Donor Search and Selection Strategy to Facilitate Comparable Transplant Rates across Donor Search Prognosis Groups: A Report from the BMT CTN 1702 Trial.

In a secondary analysis from the BMT CTN 1702 trial, we report on donor search and selection strategies according to baseline recipient search prognosis (SP). A total of 1751 patients in 3 SP groups-very likely, 958; less likely, 517; very unlikely, 276- were analyzed. The target time to HCT was most often 6 to 12 weeks (SP P = nonsignificant) and was associated with acute myelogenous leukemia remission status (P < .01). The baseline preferred alternative donor (across all SP groups was most commonly a haploidentical (haplo) donor (62.2% overall), whereas the use of mismatched unrelated donors

Blood donor safety and selection criteria for individuals self-identifying as trans or non-binary.

Social movements in favour of greater diversity, equity and inclusion have led societies to re-examine the concepts of sex assigned at birth and gender identity. These have also impacted healthcare systems and blood operators. Gender identities extending beyond the concept of cisgender binary individuals might have an impact on the safety of blood components for recipients, and on the safety of the donation process itself. Furthermore, the procedures put in place by blood operators to welcome donors from sexual and gender diversity might have a major impact on donors' psychological well-being

Immunocomplex capture fluorescence analysis for donor selection in HLA-haploidentical peripheral blood stem cell transplantation.

Donor-specific HLA antibodies (DSA) are associated with graft failure after HLA-mismatched allogeneic hematopoietic stem cell transplantation (HSCT). The most widely used method for identifying anti-HLA antibodies is the single-antigen bead (SAB) assay. Although this method has high sensitivity, it also detects natural HLA antibodies, which are considered to have low clinical significance. Instead, detection of reactivity between recipient serum and donor cells, such as lymphocyte crossmatching, may be useful for HSCT donor selection. We evaluated the efficacy of immunocomplex capture fluoresc…

Intra-abdominal lymph node flaps in lymphedema therapy: An anatomical guide to donor site selection.

BACKGROUND: Secondary lymphedema is a leading sequela of cancer surgery and radiotherapy. The microsurgical transfer of lymph node flaps (LNFs) to affected limbs can improve the symptoms. The intra-abdominal cavity contains an abundant heterogenic source. The aim of this study is to aid selection among intra-abdominal LNFs. METHODS: Eight LNFs were harvested in a microsurgical fashion at five sites in 16 cadavers: gastroepiploic, jejunal, ileal, ileocolic, and appendicular. These flaps were compared regarding size, weight, arterial diameter, and lymph node (LN) count after histologic verifica

Indian blood donor selection guidelines: Review in the context of the ongoing COVID-19 pandemic.

The National Blood Transfusion Services under the aegis of the ministry of health and family welfare, India has recently issued guidelines regarding the blood donor selection criteria and the processing of blood. Care has been taken to make the blood transfusions safer. However, COVID-19 has disrupted the organization of the voluntary blood donation drives, whole blood donations [WBD] and restricted the donors' movement to the blood transfusion centres all across the world. While sickness and the need for transfusions are very much in place, the gap of demand against blood collection has widen…

Improvement in short-term pancreas transplant outcome by targeted antimicrobial therapy and refined donor selection.

Graft thrombosis and infectious complications are the main early causes of pancreatic allograft loss in recipients of whole vascularized pancreas transplants, resulting in loss rates up to 10 per cent in the first post transplant week. In this study we sought to determine if initiation of a standardized selection criteria and posttransplant chemoprophylaxis regimen could reduce the rate of early allograft loss; we compared the rate of early allograft loss after introduction of these changes. Of the 61 diabetic recipients who underwent these protocols, 50.8 per cent were female. Average age was

Proceedings of a consensus conference: blood-borne HIV and hepatitis-optimizing the donor selection process.

Over the past several years, concern for the safety of blood transfusion has evolved from a primary focus on bacterial infections such as syphilis to viral infections including hepatitis B, C, and human immunodeficiency virus (HIV), and most recently, the theoretical risk of variant Creutzfeldt-Jakob Disease (vCJD). As a result of these changes, blood bankers have found themselves faced with the dilemma of balancing the rights of transfusion recipients to receive safe blood products, with the rights of individuals wishing to donate blood as an altruistic contribution to society.

Infectious disease transmission through cell, tissue, and organ transplantation: reducing the risk through donor selection.

The incidence of cell transplant-transmitted infection is unknown and can only be inferred from prospective studies--that have not yet been performed and reported. The possibility of donor-to-recipient disease transmission through cell transplant therapy can be considered by reviewing the risk associated with other transplanted tissues and organs. Viral, bacterial, and fungal infections have been transmitted via transplantation of organs, tissue allografts such as bone, skin, cornea, and heart valves, and cell such as islets, hematopoietic stem cells, and semen. Several types of protozoan and

PCR-SSO typing for DR4-Dw subtypes: application to unrelated bone marrow transplant donor selection.

