Behavioral economics of cocaine self-administration in male and female rats.

There are sex differences in the development of cocaine addiction. For example, the time that it takes for women from initial use to addiction is significantly shorter than for men. Thus, understanding why females are more vulnerable to cocaine addiction will provide insights into sex differences in the mechanisms underlying cocaine addiction. This study aimed to determine how cocaine demand intensity and elasticity might differ between sexes. In addition, the impact of estrous cycle and cocaine intake on demand was investigated. Male and female rats were trained to self-administer 0.125 mg of

Economics of Child Protection: Maltreatment, Foster Care, and Intimate Partner Violence.

Violence within families and child neglect are strikingly common: 700,000 children are found to be victims of abuse or neglect in the United States each year; over the course of childhood, 6% of children are placed in foster care, and 18% witness intimate partner violence. These children are at much higher risks of homelessness, criminal justice involvement, unemployment, and chronic health conditions compared to their neighbors. This article reviews the state of the economics literature on the causes and consequences of child maltreatment and intimate partner violence and calls for greater re…

Report of the ACR's Economics Committee on Value-Based Payment Models.

A major outcome of the current health care reform process is the move away from unrestricted fee-for-service payment models toward those that are based on the delivery of better patient value and outcomes. The authors' purpose, therefore, is to critically evaluate and define those components of the overall imaging enterprise that deliver meaningful value to both patients and referrers and to determine how these components might be measured and quantified. These metrics might then be used to lobby providers and payers for sustainable payment solutions for radiologists and radiology services. Th

Effect of fertility on the economics of pasture-based dairy systems.

There are significant costs associated with reproductive inefficiency in pasture-based dairy herds. This study has quantified the economic effect of a number of key variables associated with reproductive inefficiency in a dairy herd and related them to 6-week calving rate for both cows and heifers. These variables include: increased culling costs, the effects of sub optimum calving dates, increased labour costs and increased artificial insemination (AI) and intervention costs. The Moorepark Dairy Systems Model which is a stochastic budgetary simulation model was used to simulate the overall ec

Taishotoyama Symposium Barriers to colorectal cancer screening: economics, capacity and adherence.

Colorectal cancer is the second leading cause of cancer death in the United States (U.S.). Fecal occult blood testing has been shown in randomized controlled trials to decrease mortality from colorectal cancer. The incidence and mortality associated with colorectal cancer has also been illustrated to be decreased with sigmoidoscopy and colonoscopy through case-control and prospective cohort studies. Current research focuses on determining which screening strategy is optimal, and how we may improve implementation. Primary screening colonoscopy may be most effective; however, this strategy would

The economics of optimal health and productivity in the commercial dairy.

Dairy production practices are changing; in order to remain viable, producers must optimise the health and productivity of dairy herds in economic terms. Health care is important in economic terms because disease can substantially reduce the productivity of individual animals. Preventive disease control programmes can thus result in economic gains for the dairy producer. The author describes new approaches to preventing postpartum diseases and dealing with fertility problems which can result from these diseases. Other aspects of dairy production are also changing, employing new technologies wh

Clinical economics review: cost-effectiveness in the therapy of liver disease.

There have been many advances made in the management of patients with liver disease both in diagnosis and in the treatment of underlying liver disease and its complications, although comparatively few of these have been rigorously subjected to full cost-effectiveness evaluation. In this review, we have analysed a small number of the therapeutic interventions; while these have been well evaluated clinically, very few have been analysed from the viewpoint of cost-effectiveness and, thus, it is difficult to make many definitive claims. It is hoped that future studies will consider these aspects a

Behavioral Economics as a Driver of Healthcare System Reform: Methodological Approach.

OBJECTIVES: The aim of this study was to develop and apply the Behavioral Economics in Health index as a methodological tool for assessing the role of behavioral strategies in healthcare reform, with particular attention to their impact on healthcare expenditure, life expectancy, and system-level adaptation in countries with varying levels of institutional maturity and economic capacity. METHODS: The research used system-dynamics-based simulation modeling using behavioral and economic parameters, followed by statistical analysis of the results. RESULTS: The AnyLogic simulation results showed…

Behavioral Economics and Medication Adherence for Hypertension: A Randomized Clinical Trial.

BACKGROUND: Nonadherence to antihypertensive medications is common. Mobile health (mHealth)-based behavioral economic interventions may improve adherence, but remain largely untested, especially in vulnerable populations. OBJECTIVE: The study sought to test whether an mHealth incentive lottery would lower systolic blood pressure (SBP) and improve adherence. METHODS: BETTER-BP (Behavioral Economics Trial To Enhance Regulation of Blood Pressure) was a randomized trial conducted in 3 safety-net clinics in New York City. Eligible participants were adults with hypertension prescribed at least 1 a…

Economics of Sickle Cell Disease and Evidence to Support Comprehensive Care.

Sickle cell disease (SCD) is a serious blood disorder leading to complex care needs. Comprehensive, multidisciplinary programs are ideally suited to deliver patient-centered care and address other relevant social determinants of health. Patients with SCD face many inequities in health care, further reinforcing the need for comprehensive care models to address these relevant issues. Comprehensive care models can be financially advantageous to institutions that care for vulnerable patients with SCD while simultaneously increasing care quality.

Treatment of cervical radiculopathy: A review of the evolution and economics.

BACKGROUND: The surgical treatment of cervical radiculopathy has centered around anterior cervical discectomy and fusion (ACDF). Alternatively, the posterior cervical laminoforaminotomy/microdiscectomy (PCF/PCM), which results in comparable outcomes and is more cost-effective, has been underutilized. METHODS: Here, we compared the direct/indirect costs, reoperation rates, and outcome for ACDF and PCF vs. PCM using PubMed, Medline, and Embase databases. RESULTS: There were no significant differences between the re-operative rates of PCF/PCM (2% to 9.8%) versus ACDF (2% to 8%). Direct costs of

Economics of resynchronization strategies including chemical tests to identify nonpregnant cows.

Our objectives were to assess (1) the economic value of decreasing the interval between timed artificial insemination (TAI) services when using a pregnancy test that allows earlier identification of nonpregnant cows; and (2) the effect of pregnancy loss and inaccuracy of a chemical test (CT) on the economic value of a pregnancy test for dairy farms. Simulation experiments were performed using a spreadsheet-based decision support tool. In experiment 1, we assessed the effect of changing the interbreeding interval (IBI) for cows receiving TAI on the value of reproductive programs by simulating a

Health economics of blood transfusion safety--focus on sub-Saharan Africa.

BACKGROUND AND OBJECTIVES: Health economics provides a standardised methodology for valid comparisons of interventions in different fields of health care. This review discusses the health economic evaluations of strategies to enhance blood product safety in sub-Saharan Africa. METHODS: We reviewed health economic methodology with special reference to cost-effectiveness analysis. We searched the literature for cost-effectiveness in blood product safety in sub-Saharan Africa. RESULT: HIV-antibody screening in different settings in sub-Saharan Africa showed health gains and saved costs. Except …

Economics may not explain Hispanic underutilization of assisted reproductive technology services.

In a lower cost, equal-access-to-care setting, Hispanic use of assisted reproductive technology was less than half of what would have been expected based on patient demographics. Despite this use disparity, there were no significant differences between Hispanic and Caucasian women with regard to infertility diagnoses, assisted reproductive technology cycle parameters, clinical pregnancy rates, live birth rates, spontaneous abortion rates, and implantation rates.
