Recent advances in the control of dengue fever using herbal and synthetic drugs.

Dengue virus represents a global public health threat, being prevalent in tropical and subtropical regions, with an increasing geographical distribution and rising incidence worldwide. This mosquito-borne viral agent causes a wide range of clinical manifestations, from mild febrile illness to severe cases and potentially fatal outcomes due to hemorrhage and shock syndrome. The etiological agent, dengue virus (DENV), has four distinct serotypes, each capable of inducing severe clinical outcomes. The current therapeutic landscape remains limited, with management strategies mainly focused on supp

Drug Fever Due to Favipiravir Administration for the Treatment of a COVID-19 Patient.

A 55-year-old Japanese man was hospitalized with the novel coronavirus disease 2019 (COVID-19). On the 14th day after the start of favipiravir administration, the patient developed a fever with a temperature of 38.1°C. His pulse rate also became elevated to 128 bpm, so relative bradycardia was not suspected. Since he was in good overall health and no concomitant symptoms and signs were apparent, we considered it to be drug fever due to favipiravir. After the completion of favipiravir treatment, the patient's temperature normalized within 24 hours. We herein report this case of drug fever cause…

Crystal Structure of African Swine Fever Virus dUTPase Reveals a Potential Drug Target.

E165R, a highly specific dUTP nucleotidohydrolase (dUTPase) encoded by the African swine fever virus (ASFV) genome, is required for productive replication of ASFV in swine macrophages. Here, we solved the high-resolution crystal structures of E165R in its apo state and in complex with its product dUMP. Structural analysis explicitly defined the architecture of the active site of the enzyme as well as the interaction between the active site and the dUMP ligand. By comparing the ASFV E165R structure with dUTPase structures from other species, we found that the active site of E165R is highly simi

Multi-drug resistant typhoid fever in hospitalised children. Clinical, bacteriological and epidemiological profiles.

A total of 592 children with clinical diagnosis of typhoid fever admitted to the Dr B. C. Roy Memorial Hospital for Children, Calcutta, India during the period between February 1990 and January 1992, were screened for Salmonella typhi by blood culture. S. typhi was isolated from 221 (37.3%) cases. The majority of the strains (92.3%) showed multi-drug resistant (MDR). They were resistant to chloramphenicol, ampicillin, tetracycline and trimethoprim-sulphamethoxazole. However, all the strains were uniformly (100%) susceptible to gentamicin, amikacin, furazolidone, norfloxacin and ciprofloxacin.

Microbial pigments as potential anti-rift valley fever virus drugs.

Rift valley fever virus (RVFV) is among the WHO list of priority diseases, yet no effective vaccine or treatment is currently available. Microbial pigments (MPs) represent a promising small-molecules library which can be exploited for the drug discovery of anti-RVFV compound. In this study, thirteen MPs were in silico screened to identify candidates with acceptable drug-likeness and possible ability to cross the blood brain barrier. Next, the binding interaction of the filtered molecules were compared against key RVFV proteins for the selection of the optimum inhibitor. Molecular dynamics simu

Antiviral Drugs Against Severe Fever With Thrombocytopenia Syndrome Virus Infection.

Severe fever with thrombocytopenia syndrome (SFTS) is an emerging tick-borne infectious disease caused by SFTS virus (SFTSV), which is a novel bunyavirus. SFTSV was first isolated from patients who presented with fever, thrombocytopenia, leukocytopenia, and multiorgan dysfunction in China. Subsequently, it was found to be widely distributed in Southeast Asia (Korea, Japan, and Vietnam). SFTSV can be transmitted not only from ticks but also from domestic animals, companion animals, and humans. Because the case fatality rate of SFTS is high (6-30%), development of specific and effective treatmen

Fever or a soar throat after start of antithyroidal drugs? A medical emergency.

We describe two patients who developed granulocytopenia (granulocyte count < 1.0 x 10(9)/1) after treatment with antithyroid drugs. Although, perhaps, an already known side-effect of these drugs, we present these cases in order to stay alert for this serious complication. The first patient developed granulocytopenia upon reinstitution of propylthiouracil after an uncomplicated first course of ATD-treatment. In the second case we report the use of granulocyte colony-stimulating factor (G-CSF) for treatment of a patient with methimazole-induced granulocytopenia. Furthermore, we discuss current c

In Vitro and In Vivo Models for Drug Development Against Two Hemorrhagic Hareavirales: Rift Valley Fever and Crimean Congo Hemorrhagic Fever Viruses.

