HYPOTHESIS: The study was designed to investigate whether endolymphatic pressure exceeds perilymphatic pressure in an endolymphatic hydrops model with a partially functioning endolymphatic sac.
BACKGROUND: Previous investigations of perilymphatic and endolymphatic pressure measurements during endolymphatic hydrops were done in a classic endolymphatic hydrops model, with a surgically blocked endolymphatic duct and sac. This model, in contrast to the clinical situation of Meniere's patients, totally lacks the functional contribution of the endolymphatic sac.
METHODS: In the guinea pig, a parti…

Detailed information was obtained by questionnaires, office visits, chart reviews, and telephone calls from 171 of 179 consecutive patients who underwent endolymphatic sac surgery between 1976 and 1986. A comparison of surgical results was made. For patients followed more than 24 months, complete control of vertigo was obtained in 40 of 68 patients (59%) by endolymphatic subarachnoid shunt and in 27 of 60 patients (45%) who underwent a variety of sac incision operations. This difference achieved borderline statistical significance (p less than or equal to 0.057). Profound or total hearing loss…

INTRODUCTION: Endolymphatic sac tumor (ELST) is a rare neoplasm that exhibits aggressive growth primarily in the endolymphatic capsule and can potentially affect nearby neurovascular structures. The diagnosis of ELST poses challenges due to its low prevalence, gradual progression, and nonspecific symptomatology. It is currently believed that prompt surgical intervention is recommended for endolymphatic sac tumors upon diagnosis. In cases where the lesion is complex or in close proximity to vital blood vessels and nerves, making complete resection challenging, adjuvant therapy may be employed p…

PURPOSE: To evaluate objective vestibular function after endolymphatic sac surgery (ELSS) for Menière's disease (MD), using comparative vestibular function tests: videonystagmography (VNG), vestibular evoked myogenic potentials (VEMP) and video head-impulse test (VHIT) METHODS: Patients with definite MD using the American Academy of Otolaryngology-Head and Neck Surgery (AAO-HNS) of 1995 criteria modified in 2015 and treated with ELSS (sac decompression or sac opening) were included. The primary outcome was the preservation of vestibular function, comparing pre- and postoperative vestibular fun…

Endolymphatic sac tumor (ELST) is a rare neoplasm which is seldom evaluated by cytopathology. We report the clinicopathologic course and cytologic cerebrospinal fluid (CSF) findings in a 58-year-old patient with brainstem lesions who originally presented with vertigo but progressed to having left 7th, 8th, 9th, and 10th cranial nerve palsies, right-sided weakness, and occipital headaches. Cytospin of the CSF revealed large epithelioid cells similar to cells seen in a surgical resection of a brain mass three months previously. Review of the surgical specimen revealed a well-differentiated gland…

OBJECT: To determine if physiologically based MRI sequences can be used to detect endolymphatic sac tumor (ELST)-associated hydrops, the authors performed contrast-enhanced delayed FLAIR imaging in consecutive ELST patients with clinical findings consistent with hydrops.
METHODS: Consecutive patients with von Hippel-Lindau (VHL) disease and clinical findings of endolymphatic hydrops and ELSTs underwent contrast-enhanced delayed FLAIR MRI. Clinical, audiological, operative, and imaging findings were analyzed.
RESULTS: Three patients (2 male, 1 female) with 4 ELSTs (1 patient had bilateral ELS…

Posterior semicircular canal dehiscence is a rare otologic entity that presents with third window signs and symptoms. Petrous apex cholesteatoma, fibrous dysplasia, high riding jugular bulb, and eosinophilic granuloma have been reported to be associated with posterior semicircular canal dehiscence. Here we report a case of development of posterior semicircular canal dehiscence following an endolymphatic sac surgery for the first time.

OBJECT: Von Hippel-Lindau (VHL) disease is a hereditary multiple-neoplasia syndrome mapping to chromosome 3p25-26. Endolymphatic sac (ELS) tumors have been identified as a neoplastic manifestation of VHL disease. The purpose of this study was to evaluate comprehensively the natural history of inner ear disease in a large population of patients with confirmed or suspected VHL disease and to correlate the clinical features with the VHL genotype.
METHODS: The authors collated and analyzed clinical and genotypic data obtained in patients enrolled in an Institutional Review Board-approved protocol…

The human endolymphatic sac was analysed electron microscopically in patients undergoing acoustic Schwannoma surgery or vestibular nerve section. In addition, endolymphatic sacs from cadavers were analysed light microscopically. The results show that the human sac is endowed with a variable number of leucocytes and that there is a continuous recirculation of immuno-competent cells in this area of the inner ear that may be of importance for clearance of the inner ear from foreign substances and microorganisms derived from nearby located infection-prone areas. The possibility in Meniere's diseas…

Surgery of the endolymphatic sac (ELS) in classical Meniere's disease has fallen under attack and has been abandoned by some surgeons. We studied our results for vertigo and hearing in 83 patients undergoing surgery of the ELS for intractable classical Meniere's disease. Half of the patients were completely free of definitive attacks at 2 years; another quarter experienced substantial control. Three fourths noted an improvement in their level of disability. Results for vertigo were sustained at longer follow-up intervals. Nineteen percent had revision surgery (usually retrolabyrinthine vestibu…

The ultrastructure of the normal human endolymphatic sac (ES) has been observed by transmission electron microscopy (TEM). The role of the epithelium, the various regions of the subepithelial space and the general anatomy of the ES in pressure regulation were morphologically studied to generate testable hypotheses of human ES function. Light microscopic (LM) and TEM evidence of pressure regulatory mechanisms by endolymph resorption, mechanical factors, and secretory activity are presented. These mechanisms may be useful in designing experimental studies of the ES, and in interpretation of retr…

Principles of diagnosing and treating Meniere's disease using endolymphatic sac enhancement are described. Based on a current understanding of the natural history and pathogenesis of this disease, the terminology, diagnosis, and thereby are discussed. Techniques (methods) and surgical objectives of endolymphatic sac enhancement are designed to counteract the pathogenesis (endolymph malabsorption). Current surgical methods and results are presented for 100 patients selected at random from a larger group. Discussion of this method of treatment and reporting criteria follow.

Endolymphatic sac tumor (ELST) is a group of low-grade malignant tumors originating from the endolymphatic sac of the inner ear. It is rare in the clinic and has the biological characteristics of slow growth and local aggression. Due to the lack of specificity in the clinical manifestations of patients with ELST, many cases have entered the advanced stage at the time of diagnosis. However, there are still great challenges in the treatment of advanced ELSTs. Here, the authors describe a case of advanced ELST, which relapsed after 2 operations. This time, the authors chose the transotic approach…

BACKGROUND: Outcome of endolymphatic sac shunt surgery crucially depends on endolymphatic sac morphology. The intraoperative finding of a fibrotic sac, e.g., will lead to therapy-failure. Aim of the present study was to determine possible developmental changes in endolymphatic sac morphology which could imply, that a shunt operation should not be undertaken in older age.
METHOD: 46 histological temporal bone serial sections were examined. All sections were taken from the "Wittmaack Temporal Bone Collection". All specimens were obtained from patients with morphologically healthy ears, especial…
