Laser-assisted cartilage reshaping (LACR) for treating ear protrusions: a clinical study in 24 patients.

BACKGROUND: Protruding ears are the most common congenital ear deformity, with a frequency of 13.5%. Since 1845 and the first works of Dieffenbach, over 170 classical surgical methods have been proposed to correct it. We introduced laser-assisted cartilage reshaping (LACR) in 2004 as an alternative to invasive surgical otoplasty. METHODS: Between January 2008 and June 2008, 24 patients underwent LACR for treatment of bilateral ear protrusion. Fourteen adults and ten children were treated (mean age = 16.0 years, range = 6-45 years). No anesthesia was used. Both sides of the entire helix and th

[Clinical study of chest contour deformity after harvesting of costal cartilage for total ear reconstruction].

OBJECTIVE: To investigate the chest contour deformities after harvesting of costal cartilage for total ear reconstruction, so as to prevent donor site morbidity. METHODS: 107 cases of microtia who underwent ear reconstruction with costal cartilage during the period from 1994 to 2003, were retrospectively studied. Both the chest depression deformity and the costal arch deformity were evaluated. The possible related factors to the chest contour deformities were analyzed statistically with Stata software. RESULTS: The incidence of chest depression was 54.55% (48/88), including 35 cases of mild

[Tragus perichondrium-cartilage island transplant in middle ear surgery. Method and results after 5 years].

Temporalis fascia remains the most widespread material for reconstructing the tympanic membrane in tympanomastoid surgery. If total or partial ossicular replacement prostheses are needed or pathology of the eustachian tube causes ventilation impairment, a more rigid transplant material is required. Between 1989 and 1994 perichondrium-cartilage composite grafts were used in a series of 597 cases of tympanomastoid surgery performed at the University ENT Clinic of Würzburg. The graft was taken from the tragus and prepared as a cartilage island with perichondrium attached to one side, the perichon

[Preserved human cartilage grafts and autogenous viable transplants for nose and middle ear (author's transl)].

57% of the preserved human cartilage chips being implanted in the nose lead to a remarkable up to a complete resorption. 94% of the patients have nevertheless a sufficiently or well working function of the nose; besides that they are as well satisfied with the shape of their nose. The most important reason for the resorption are mechanical factors; this has been shown by comparing the load of implanted tissue on the columella and on the dorsum of the nose. We can't exclude the possibility of late immunologic reaction. Comparing these results with those of the middle ear by missing any mechanic

Type IB Cauliflower Ear Deformity Correction Using Subunit Reconstruction Technique Combined With Auricular Cartilage Stretching Surgery.

BACKGROUND: Yotsuyanagi type IB cauliflower ear deformity, characterized by helix curling and loss of antihelix structure, presents unique challenges in reconstructive surgery. Traditional approaches often struggle to achieve satisfactory aesthetic outcomes while maintaining natural auricular morphology in these cases. This study presents a novel surgical technique combining subunit reconstruction with auricular cartilage stretching specifically for treating Yotsuyanagi type IB cauliflower ear deformity. METHODS: From January 2020 to January 2024, patients with Yotsuyanagi type IB cauliflower

Rib Cartilage Assessment Relative to the Healthy Ear in Young Children with Microtia Guiding Operative Timing.

BACKGROUND: The optimal age at which to initiate for auricular reconstruction is controversial. Rib cartilage growth is closely related to age and determines the feasibility and outcomes of auricular reconstruction. We developed a method to guide the timing of auricular reconstruction in children with microtia ranging in age from 5 to 10 years. METHODS: Rib cartilage and the healthy ear were assessed using low-dose multi-slice computed tomography. The lengths of the eighth rib cartilage and the helix of the healthy ear (from the helical crus to the joint of the helix and the earlobe) were mea

Surgical treatment of lagophthalmos in facial palsy: ear cartilage graft for elongating the levator palpebrae muscle.

Ocular exposure, conjunctival irritation and corneal keratitis are disturbing consequences of facial paralysis with lagophthalmos. Many techniques have been described for correcting this condition. A new method for decreasing the palpebral fissure is presented. An ear cartilage graft is interposed between the tarsal plate and the levator palpebrae superioris muscle. 12 patients with paralysis of the orbicularis oculi muscle, as assessed by electromyography, were treated with this technique. The palpebral fissure was decreased by 2-3 mm in all cases. 11 patients showed significant improvement o…

[Relapsing polychondritis. Histochemical, histoenzymological, immunoflourescent and ultrastructural studies of ear cartilage in three cases (author's transl)].