Rift Valley fever virus (RVFV) and Crimean-Congo hemorrhagic fever virus (CCHFV) are designated by the World Health Organization as priority pathogens due to their epidemic potential, zoonotic transmission, and the absence of licensed therapeutics or vaccines. The development of effective antivirals critically relies on robust in vitro and in vivo models; however, progress is limited by the requirement for high-containment facilities. In this review, we provide a comprehensive overview of the experimental models currently available for RVFV and CCHFV, ranging from cell-based assays to animal m

Salbutamol intoxication: is salbutamol a drug-inducing fever? A case report and treatment strategy.

A four-year-old female with salbutamol intoxication was referred to our paediatric emergency medicine unit, due to agitation, tremulousness, sinus tachycardia, mild hypokalaemia and hyperglycaemia. On admission the child was agitated and had a noticeable tremor, an axillary temperature of 38 degrees C and a pulse rate of 185 beats/min. She had no identifiable focus of infection on physical examination to explain her fever. Gastric lavage, activated charcoal, intravenous hydration and electrocardiogram (ECG) monitoring were performed. Her plasma potassium level, blood sugar and QT interval were

Methicillin induced drug-fever during treatment for acute post traumatic osteomyelitis and septic arthritis.

High dose intravenous methicillin therapy is a common form of treatment for post traumatic acute osteomyelitis and septic arthritis, with positive staphylococcus aureus cultures. A high and spiking fever, persisting in the face of adequate antibiotic therapy, presents a difficult diagnostic dilemma in such patients. In seven patients, methicillin induced drug fever was found to be the cause of such as persisting pyrexia, inspite of an adequate antibiotic regime. This diagnosis must be established by a cautious clinical evaluation and by a careful elimination of all other causes for fever. Upon…

Drug-induced fever: a pharmacovigilance analysis based on the FDA adverse event reporting system.

INTRODUCTION: Drug-induced fever is a common but often under-recognized adverse reaction that can complicate clinical management. AIM: This study aimed to systematically evaluate the risk of drug-induced fever using real-world pharmacovigilance data and to characterize its temporal onset patterns and clinical features. METHOD: A pharmacovigilance study was conducted utilizing data from the FAERS, covering reports from the first quarter of 2004 to the fourth quarter of 2024. The detection of adverse drug event signals was conducted using disproportionality analysis. The robustness of identifi

Successful Sequential and Additive Drug Challenge With Rapid Oral Desensitization in Rifampicin-Induced Fever.

Adverse drug reactions to first-line antituberculosis (anti-TB) therapy are common and may necessitate treatment interruption. While drug desensitization is increasingly used for delayed hypersensitivity, evidence supporting outpatient rapid oral desensitization in mild delayed reactions such as drug-induced fever is limited. We report a 63-year-old man with active pulmonary tuberculosis who developed drug-induced fever after commencing standard anti-TB therapy. Sequential and additive drug challenge identified rifampicin as the offending agent. The patient subsequently underwent successful ra

Antiviral Drug Discovery and Development for Mayaro Fever - What do we have so far?

Tropical infectious diseases cause millions of deaths every year in developing countries, with about half of the world population living at risk. Mayaro virus (MAYV) is an emerging arbovirus that causes Mayaro fever, which is characterized by fever, headache, diarrhea, arthralgia, and rash. These symptoms can be clinically indistinguishable from other arboviruses, such as Dengue, Zika, and Chikungunya, which makes the diagnosis and treatment of the disease more difficult. Though, the Mayaro virus is a potential candidate to cause large-scale epidemics on the scale of ZIKV and CHIKV. Despite th

Drug-resistant enteric fever worldwide, 1990 to 2018: a systematic review and meta-analysis.

BACKGROUND: Antimicrobial resistance (AMR) is an increasing threat to global health. There are > 14 million cases of enteric fever every year and > 135,000 deaths. The disease is primarily controlled by antimicrobial treatment, but this is becoming increasingly difficult due to AMR. Our objectives were to assess the prevalence and geographic distribution of AMR in Salmonella enterica serovars Typhi and Paratyphi A infections globally, to evaluate the extent of the problem, and to facilitate the creation of geospatial maps of AMR prevalence to help targeted public health intervention. METHODS:
