Dermatosurgery has become ever more popular and important in recent years, mostly due to the increasing prevalence of skin malignancies. It also encompasses a wide variety of methods to remove or modify skin tissue for numerous cosmetic reasons. Nowadays, many dermatologists provide complete dermatologic care for their patients, including surgery. Therefore, it is important to be aware of the possible complications and to be able to manage them properly. Complications in cutaneous surgery are not very often, but they can be serious and worrisome including bleeding, infections, allergic reactio

There is a limited number of options in vitiligo treatment, with the disease frequently refractory to all existing treatment modalities. This warrants development of novel and improving existing vitiligo treatments as well as finding predicting factors to improve treatment outcome through appropriate selection and the most efficient application of a treatment. These issues are addressed in clinical studies aiming to evaluate safety and efficiency of novel treatments, improvements and modifications introduced to existing treatments, and to define predictors of treatment efficiency and their lim

BACKGROUND: Antibiotic prophylaxis is frequently used for the prevention of infection at a distant site such as the heart valve or prosthetic joint. However, there are no published data describing how dermatologists manage patients "at risk".
OBJECTIVE: The purpose of this study is to document prophylactic antibiotic use by dermatologists.
METHODS: A total of 211 randomly selected community dermatologists and 69 academic dermatologic surgeons were surveyed by questionnaire to determine whether they would provide antibiotic prophylaxis in 20 different clinical situations.
RESULTS: This surve

Comprehensive training in nail surgery is usually difficult to provide as part of clinical training and continuing education due to a lack of sufficient case numbers. The Dermatology Clinic and Skin Tumor Center at Ludwigshafen Hospital has been a surgical observation and training center of the German Society for Dermatologic surgery (DGDC) since 2014 and, under the direction of Dr. Christoph Löser, is counteracting this deficiency with observation seminars, including the topic of nail surgery. As part of this one-day work shadowing program, six participants from German-speaking countries were

Dermatologists face a wide range of pain control challenges in daily practice, from the transient pain of dermatologic surgery to the persistent pain that plagues some chronic dermatologic conditions. Although the pathophysiology of pain is well described and the profound impact of dermatological pain on patients' quality of life is well appreciated, there is an identified need for clear therapeutic plans for providing symptomatic pain relief of common painful dermatoses. In this paper, we will review and clearly outline approaches to pain management of a number of common painful dermatoses su

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic painful skin disease that severely impairs patients' quality of life. While high-quality trials of HS therapies remain limited, medical knowledge of best treatment practices is rapidly evolving, leading to the recent publication of multiple international treatment guidelines for HS.
SUMMARY: This review compares international HS treatment guidelines, describes evidence for effectiveness of common and emerging HS therapies, and provides guidance for integrating evidence-based HS care into practice. Although over 50 medical and procedural t

Hidradenitis suppurativa (HS) is an inflammatory follicular dermatological condition that typically affects the intertriginous and anogenital regions of the apocrine gland-bearing skin. The management of this chronic and recurring disease necessitates a combination of lifestyle changes, medication, and surgical approaches to achieve the best possible outcomes. While medical treatments are recommended for this multimodal disease, surgical therapy, which is the gold standard of treatment for HS, has proven to be the most effective treatment because it provides long-lasting local disease control,

INTRODUCTION AND OBJECTIVE: Continent cutaneous diversion is an appealing option when orthotopic diversion is not feasible and continence preservation is desired. We describe the surgical technique of creating an ileal w-neobladder with a continent catheterizable channel using the extramural serous lined (Mansoura) principle in a bladder exstrophy patient.
MATERIALS AND METHODS: A 9-year-old boy born with classic bladder exstrophy, underwent cystectomy and a sigma rectosigmoid pouch after multiple unsuccessful attempts of exstrophy reconstruction. Colonic diversion was complicated by refracto