Thirty-seven DR4-positive patient-unrelated bone marrow donor pairs previously DR/DQ restriction fragment length polymorphism (RFLP) typed and tested in mixed lymphocyte culture (MLC), have been DR4-Dw subtyped retrospectively using sequence specific oligonucleotide probes. We found that DR4-Dw subtyping substantially increased the accuracy of pre-MLC matching and could potentially accelerate donor searches by avoiding unnecessary MLC tests on Dw-mismatched donors.

Convalescent plasma therapy for COVID-19 - Donor selection strategies and establishment of a plasma bank.

BACKGROUND: Early in the COVID-19 pandemic, convalescent plasma (CP) emerged as a potentially effective treatment neutralising SARS-CoV-2. Early CP therapy with high neutralising antibody (NAb) titre may benefit COVID-19 outpatients and, in sufficient quantities even some hospitalised patients. This study details the process of setting up a CP bank, containing high- and low-titre CP for a clinical trial. STUDY DESIGN AND METHODS: We identified 18-65-year-old convalescents with SARS-CoV-2 NAb titres of ≥1:40 in microneutralisation test (MNT). Following eligibility pre-screening, the Finnish Re

Creatinine clearance in selection of living kidney donor among the Malaysian population: is it safe?

BACKGROUND: Assessment of donor renal function is made by the measurement of Glomerular Filtration Rate (GFR). Exogenous markers are preferred over creatinine clearance and are widely used for measuring GFR. However, they are difficult to obtain, costly and laborious. This is a study to look into the safety and accuracy of creatinine clearance for renal assessment among the living kidney donors in the Malaysian population. METHODS: This is a retrospective, single-centre study comprising 105 living kidney donor candidates from the year 2007 to 2020. By comparing against 51-Chromium ethylenedia

Donor selection in a pediatric stem cell transplantation cohort using PIRCHE and HLA-DPB1 typing.

BACKGROUND: New strategies to optimize donor selection for hematopoietic stem cell transplantation (HSCT) have mainly been evaluated in adults, but the disease spectrum requiring HSCT differs significantly in children and has consequences for the risk of complications, such as graft-versus-host disease (GvHD). PROCEDURES: Here we evaluated whether HLA-DPB1 and Predicted Indirectly ReCognizable HLA-Epitope (PIRCHE) matching can improve donor selection and minimize risks specific for a pediatric cohort undergoing HSCT in Berlin between 2014 and 2016. RESULTS: The percentage of HLA-DPB1-mismatc…

Impact of the individualized risks of end-stage renal disease on living kidney donor selection.

BACKGROUND: It is recommended to determine the risks of end-stage renal disease (ESRD) in living donor candidates. The aim of this study was … cted lifetime ESRD risk <0.5%. CONCLUSIONS: The use of these ESRD risks would significantly modify donor selection by increasing the percentage of donors ≥61 years of age with GFR <80 mL/min/1.73 m2 and by decreasing the percentage of donors aged 18-35 years with a high GFR.

Prediction of NK Cell Licensing Level in Selection of Hematopoietic Stem Cell Donor, Initial Results.

Natural killer (NK) cell licensing status depends on clonal expression of inhibitory killer cell immunoglobulin-like receptors (iKIR) and sh … over downward resetting inhibitory KIR:HLA constellation and inclusion of the KIR genotyping in the donor selection algorithm for malignant patients. Further studies using enlarged cohorts of patients with more homogenous diagnosis are essential to reliably verify these preliminary data.

Implication of molecular analysis of HLA-A*02 subtyping for unrelated bone-marrow donor selection.

The HLA-A*02 allele is the most heterogeneous allele at HLA-A locus with 22 different subtypes so far identified. All of these subtype polymorphisms are located in alpha 1 and alpha 2 domains which are responsible for peptide biding and HLA restricted recognition by T-cell receptor. The aim of the present study was to determine the frequency of different HLA-A*02 alleles in 33 healthy unrelated Croatians. HLA-A*02 subtyping has also been retrospectively performed in 2 recipient-unrelated donor pairs and in 4 recipient-HLA phenotypically identical parent pairs. All subjects, previously typed as

A Worldwide Survey of Live Liver Donor Selection Policies at 24 Centers With a Combined Experience of 19 009 Adult Living Donor Liver Transplants.

BACKGROUND: Although surgical technique in living donor liver transplantation (LDLT) has evolved with a focus on donor safety and recipient challenges, the donor selection criteria remain considerably disparate. METHODS: A questionnaire on donor selection was sent to 41 centers worldwide. 24 centers with a combined experience of 19 009 LDLTs responded. RESULTS: Centers were categorized into predominantly LDLT (18) or deceased donor liver transplantation (6), and high- (10) or low-volume (14) centers. At most centers, the minimum acceptable graft-to-recipient weight ratio was 0.7 or less (67%…