Ear cartilage has been studied by histochemistry, histoenzymology, immunofluorescence and electron microscopy in three cases of relapsing polychondritis. The most significant lesions have been observed at the cellular level, both by enzymology and electron microscopy : chondrocytes of peripheral zones seem to be first hypertrophic and then necrotic ; at the opposite, there is no correlation between histochemical and electron microscopic studies about the chondroid intercellular substance whose lesions are probably secondary to the cellular ones. A few chondrocytes are positive with antiimmunog…

Hybrid Auricular Framework of Autologous Rib and Ear Cartilage for Microtia Reconstruction through a Preauricular Incision.

BACKGROUND: Fabrication of the auricular framework is a key aspect of ear reconstruction. The use of autologous rib cartilage, which is considered an ideal grafting material, can minimize harvesting damage and optimize its use. This study introduces a method that combines both rib and ear cartilages to reconstruct the auricular framework using an anterior auricular approach, which can help achieve optimal results. METHODS: Between 2020 and 2024, 153 patients with microtia underwent ear reconstruction using a hybrid framework of autologous rib and ear cartilage. Reconstruction was performed th…

Comparison of functional outcomes of cartilage tympanoplasty with silastic sheet versus Gelfoam packing in middle ear.

BACKGROUND AND AIM: Tympanoplasty is a common surgery in otorhinolaryngology field. In majority of procedures, in addition to the graft used for closure of tympanic membrane, a packing material is essential to be placed in the middle ear cavity. The main goals of packing can be summarized as providing support to the tympanic membrane and ossicular grafts, aeration of middle ear cavity and hemostasis. Several packing materials are currently available for using in middle ear surgeries. Each agent is associated with particular advantages and disadvantages, so choosing the proper packing agent is

A new technique for transcutaneous fixation of the costal cartilage block utilized in reconstructed ear elevation for microtia.

PURPOSE: With the aim of implementing good projection of the three-dimensional frame (auriculo-cephalic angle) and maintaining projection, a significant issue during the stage of auricular elevation for avoiding the potential for dislodging the cartilage blocks remains. Herein we present an innovative and technically simple method of transcutaneous fixation of the costal cartilage block during the stage of auricular elevation. MATERIALS AND METHODS: After elevation of ear frame from the basement, two suspension stitches on the surface of the elevated ear were used to fix the embedding cartila

Reconstruction of constricted ears by combing bilateral cartilage flaps bridging with V-Y advancement flap.

BACKGROUND: Despite a number of surgical procedures for the reconstruction of moderate to severe constricted ears described in the literature, a most cost-effective method remains to be explored. It is still a challenge to maximize the full use of the ear cartilage and surrounding skin while achieving the best results. METHODS: From 2011 to 2016, seven constricted ear patients were enrolled in this study. Five of them were moderate (type IIB Tanzer classification) deformities, and two were severe (type III Tanzer classification). All constricted ear patients were treated with bilateral cartil…

A modified crescent cartilage block for improving the retroauricular contour of the reconstructed ear: A retrospective cohort study.

BACKGROUND: The use of a semilunar cartilage block to elevate a reconstructed ear does not always achieve a well-defined auriculocephalic sulcus. Herein, we modified the conventional crescent cartilage block through sculpting a concavity at its posterior surface, with the aim of improving the retroauricular contour. The study aimed to verify the effectiveness and reliability of this modified cartilage block through a retrospective cohort study. METHODS: A retrospective review of patients who underwent Nagata microtia reconstruction between October 2017 and November 2018 were conducted. The es

Establishment of a Standardized Technique for Concha-type Microtia-How to Incorporate the Cartilage Frame into the Remnant Ear.

UNLABELLED: We have already reported surgical procedures for lobule-type microtia that provide an excellent contour and shape of the ear with minimum sacrifice of the donor. We have succeeded in establishing a standard surgical technique for almost all types of concha-type microtia that effectively uses the remnant ear and can use a unified costal cartilage frame. METHODS AND RESULTS: The concept of our technique is that remnant cartilage should be used maximally but that the deformed area should be completely replaced by the costal cartilage frame. The differences between the cartilage frame
