Worldwide variations in purchase and per-procedure costs of single-use flexible ureteroscopes and flexible navigable access sheaths: the economic impact of reuse strategies on procedural outcomes from a 40-country YAU-EAU endourology international collaboration.

Endourological practice increasingly relies on single-use devices, the costs of which vary considerably across regions and healthcare systems. Flexible single-use ureteroscopes (FURS) and flexible and navigable suction ureteral access sheaths (FANS) represent a substantial proportion of ureteroscopy-related expenditure. To mitigate these costs, some centres have adopted reuse strategies aimed at reducing per-procedure expenses. The aim of this study was to evaluate international variation in the purchase prices and real-world per-procedure costs of FURS and FANS, and to assess the economic imp

Comparative carbon footprinting study of reusable vs. disposable instruments in cataract surgery.

BACKGROUND: Theatre activity is a major source of hospital waste, accounting for 21-30% of total output, and is 3-6 times more energy-intensive than any other hospital department. Cataract remains the leading cause of blindness worldwide, and cataract surgery is the most commonly performed surgical procedure worldwide, but to date has received little analysis in this field. The aim of this study was to evaluate the carbon impact of streamlining single-use packs, and transitioning from single-use items, to reusable equivalents in cataract surgery. METHODS: A focus group and prospective audit w

Survey of cataract surgeons' and nurses' attitudes toward operating room waste.

In an online survey of more than 1300 cataract surgeons and nurses, 93% believed that operating room waste is excessive and should be reduced; 78% believed that we should reuse more supplies; 90% were concerned about global warming; and 87% wanted medical societies to advocate for reducing the surgical carbon footprint. The most commonly cited reasons for excessive waste were regulatory and manufacturer restrictions on reuse or multiple use of devices, supplies, and pharmaceuticals. More than 90% believed that profit, liability reduction, and failure to consider carbon footprint drive manufact

Aluminium release by coated and uncoated fluid-warming devices.

The use of fluid-warming systems is recommended for infusion rates > 500 ml.h-1 to avoid peri-operative hypothermia. Some fluid-warming devices use disposable aluminium-heated plates for heat transfer, but there is no protective coating to separate the fluid from the heated aluminium surface. It is unknown if this could promote release of aluminium into infusion fluids. We investigated a coated (Fluido compact) and an uncoated (enFlow) fluid-warming device using normal saline or balanced electrolyte solution as infusion fluids, pumped through the heated disposables at flow rates of 2, 4 and 8

The uses of mercury equipment and products in Irish healthcare.

Mercury is a naturally occurring metal, one of the most toxic metals within the food chain and has a number of uses in the Irish health care sector. This paper reports on a mercury survey in Ireland with the purpose to present a picture of mercury use in health settings. The findings show 50% of health settings free of all mercury medical equipment. Mercury spills occur within 30% of health settings, with no training provided in handling mercury equipment, in the management of mercury spills and only 10% of health services have mercury clean up kits. With 50% of mercury waste disposed inapprop

Allergic contact dermatitis from glutaraldehyde exposure.

A hospital maintenance employee developed an airborne contact dermatitis when cleaning respiratory therapy equipment. Patch-testing determined that she is allergic to glutaraldehyde, an ingredient in a popular commercial germicidal product. The increasing concern over acquired immunodeficiency syndrome (AIDS) and other infectious diseases has resulted in more intensive cleaning, sterilizing, and disposal procedures in health care facilities. This, in turn, may lead to an increased risk of contact dermatitis among health care workers. Glutaraldehyde is commonly used as a sterilizing agent in me

A disposable needle for frontalis suspension surgery in congenital ptosis.

In six patients affected by congenital ptosis, a disposable epidural anesthesia needle was used instead of a Wright needle for frontalis suspension surgery. For each of the patients in this study, the use of a disposable needle permitted smaller supra-brow area incisions with a minimal scar. The needle also created a smaller breach in the plane of levator aponeurosis and preaponeurotic fat, causing less tissue edema and bleeding than with a Wright needle.

Emanating challenges in urban and healthcare waste management in Isfahan, Iran after the outbreak of COVID-19.

This study aims to investigate the emanating challenges in urban and healthcare waste management in Isfahan, after the new Coronavirus outbreak. Production of 'dry wastes' in Isfahan, with a population of more than 1.9 million, has increased during the pandemic. Personal protective equipments (PPEs) including face masks, single-use gloves and face shields are an emerging group of materials in Isfahan's waste stream. On average, Isfahanian residents have daily discarded over 1.49 and 2.98 million pieces of facemasks and plastic gloves, respectively, after the new Coronavirus pandemic. Overall p

Carbon Footprint of Laparoscopic Appendicectomy: A Prospective Observational Study at a Tertiary Care NHS Trust Hospital.

Introduction Climate change is a significant global challenge as a result of rising greenhouse gas emissions. The National Health Service (NHS), as one of the biggest contributors to the United Kingdom's emissions, has a net-zero carbon emissions target. Operating theatres have a large resource burden in hospitals, and the laparoscopic appendicectomy (LA) is one of the most commonly performed procedures. As of yet, the laparoscopic appendicectomy has not been effectively carbon footprinted. Aims The objective of this study is to calculate the carbon footprint of the laparoscopic appendicectomy

Route of administration of BCG in a school population: outcome of an audit of clinical practice in North Bedfordshire.

Following findings of variations in policies and practices of BCG (Bacillus Calmette-Gúerin) administration in different parts of England and Wales, recommendations were made for BCG vaccination to be administered intradermally using disposable equipment. Intradermal methods were used for BCG in Bedfordshire until 1981 but it was changed in 1982 to percutaneous method using the reusable modified heaf gun. Suggestion to revert from percutaneous to intradermal method caused anxiety amongst health professionals with anecdotal reports of scar formations and keloids causing disfigurement at injecti

Optometry Australia's infection control guidelines 2020.

Optometry Australia's infection control guidelines 2020 deliver a revision of the paper: Infection control guidelines for optometrists 2016. A review of recent literature was undertaken, with information collected from peer-reviewed journal articles, guidelines from professional societies, government health departments and instructions from equipment manufacturers. This information was used to provide an update on current infection control best practice. The guidelines are presented in two sections: standard precautions and transmission-based precautions. The standard (routine) precautions sec

Single-Use Flexible Ureteroscopes: How Do They Compare with Reusable Ureteroscopes?

Flexible ureteroscopy has become an important tool in the urologist's armamentarium. Until recently, reusable ureteroscopes were the only tools available to perform ureteroscopy. However, in recent years, single-use flexible and semirigid ureteroscopes have been developed as an alternative to reusable ureteroscopes. These disposable ureteroscopes were designed to mitigate problems associated with the use of reusable ureteroscopes, including the high costs related to ureteroscope acquisition, maintenance, processing, sterilization, and repairs. In this review, we provide an overview of currentl
