information obtained from the history and examination, supplemental provocative use tests often clarify wha

The author describes two patients with acute contact allergy to 3-(aminomethyl)-pyridyl salicylate in an analgesic spray. In one case, the patient reacted when someone used the spray not far from her.

125 children under the age of 12 years were patch tested over a period of 7 years. 60 (48%) of the children had 1 or more positive (+ve) reactions, of which 92% (55/60) were considered relevant. The most common allergens were metals (35 + ves), fragrances (24 + ves) and rubber compounds (11 + ves). 40 of the children were also tested for contact urticaria against food additives and fragrances, of whom 20 were positive (benzoic acid 14, cinnamaldehyde 12).

18 cases of hand dermatitis in hairdressers seen over a 5-year period are reviewed. The diagnoses in these patients are discussed with reference to other studies of hand dermatitis in hairdressers. Contact allergy due to paraphenylenediamine and related hair dyes was the presenting complaint in younger hairdressers, while formaldehyde allergy occurred in those who were older. The prognosis in the former group of workers with respect to continued employment in the trade tended to be poorer than the latter. Follow-up revealed that hand dermatitis often resulted in the worker not continuing to wo

52 (5.4%) of 1063 eczema patients showed a positive reaction to chlorhexidine gluconate 1% aq. and/or 1% pet. on patch testing. At retesting, 21/29 (72%) were still positive and the results of use-test, anamnesis and supplementary testing indicated allergic sensitization in about 50% of these patients. Patients with leg eczema/leg ulcer seem to be particularly at risk.
