Prominent ears.

The prominent ear deformity can be divided into two basic types, and combinations of the two. Two different operations are proposed to treat the cause of the deformity without foreign bodies or suture tension. The result should be an ideal shape. The ear should also feel normal and should not show any stigma of surgery.

Perichondritis of the ear.

A total of five ears with perichondritis secondary to burns was treated using a method of excising cartilage. This treatment resulted in the need for general anesthesia, frequent repeat debridement and frequent loss of structure of the ear with subsequent deformity. The presentation, etiology, pathology and present modes of therapy are presented. Our experience stresses the importance of early recognition and treatment and further consideration for the use of drains with continuous antibiotic and proteolytic enzyme administration.

Chronic ear pruritus.

Chronic ear pruritus, itching of the ear that lasts longer than six weeks, is a common symptom with a wide differential diagnosis that can significantly impair the quality of life. It can arise from such primary dermatologic conditions as psoriasis, seborrheic dermatitis, contact dermatitis, atopic dermatitis, and lupus erythematosus, as well as such infestations as scabies and pediculosis capitis. Otolaryngologic causes-for example, otomycosis, chronic otitis externa, and temporomandibular joint disorders-may also lead to chronic ear pruritus. Systemic diseases, such as diabetes, hematologic …

Inner ear immunity.

The inner ear, like all organs, interacts with the systemic immune system via lymphatic drainage and vascular circulation to protect itself from infections and stress such as acoustic trauma. The adult mammalian inner ear including the endolymphatic sac is populated with bone-marrow derived resident macrophages. Circulating macrophages continually renew the resident macrophage population. Cells within the endolymphatic sac participate in and affect inner ear immune responses, but specific mechanisms for the interactions are unknown. Resident macrophages are present within the cochlear modiolus…

Preventing Cauliflower Ear.

Traumatic auricular hematomas may be encountered in emergency care settings due to blunt trauma such as assaults or contact sports. Emergency nurse practitioners should be able to recognize and treat this injury to prevent complications. Treatment usually consists of evacuation of the hematoma by aspiration or incision and drainage using local or regional anesthesia, followed by the application of a pressure dressing or suturing. Without proper evacuation and steps to prevent reaccumulation, tissue necrosis and deformity (i.e., cauliflower ear) may occur.

Incomplete Ear Amputation.

Ear amputations are common in emergency departments as the auricle's protrusion from the lateral aspect of the head makes it particularly susceptible to trauma. Of the numerous approaches for auricle injuries, the classic methods include microsurgical replantation, primary reattachment, composite graft, retroauricular pocket principle, secondary reconstruction with rib cartilage, and methods using postauricular flap. The success of the operation depends largely on choosing the appropriate surgical procedure since many factors can influence surgical outcome. There is no 1 gold standard techniqu…

Chronic Ear Disease.

Chronic ear disease is composed of a spectrum of otologic disorders intrinsically tied to Eustachian tube dysfunction. Presentation can range from asymptomatic findings on physical examination to critically ill patients with intracranial complications. Internists represent the first line in diagnosis of these conditions, making awareness of the common signs and symptoms essential. With surgical management often required, partnership between internal medicine and otolaryngology is fundamental in the management of patients with chronic ear disease.

Autologous Ear Reconstruction.

Since the pioneering use of autologous rib cartilage for the reconstruction of microtia, there have been significant advances in surgical technique that have helped to ameliorate the psychological burden of microtia. To date, the use of rib cartilage for auricular reconstruction is one of the most enduring and ubiquitous techniques for microtia reconstruction as it provides excellent aesthetic results with lasting durability. In this review, the authors outline the most common methods of microtia reconstruction with a comparison of each technique and illustrative case examples.

[MIDDLE EAR INFECTION].

Middle ear infection is one of the most common childhood infections and the leading reason for antibiotic prescriptions. Although the etiological diagnosis is rarely discovered, successful identification of pathogens depends on properly collected sample, chosen method and microbiological analysis made on time. The most common bacterial pathogen is Streptococcus pneumoniae. Others include Haemophilus influenzae, Moraxella catarrhalis and Pseudomonas aeruginosa, known as the most common bacterial pathogen of chronic inflamations. Viral or polimicrobial upper respiratory tract infections often pr…

Cauliflower ear dissection.

Cauliflower ear (CE) is caused by repeated direct trauma to the external ear. Surgical correction of an established CE is one of the most challenging problems in ear reconstruction. However, no reports have clarified the dissection of an established CE in detail. In this report, the dissection of a CE is described based on macroscopic, microscopic and imaging features.
