The toxicological findings of a multi-drug related fatal poisoning are described here. A 35-year-old Caucasian male found dead on the kitchen floor was a known user of abused drugs and had been taking aspirin alone or in combination with phenacetin and caffeine for the relief of joint pains. The gross examination of the organs at autopsy revealed slight grooving of the uncus and various stages of necrosis in the renal papillae. The vast majority of drug deaths in St. Louis City and County through 1977-1979 were of adults who ingested multiple drugs with suicidal intent. In this 3-year survey, seven deaths were due to accidental overdose of ethanol, seven to "over-the-counter" products or laboratory reagents, eight from "classic poisons," and only eight to drug abuse. Three cases are presented of patients who experienced potentiation of propoxyphene's effects by different antihistamines. These included increased analgesia and mild euphoria as well as moderately severe symptoms of opiate withdrawal at therapeutic doses of propoxyphene. In one case, withdrawal symptoms were blocked with clonidine, an alpha-2 agonist. Since the specific antihistamine-opiate combination, pentazocine and tripelennamine, is associated with increased lethality, physicians are cautioned to avoid combining propoxyphene with antihistamines.

A child is reported with a constellation of anomalies which include acetabular dysplasia with bilateral dislocated hips, persistent cloaca, hypoplastic kidney, two umbilical vessels, anal atresia and no obvious external genitalia. These anomalies are strikingly similar to a case reported by Peeden et al ('79) which was referred to as prune perineum. A discussion on possible underlying cause(s) of caudal dysplasia is included.

From 1971 through 1980, 1115 deaths were ascribed by the Dallas County Medical Examiner's Office to poisoning of some type. An analysis of these deaths, with emphasis on the substances involved, is the purpose of this study. Relatively few agents or groups of agents accounted for 87% of these deaths: carbon monoxide, barbiturates, propoxyphene, narcotics, tricyclic antidepressants, ethyl alcohol, and inhalants (gases and solvents).
