Features of primary care practice influence emergency care-seeking behaviors by caregivers of persons with dementia: A multiple-perspective qualitative study.

BACKGROUND: Persons with dementia use emergency department services at rates greater than other older adults. Despite risks associated with emergency department use, persons with dementia and their caregivers often seek emergency services to address needs and symptoms that could be managed within primary care settings. As emergency departments (EDs) are typically sub-optimal environments for addressing dementia-related health issues, facilitating effective primary care provision is critical to reduce the need for, or decision to seek, emergency services. The aim of this study is to explore how

[Emergency cesarean section--organization and decision-delivery time].

The German Society of Gynaecology and Obstetrics has published standards for obstetrical services concerning equipment, personnel and organisation. All obstetrical services must be able to perform an emergency Caesarean section with a 20 minutes interval from decision to delivery (D-D time). This study represents an analysis of the 75 emergency Caesarean sections performed at the University hospital Grosshadern of Munich during the interval from 1987 to 1994. This being a level III hospital, there is a 24 hour obstetrical, anaesthesia and neonatal service, and personnel is readily available. T

METHODS: A qualitative, interview-based needs assessment was conducted during February 10-20, 2025, at Hawassa University Comprehensive Specialized Hospital and Motite Furra Hospital, Sidama National Regional State, Et

Purchasing and Using Personal Emergency Response Systems (PERS): how decisions are made by community-dwelling seniors in Canada.

BACKGROUND: As the demographic of older people continues to grow, health services that support independence among community-dwelling seniors have become increasingly important. Personal Emergency Response Systems (PERS) are medical alert systems, designed to serve as a safety net for seniors living alone. Health care professionals often recommend that seniors in danger of falls or other medical emergencies obtain a PERS. The purpose of the study was to investigate the experience of seniors living with and using a PERS in their daily lives, using a qualitative grounded theory approach. METHODS

Creating a Regional Model to Coordinate and Prioritize Access to Permanent Supportive Housing.

Demand for supportive housing outstrips availability in metropolitan regions around the country. Individuals who are homeless with serious mental illnesses, substance abuse, and other debilitating health conditions are often heavy users of publicly financed services and institutions, such as jails, emergency departments, psychiatric and medical hospitals, and sobering and detoxification services. King County, in collaboration with community partners, has developed a regional system for coordinating and prioritizing access to this limited resource based on utilization of publicly financed servi

Multiple-Casualty Incident Following Lightning Strike at Mount Giewont: An Analysis of Disaster Rescue Response.

On August 22, 2019, several cloud-to-ground currents struck the top of the popular hiking mountain Giewont (Tatra Mountains, Poland). At the scene, first aid and evacuation were provided by mountain emergency rescue services. The injured patients received care and were initially stabilized at a local general hospital. Some of the victims were then relocated to other nearby hospitals, regional trauma centers, or regional burn centers. This study is a retrospective analysis of regional health system response. The official records of the disaster response from the institutions involved were exami

On the Design of a Sign Language Corpus of Medical Terms for Automatic Translation Systems: Mixed Methods Approach.

BACKGROUND: Hearing loss is a global health issue affecting millions and creating significant communication barriers, particularly in accessing health care services. These barriers can lead to complications and iatrogenic events, emphasizing the need for assistive technologies that enhance communication efficiency. OBJECTIVE: This study aimed to develop a corpus of medical terms for the "Captar-Libras" project, designed to improve communication between health care professionals and deaf patients through a bidirectional sign language system. METHODS: This study used the Delphi method to obtai

The benefits and tradeoffs for varied high-severity injury risk thresholds for advanced automatic crash notification systems.

OBJECTIVES: The objectives of this study are to (1) characterize the population of crashes meeting the Centers for Disease Control and Prevention (CDC)-recommended 20% risk of Injury Severity Score (ISS)>15 injury and (2) explore the positive and negative effects of an advanced automatic crash notification (AACN) system whose threshold for high-risk indications is 10% versus 20%. METHODS: Binary logistic regression analysis was performed to predict the occurrence of motor vehicle crash injuries at both the ISS>15 and Maximum Abbreviated Injury Scale (MAIS) 3+ level. Models were trained using

The policy case for designating EMS teams for vulnerable patient populations: Evidence from an intervention in Boston.

This study documents more than five years of analysis that drove the policy case, deployment, and retrospective evaluation for an innovative service model that enables Boston Emergency Medical Services (EMS) to respond quickly and effectively to investigation incidents in an area of heavy need in Boston. These investigation incidents are typically calls for service from passers-by or other third-party callers requesting that Boston EMS check in on individuals, often those who may appear to have an altered mental status or to be unhoused. First, this study reports the pre-intervention analytics

Perceived readiness of South African community service speech-language therapists to manage complex paediatric dysphagia.

BACKGROUND: Feeding and swallowing difficulties usually demand specialised knowledge and skills for effective assessment and management. In South Africa, community service speech-language therapists (SLTs) play an integral role in delivering care within the public healthcare system, where the ratio of patients to medical professionals is notably disproportionate. However, the extent to which they feel prepared when managing these conditions remains relatively underexplored. OBJECTIVES: To explore the extent to which community service SLTs feel prepared when managing paediatric feeding and swa

Changes In Health Services Use After Receipt Of Medications For Opioid Use Disorder In A Statewide Correctional System.

To decrease opioid overdose mortality, prisons and jails in the US are increasingly offering medications for opioid use disorder (OUD) to incarcerated people. It is unknown how receipt of these medications in a correctional setting affects health services use after release. In this article we analyze changes in postrelease health care use after the implementation of a statewide medications for OUD program in the unified jail and prison system of the Rhode Island Department of Corrections. Using Medicaid claims data, we examined individual health care use in the community before and after recei

Operationalizing the Deployment of Low-Titer O-Positive Whole Blood Within a Regional Trauma System.

INTRODUCTION: The implementation of a low-titer O+ whole blood (LTOWB) resuscitation algorithm, particularly in the prehospital environment, has several inherent challenges, including cost, limited and inconsistent supply, and the logistics of cold-chain management. The Southwest Texas Regional Advisory Council has implemented the nation's first multidisciplinary, multi-institutional regional LTOWB program. This research effort was to illustrate the successful deployment of LTOWB within a regional trauma system. MATERIALS AND METHODS: A deliberate systems approach to the deployment of LTOWB w
