The impact of postoperative cartoon viewing on emergence delirium in young children: A randomized controlled trial.

BACKGROUND: Research has revealed non-pharmacological methods mitigate the occurrence of emergence delirium with a lower incidence of negative side effects, but the mechanism remains unclear. This study aimed to investigate whether postoperative cartoon viewing after awakening from sevoflurane anesthesia in the postanesthesia care unit reduces the incidence of emergence delirium in young children, and to examine relevant electroencephalography findings. METHODS: This was a single-center, randomized controlled trial. One hundred thirty-eight children aged 2-7 years who underwent elective tonsi…

Improving Identification of Pediatric Emergence Delirium in the Post Anesthesia Care Unit: A Quality Improvement Opportunity.

OBJECTIVE: This article offers a recommendation on how the Pediatric Assessment Emergence Delirium Scale (PAEDS) could be implemented in the post anesthesia care unit (PACU) to improve the assessment and treatment of pediatric emergence delirium (PED). BACKGROUND: PED is an anticipated complication in the PACU characterized by mental confusion, irritability, disorientation, inconsolable crying, and prolonged postanesthetic recovery time. Although it is a short-lived phenomenon, it increases the risk for traumatic injuries and may lead to a decrease in overall parent satisfaction with their ch…

Emergence delirium with transient associative agnosia and expressive aphasia reversed by flumazenil in a pediatric patient.

Multiple factors may contribute to the development of emergence delirium in a child. We present the case of a healthy 12-year-old girl who received preoperative midazolam with the desired anxiolytic effect, underwent a brief general anesthetic, and then exhibited postoperative delirium, consisting of a transient associative agnosia and expressive aphasia. Administration of flumazenil led to immediate and lasting resolution of her symptoms. We hypothesize that γ-aminobutyric acid type A receptor-mediated effects, most likely related to an atypical offset of midazolam, are an important subset of…

Neglected emergency: Autopsy case report on fatal delirium tremens.

Delirium tremens (DT) is one of the most severe and potentially fatal manifestations of alcohol withdrawal syndrome most often found in individuals with a history of prolonged and heavy alcohol use who suddenly stop. Without prompt intervention, DT may lead to life-threatening complications such as seizures, cardiovascular collapse and autonomic dysfunctions. The importance of understanding DT extends beyond its acute neurological and psychiatric symptoms. From a forensic perspective, autopsy findings can reveal a cascade of underlying physiological derangements. We present an autopsy case rep

[Comments on the article "Incidence and factors associated with delirium in an Emergency Department"].

Changes in cognition, attention, consciousness, and perception rapidly occurring and unpredictably evolving are the distinctive hallmarks of delirium syndrome. In Mexico the incidence is reported to be 12%. In our study, subject of the quoted letter to the editor, there were included 169 patients over 65 years of age, to which the Confusion Assessment Method (CAM) questionnaire was administered upon admission to the Emergency Department and 24 hours after admission to confirm or exclude a diagnosis of delirium. An incidence of 43.8% was reported, with no statistically significant association f

Prevalence and Risk Factors for Delirium on Arrival at the Emergency Room by Ambulance.

INTRODUCTION: Delirium is an acute and often fluctuating disturbance in attention and awareness and is one of the organ dysfunctions caused by acute illness. A previous study revealed that 8.3% of older patients in the emergency room (ER) had delirium, and dementia and older age were identified as independent risk factors for delirium in the ER. However, different ER structures (intermediate or acute care units) and disease clusters may have different prevalence rates and risk factors. PURPOSE: This study aimed to identify the prevalence and risk factors for delirium on arrival at the ER. ME

Implementing delirium screening in the emergency department: a quality improvement project.

INTRODUCTION: Delirium is a serious medical condition that is common in older adults in acute settings. Clinical practice guidelines recommend that all older patients in acute care settings should be screened for delirium using standardised outcome measures. PROBLEM: In our institution, an audit showed that only 16% of older adults presenting to the emergency department were screened for delirium. The goal of this project was to increase the number of patients being screened for delirium using Lean Six Sigma (LSS) methodology and tools and a multidisciplinary approach. METHOD: A multidiscipl