Using a series of 20 dissections and two anatomic transverse sections of a lower limb, the authors investigated the lateral approaches to the popliteal artery. The high lateral approach (above the knee) is not very aggressive and gives access to the retro-genicular part of the popliteal artery. After cutaneous and fascial incision, a simple gap between the vastus lateralis and biceps femoris mm. allows easy exposure of the popliteal vessels after backward retraction of the sciatic nerve. The low lateral approach to the artery (below the knee) is very aggressive for the vessels, nerves, and lig

INTRODUCTION: Identifying risk factors for wound infection may guide clinical practice for optimal use of perioperative antibiotic prophylaxis in dermatologic surgery.
OBJECTIVE: To summarize the current evidence whether specific body sites have higher risks for surgical site infections (SSI).
METHODS: The systematic literature search included MEDLINE, Embase, CENTRAL, and trial registers. Only observational studies qualified for inclusion and meta-analysis. We assessed the risk of bias according to the Newcastle-Ottawa Scale.
RESULTS: Eighteen studies with 33,086 surgical wounds were eligi

The repair of the weight bearing zones of the foot is a difficult problem for which an ideal solution does not exist. The contribution of pedicled flaps and free flaps allows it nevertheless to obtain reconstructions of good quality, which are however prone to mechanical complications. The pedicled flaps most commonly used are the instep island flap, the lateral supra-malleolar flap and the cross-leg flap. Among the free flaps figure the latissimus dorsi flap, the forearm flap and the free scapular flap. The sensitivity of the flap does not seem to be a decisive advantage if the receptor regio

Prevention of postoperative wound infection in dermatologic surgery and appropriate use of antibiotics to prevent endocarditis and joint-replacement infections are controversial issues. Dermatologists often may misunderstand the use of antibiotics to prevent endocarditis, surgical site infections, and prosthesis infections. In order to prevent endocarditis associated with surgical procedures, the American Heart Association (AHA) has developed clinical practice guidelines that apply to surgical patients with prosthetic cardiac valves, previous bacterial endocarditis, mitral valve prolapse with

BACKGROUND: With Mohs micrographic surgery being a common outpatient procedure, it is important to establish the safety of outpatient large flap surgery and investigate factors associated with complications such as infection, necrosis, and bleeding.
OBJECTIVE: This study aimed to assess the incidence of postoperative complications such as infection, bleeding, necrosis, dehiscence, and hospitalization after outpatient dermatologic reconstructions using large adjacent tissue transfer with flap areas of 60 cm 2 or greater performed under local anesthesia.
METHODS: Eligible patients were identif

The multidimensional nature of dermatology-encompassing pediatric, medical, surgical, and cosmetic dermatology and grounded in visual diagnosis, procedural care, dermatopathologic correlation, and longitudinal disease monitoring-demands electronic medical record systems tailored to the specialty. In parallel, dermatologic practice requires fully integrated practice management systems capable of supporting precise documentation and accurate assignment of dermatology-specific CPT codes tied to granular anatomic skin locations. This article examines leadership and strategic considerations underly

Punch-based techniques are widely used minimally invasive surgical procedures in dermatologic surgery. Beyond diagnostic use, punch-based procedures are increasingly applied therapeutically across multiple conditions. This review summarizes the current therapeutic applications of punch-based techniques and discusses the strengths and limitations of existing evidence. Current literature supports the use of follicular unit extraction (FUE) for androgenetic alopecia, with generally predictable and reproducible outcomes. In contrast, applications of punch-based techniques for scar improvement, vit

Elevated blood pressure is a major cause of perioperative bleeding and postoperative hematoma. Seventy-five patients scheduled for dermatologic surgical procedures were prospectively asked if they were aware of any personal history of hypertension. If patient response alone is relied on as a preoperative screen for hypertension, over half of the individuals affected with this disease will be missed. Epinephrine-containing local anesthetics were found not to increase blood pressure in doses commonly employed by dermatologists; in fact, they lowered blood pressure in hypertensive individuals.
