[Physiopathology of infectious endocarditis].

Infectious endocarditis (IE) develops following bacteremic episodes during which bacteria may attach to sterile thrombotic vegetations. Such thrombotic vegetations result from the deposition of platelets and fibrin on lesioned endothelium. These sterile vegetations are the most susceptible to infection in the left side of the heart, since this localization is found in as much as 80% of the patients with IE. Any circulating bacterial or mycotic organism may induce endocarditis, but streptococci are most often responsible, possibly because of their high capacity to adhere to thrombotic vegetatio

SBE presenting with type III cryoglobulinaemia.

A case of subacute bacterial endocarditis which presented with cryoglobulinaemia is reported. A secondary cause of cryoglobulinaemia was suggested following characterization, which revealed a Type III (mixed polyclonal) cryoprotein. Levels of cryoglobulin were high and remained so until the source of infection was eradicated, and the cause of the cryoglobulinaemia is briefly discussed.

Renal and cardiac complications of drug abuse.

The case is presented of a young man who, after prolonged intravenous narcotic administration, developed subacute bacterial endocarditis involving the aortic valve, and rapidly progressive glomerulonephritis. After treatment of and recovery from renal failure, persistent proteinuria was shown to be caused by focal glomerulosclerosis. The association of these lesions with "mainlining" is reviewed.

Staphylococcal infective endocarditis.

Staphylococcus aureus causes an acute endocarditis, often involving previously normal valves. The criteria used for diagnosis of subacute bacterial endocarditis are frequency absent, and distinguishing acute endocarditis from bacteremia without valvular involvement is difficult. In vitro studies, including teichoic acid antibody assay and bactericidal tolerance, have been developed to aid in making the diagnosis and planning the treatment. Cases of native valve, prosthetic valve, and addict-associated endocarditis are considered separately because of differences in prognosis and approach. Use

Osler's nodes and Janeway lesions.

The pathogenesis of Osler's nodes and Janeway lesions remains a mystery despite vigorous debate over the last 113 years. They are given great emphasis among the clinical signs of bacterial endocarditis but are seldom seen in practice. Two cases of subacute bacterial endocarditis are presented. A 66-year-old woman with Bartonella henselae endocarditis developed Osler's nodes on the hands postoperatively, and a 23-year-old man with Streptococcus oralis endocarditis developed tender macules with an appearance suggestive of Janeway lesions on one heel. The dermatopathology was similar in the two c

Evaluation of an anti-infective combination. Trimethoprim-sulfamethoxazole (Bactrim, Septra).

Trimethoprim and sulfamethoxazole, combined in a synthetic anti-infective preparation, act synergistically to inhibit the growth of certain microorganisms. This combination is effective orally in acute and chronic urinary tract infections. It also has been used successfully to treat other infectious conditions, including bacterial upper respiratory tract infections, ear infections, skin and soft-tissue infections, septicemias, acute and subacute bacterial endocarditis, enteric fever, brucellosis, prostatitis, and gonorrhea. It is well tolerated by most patients.

Bacterial Endocarditis Presenting as Leukocytoclastic Vasculitis.

Subacute bacterial endocarditis can have many different presentations; in rare instances, it can present as leukocytoclastic vasculitis owing to the effect of circulating immune complexes and micro-emboli on the vascular endothelium. A high index of suspicion needs to be maintained to differentiate between infectious vs noninfectious autoimmune vasculitides, keeping in mind that missing a diagnosis can have fatal results. In this case report, we introduce a young female patient who initially presented with a picture of idiopathic autoimmune cutaneous vasculitis delaying the diagnosis of an und

[A rare cause of Pierre Marie hypertrophic osteoarthropathy: subacute infectious endocarditis].

Secondary hypertrophic osteoarthropathy occurred in a patient with subacute endocarditis. Chest x-ray in this smoker with ethylic cirrhosis showed a pulmonary opacity. Clinical signs of osteoarthropathic inflammation resolved with antibiotics before surgical cure of the aortic insufficiency. The diagnosis was retained on the basis of outcome after antibiotic therapy and the absence of any other etiology, notably bronchogenic cancer. Endocarditis or infectious endarteritis should be entertained in case of hypertrophic osteoarthropathy in patients with an infectious syndrome. Pathogenic hypothes

Patent ductus arteriosus presenting in old age.

This report describes an unusual case of patent ductus arteriosus, presenting in old age. The patient is the oldest living female with patient ductus reported thus far in the medical literature. She initially presented with chest pressure and dyspnea, and subsequently developed subacute bacterial endocarditis and typical angina pectoris with ECG changes, but with normal coronary anatomy.

Infective endocarditis: a consumptive disease among the elderly.

The clinical presentation of infective endocarditis varies according to the etiologic agent and the host. In elderly individuals, infective endocarditis can be difficult to diagnose and poses a challenge for the physician. The course of subacute infective endocarditis is indolent, and the onset of cardiac structural lesion is slow and gradual. In elderly patients, anemia and weight loss are occasionally the only or the most striking symptoms. In such cases, the clinical reasoning process leads to a hypothesis of wasting syndrome or neoplastic disease, especially when there is no fever. We repo

Endocarditis due to Cardiobacterium hominis.

A case of subacute bacterial endocarditis was caused by Cardiobacterium hominis in a 55-year-old man who had a history of rheumatic fever. A survey of the literature showed few reports dealing with the isolation of these organisms from human sources. Microbiologically, the chief distinguishing features of C. hominis are its characteristic colonial morphotype, a positive reaction, and its production of indole.

[Endocarditis caused by Coxilla burnetii].

Q-fever is antropozoonosis caused by Coxiella burnetii. The microorganism usually transmitted to humans through the inhalation of infected dust from sub clinical infected mammals. The most common reservoirs for Coxiella burnetii are large numbers domestic and wild animals. The illness can occur by farmers or slaughterhouses, or veterinarian and laboratories workers. In this report we presented case of endocarditic caused by Coxiella burnetii. The patient is driver 43 years old, who lived in the countryside Nahorevo. He hospitalized in May, 2002. He had symptoms of hepatitis, with elevation of

Subacute Infective Endocarditis Misdiagnosed as Polymyalgia Rheumatica: A Case Report.

Infective endocarditis (IE) is a rare disease with a myriad of signs and symptoms that overlap with other systemic inflammatory disorders. We present the case of an otherwise healthy 57-year-old male patient with a three-month history of back pain, muscle weakness, and weight loss that was initially misdiagnosed as polymyalgia rheumatica (PMR), when in fact he had been suffering from subacute IE due to viridans streptococci. The patient's diagnosis was delayed until there was clinically apparent valve destruction. Our case highlights the crucial importance of excluding clinically subacute bact
