Acute epiglottitis in adults.

Epiglottitis in adults, while uncommon, is certainly not rare. Despite the increasing reports of the disorder in the literature, it continues to be misdiagnosed in emergency departments. The first step in the diagnosis of acute epiglottitis is for the emergency physician to think of it. The literature concerning the etiology, pathology, and the management of epiglottitis in adults is reviewed.

Acute epiglottitis in adults.

Fourteen cases of adult acute epiglottitis with their clinical manifestations are presented. The history, etiology, pathophysiology, and diagnosis of the disease are reviewed. The controversies of management are discussed and a concise management protocol is outlined.

Acute epiglottitis in children.

Hemophilus influenzae type B (30 cases), untyped (two cases), and type A (one case), and H parainfluenzae (one case) were recovered in blood cultures from 44 cultures of 53 children with acute epiglottitis. These patients were managed by observation, tracheotomy, or intubation, with intubation being the most satisfactory. Both ampicillin and chloramphenicol have been employed recently because of the emergence of ampicillin resistance, which was 18.7% of all cases in 1977.

Acute epiglottitis in adults.

Epiglottitis occurs more often in adults than is generally recognized. Six adults with epiglottitis were treated at Evanston (III) Hospital from March to August 1978. Two required tracheostomy because of delayed diagnosis. There were no deaths. This experience suggests that any patient with acute, painful dysphagia should have indirect laryngoscopy or a carefully interpreted lateral roentgenogram to the neck to rule out epiglottitis.

Management of acute epiglottitis.

Airway obstruction secondary to acute epiglottitis is a dramatic, life-threatening emergency. Early diagnosis and provision of an adequate artificial airway are critical in the management of these patients. Tracheostomy has been the traditional method of securing the airway in acute epiglottitis, but more recently, endotracheal intubation has been advocated. Whichever method of airway management is preferred, it is imperative that every hospital have a protocol for the management of acute epiglottitis so that immediate action can be taken.

Acute epiglottitis in adults.

Acute epiglottitis in adults is a serious, often rapidly fatal disease, unless diagnosed promptly. Any adult with pain the the throat out of proportion to the visible signs of pharyngitis should have a right and left lateral soft tissue radiograph of the neck and/or indirect laryngoscopy. When expertly interpreted, the roentgenograms can be of inestimable value. Appropriate therapy, including antibiotics and possibly a tracheostomy, should be started immediately.

Prediction of Epiglottic Collapse in Obstructive Sleep Apnea Patients: Epiglottic Length.

OBJECTIVE: This study aims to explore the factors that contribute to epiglottic collapse (EC) in patients with obstructive sleep apnea (OSA). METHODS: This study enrolled 35 patients (34 males; median age, 39 years; median apnea-hypopnea index (AHI), 55.4 events/h; median body mass index (BMI), 26.9 kg/m2). EC (epiglottis attaching onto the posterior pharyngeal wall) was diagnosed by drug-induced sleep computed tomography (DI-SCT). Three dimensions were assessed for comparison between the EC and non-EC (NEC) groups that included anatomical measurement: epiglottic length and angle, endoscopic

Epiglottic cyst in an infant.

Epiglottic cyst is a rare cause of stridor and respiratory distress in newborns and infants. A 2-year-old girl was referred to our department for the treatment of an epiglottic cyst causing inspiratory stridor. Flexible fiberoptic laryngoscopy and a computed tomography (CT) scan revealed a cystic lesion on the lingual surface of the epiglottis. Frequent episodes of sleep apnea accompanied by desaturation had been observed during her sleep. Endoscopic deroofing was performed under general anesthesia. After the operation, stridor and sleep apnea disappeared.
