Neonatal Perforated Appendicitis Masquerading as Necrotizing Enterocolitis.

A preterm neonate underwent emergent laparotomy for presumed necrotizing enterocolitis (NEC). Intra-operatively, neonatal perforated appendicitis (NPA) was encountered. This may represent a form of NEC localized to the appendix. A high index of clinical suspicion and early laparotomy are recommended.

Maternal risk factors for neonatal necrotizing enterocolitis.

OBJECTIVE: This study aimed to investigate the relationship between maternal hypertensive disease and other risk factors and the neonatal development of necrotizing enterocolitis (NEC). METHODS: This was a retrospective case-control study of infants with NEC from 2008 to 2012. The primary exposure of interest was maternal hypertensive disease, which has been hypothesized to put infants at risk for NEC. Other variables collected included demographics, pregnancy complications, medications and neonatal hospital course. Data were abstracted from medical records. RESULTS: Twenty-eight cases of si

Rectosigmoid pHi monitoring during experimental necrotizing enterocolitis.

BACKGROUND/PURPOSE: Gastric or rectosigmoid intramural pH (pHi) is considered a reliable indicator of splanchnic perfusion. The aim of this study was to evaluate whether rectosigmoid pHi reflects the severity of bowel damage in experimental necrotizing enterocolitis (NEC). METHODS: A total of 36 neonatal piglets, (median age, 3; range, 1 to 11 days; median weight, 2.5; range, 1.2 to 3.8 kg), were anesthetized, ventilated mechanically, and had invasive monitoring of hemodynamics. A sigmoid tonometer was inserted into the rectosigmoid colon. Enterocolitis was induced in 27 piglets by intralumin

[Necrotizing enterocolitis: a 12-year retrospective study].

UNLABELLED: Necrotizing enterocolitis (NEC) is the most relevant intestinal acquired complication during the neonatal period. Due to the improvements in perinatal medicine during the last decade, we wanted to work out possible differences in the incidence, diagnosis and clinical courses of NEC during a 12 years period. PATIENTS AND METHODS: All premature or term newborns were eligible for the study, if a necrotizing enterocolitis > or = stage 2a according to Bell was diagnosed between January 1980-December 1991. RESULTS: During the study period, 90 preterm or term newborns were treated for n

Endotoxinemia and thrombocytopenia during neonatal necrotizing enterocolitis.

Thrombocytopenia frequently complicates neonatal necrotizing enterocolitis (NEC) and has been postulated to result from absorption of bacterial endotoxins from the injured gut. The authors tested blood obtained during 47 episodes of NEC for endotoxin-like activity (ELA), using a Limulus amoebocyte lysate assay and found 23 patients (49%) had positive results. Concentrations of ELA in plasma ranged from 0.26 to 300 ng/mL of Escherichia coli equivalent activity, with a geometric mean of 1.1 ng/mL. Serial platelet measurements were available from 40 infants, 11 (28%) of whom had nadir counts belo

Neonatal necrotizing enterocolitis. Clinical and radiological features.

Necrotizing enterocolitis is an uncommon but dangerous disease in premature infants. Ten cases, seen over a three-year period at the Stanford University Medical Center, represented an incidence of 0.4 percent. The patients, six of whom died, derived from a general population, in contrast to the large series of patients reported in the literature in which the incidence was from 0.9 percent to 3.7 percent.(3-6)The initial symptoms-rapid respiration, periodic breathing, lethargy and irritability-were identical to those which occurred in numerous infants who had respiratory disease. Subsequent sym

Rare case of disseminated neonatal zygomycosis mimicking necrotizing enterocolitis with necrotizing fasciitis.

A set of monochorionic male twins presented with intestinal perforation. The smaller twin was diagnosed with necrotizing enterocolitis followed by sepsis, disseminated intravascular coagulation, and necrotizing fasciitis of the abdominal wall. The infant died on the fourth day after surgery, 16 days after birth. Surgical specimens and autopsy revealed a disseminated zygomycotic infection. Gastrointestinal zygomycosis followed by necrotizing fasciitis in premature infants is a rare condition and mimics necrotizing enterocolitis clinically. Necrotizing fasciitis after gastrointestinal zygomycosi

Donor Milk: Practical Considerations for Necrotizing Enterocolitis Reduction.

The practical considerations for using pasteurized donor human milk (PDHM) require a standardized yet individualized approach. Mother's own milk should be prioritized with PDHM serving as a bridge. Neonatal intensive care unit should focus on early feeding and a base diet of human milk MOM supplemented with PDHM. Human milk is critical to reducing necrotizing enterocolitis risk in very preterm infants. Given variability in PDHM composition, strategic fortification is necessary to optimize growth. Reliable milk handling, staff and family education, and shared decision-making with parents are es

Recent Discoveries of Mechanisms Contributing to Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) is a devastating neonatal intestinal disease driven by complex interactions between prematurity, microbial dysbiosis, immune dysregulation, and impaired intestinal barrier function. This article highlights recent advances in NEC pathogenesis, focusing on the role of toll-like receptor 4-mediated inflammatory signaling and the contribution of maternal inflammation and chorioamnionitis to disease susceptibility. Transcriptomic and spatial profiling studies further demonstrate that classical NEC and cardiac NEC possess distinct molecular signatures and identify div

Is Necrotizing Enterocolitis Totalis a Distinct Disease Process?

PURPOSE: This study aimed to characterize the clinical features of necrotizing enterocolitis totalis (NEC-T) in infants compared with NEC non-totalis (NEC-non-T) and to identify associated risk factors, thereby providing insights into its management for clinicians and parents. METHODS: A retrospective study was conducted at the Guangzhou Women and Children's Medical Centre and included infants with Bell's Stage II or III NEC from January 2015 to March 2021. Infants were divided into NEC-T and NEC-non-T groups based on intraoperative findings. Clinical data were collected and compared between

Hepatoblastoma with neonatal necrotizing enterocolitis: Two case reports.

We report two children with hepatoblastoma (HB) with a history of neonatal necrotizing enterocolitis (NEC). Case 1 was diagnosed with HB at 5 months of age. Liver enlargement was found during the NEC operation at 3 months of age and then was clinically diagnosed by imaging. After six chemotherapy courses, a partial hepatectomy was performed. Three months after ceasing the chemotherapy, a chest computed tomography scan suggested that distant metastasis of the tumor should be considered, and the lesion was removed. However, 9 months after the operation, alpha-fetoprotein concentrations were incr

Endoscopy in a Pediatric Patient After Necrotizing Enterocolitis.

Necrotizing enterocolitis (NEC) is a gastrointestinal condition usually found in premature neonates. Our case involves a full-term, 3-month-old male who was found to have pneumatosis after surgical repair of congenital cardiac defects. After cessation of enteral feeds, nasogastric tube decompression, and broad-spectrum antibiotics, breast milk was reintroduced 8 days after his procedure. Hematochezia developed, but repeat abdominal X-rays were normal with benign abdominal examinations, stable vital signs, and improved laboratory findings. Although feeds were slowly restarted with an amino acid

Intestinal atresia and necrotizing enterocolitis: Embryology and anatomy.

The primitive gut originates at week 3 of gestation from the endoderm, with posterior incorporation of the remaining embryo layers. Wnt, Notch and TLR4 pathways have been shown to play central roles in the correct development of the intestine. The classical hypothesis for intestinal atresia development consists of failure in bowel recanalization or a vascular accident with secondary bowel reabsorption. These have been challenged due to the high frequency of associated malformations, and furthermore, with the discovery of molecular pathways and genes involved in bowel formation and correlated d

Necrotizing Enterocolitis Following Pediatric Living Donor Liver Transplant.

Necrotizing enterocolitis is one of the most common and worrying diseases in neonates, commonly shown in premature neonates, and is associated with significant mortality and morbidity. Necrotizing enterocolitis is characterized by intestinal mucosal injury that can progress to transmural bowel necrosis, and radiologically it can present with either pneumatosis intestinalis or portal venous gas. It is postulated to develop in an immunocompromised host in the setting of bacterial colonization, usually after administration of non-breast milk feed. Cow's milk allergy association with necrotizing e
