Religious freedom, reproductive health care, and hospital mergers.

This article examines the impact of hospital mergers and the formation of health care networks involving religious hospitals on the provision of reproductive health care. Although instances of access to such services being curtailed at non-Catholic religious facilities have been reported, no systematic study of hospitals owned by other religious denominations has yet been done. Accordingly, the author focuses on Roman Catholic institutions and policies. Efforts by Roman Catholic Bishops to enforce the Ethical and Religious Directives for Catholic Health Care Services on new institutional partn

Layered moral distress in end-of-life nursing under occupation: A hermeneutic inquiry.

BackgroundNurses providing end-of-life care in conflict-affected settings navigate cultural traditions, professional duties, and structural violence. Existing frameworks address cultural and institutional dimensions separately but do not account for their simultaneous interaction under protracted conflict.AimThis study explored how nurses' beliefs, intersecting with structural conditions, shape end-of-life care in a conflict-affected setting.Research designA Gadamerian hermeneutic inquiry employed semi-structured interviews with dialogical openness. Data were analyzed using hermeneutic princip

Ethical, clinical, and legal challenges of mental health care in prisons: between constraints and clinical integrity.

BACKGROUND: Prison populations experience disproportionately high rates of mental disorders and suicidality, often receiving inadequate care in settings primarily designed for punishment rather than treatment. Prisons, increasingly serving as de facto psychiatric institutions, present distinct ethical, legal, and clinical challenges for mental health professionals. OBJECTIVES: This paper explores the ethical dilemmas in correctional mental health care, focusing on confidentiality, the principle of equivalence of care, and the dual role of clinicians. It critically examines clinical practices

Veritas amid Post-truth: Catholic Ethics and the Moral Conditions of Philippine Public Health.

Post-truth presents a challenge to public health not only through misinformation, but also through the erosion of truth as a norm of public reasoning, communication, and institutional accountability. In the Philippine context, this problem intersects with religious culture, where Catholic beliefs, practices, institutions, and digital communities can shape how health claims are interpreted and whom communities regard as trustworthy. This philosophical article employs conceptual and normative analysis to examine the relationship among post-truth, public-health trust, Philippine Catholic religiou

Clinical Ethics in Name Only? The Case for Moral Realism at the Bedside.

As Clinical Ethics Consultation (CEC) becomes more institutionalized, concerns about its legitimacy and efficacy have grown. We defend a realist orientation, according to which CEC has normative force only if its recommendations answer to stance-independent reasons, not to individual preferences and institutional policies. Public justification can and should proceed through familiar means (e.g., shared mid-level norms and transparent procedures) so that stakeholders can scrutinize the reasoning. We show how neutrality, admirable as a procedural virtue, hollows CEC when treated as a master idea

The licitness (according to Roman Catholic premises) of inducing the non-viable anencephalic fetus: reflections on Professor Drane's policy proposals.

James Drane's policy proposals for Roman Catholic hospitals, which would allow induction of a firmly diagnosed anencephalic fetus upon the mother's considered request, are defended. Drane's defense of his proposals, by focussing upon the empirical facts of anencephaly and attempting to abstract from the question of whether or not the anencephalic is a potential person, is held to be untenable. However, examination of the Roman Catholic proscription against abortion shows that it applies to humans who are, or might be, persons or potential persons. And the facts of anencephaly show that the ane

Clinical Ethics Consultation for Patients Impacted by Incarceration: A Single Center Retrospective Review.

Health care professionals and patients impacted by incarceration face unique medical, legal, and ethical issues. The frequency and nature of ethics consultations for these patients are underexplored. This study aimed to characterize the primary ethical issue and contextual features of ethics consultations involving patients impacted by incarceration. We conducted a qualitative concept content analysis of ethics consultations involving patients impacted by incarceration and calculated descriptive statistics of demographics to compare these patients with the broader population of patients impact

Ethical AI in healthcare: insights from European policy discourses.

BACKGROUND: The rapid integration of Artificial Intelligence (AI) into healthcare presents unprecedented opportunities for innovation in diagnostics, treatment and patient care. However, this paradigm shift is accompanied by a concomitant series of critical regulatory and ethical challenges. Relying on the pillars of the New Institutional Theory, this study undertakes a comprehensive examination of how ethical AI in healthcare is discursively constructed in European Commission’s (EC) policy communications and how institutional narratives shape emerging governance frameworks. METHODS: A multi-

Leading the transition to ethical human body sourcing in Africa: The South African experience.

BACKGROUND: The use of human bodies for anatomy education and research forms an integral part of the training of health professionals around the world. However, the ethical acquisition of human remains for this purpose has been a challenge in many countries, particularly for those on the African continent. South African institutions have however, been able to progressively transition to a more ethical approach to human body acquisition. The aim of the current study was to investigate the provenance of human bodies and the number used in South African health sciences institutions during the per

Ethical considerations in pediatric dysphagia.

Speech-language pathologists (SLPs) are bound by ethical codes that reflect professional and institutional commitments. When professional activities involve working with children with dysphagia and other complex medical issues, SLPs frequently must make judgments that have bioethical implications. This article reviews general ethical theories and principles, examines SLPs' ethical commitments, and presents an approach for ethical decision making and reasoning. Case studies of children with dysphagia are presented to illustrate ethical dilemmas.

Use of a penicillin allergy clinical decision rule to enable direct oral penicillin provocation: an international multicentre randomised control trial in an adult population (PALACE): study protocol.

INTRODUCTION: Penicillin allergies are highly prevalent in the healthcare setting and associated with the prescription of second-line inferior antibiotics. More than 85% of all penicillin allergy labels can be removed by skin testing and 96%-99% of low-risk penicillin allergy labels can be removed by direct oral challenge. An internally and externally validated clinical assessment tool for penicillin allergy, PEN-FAST, can identify a low-risk penicillin allergy without the need for skin testing; a score of less than 3 has a negative predictive value of 96.3% (95% CI, 94.1 to 97.8) for the pres

Relational-temporal dignity at the end of life: Ethical foundations for the DiRePal model in the algorithmic age.

OBJECTIVES: The increasing presence of artificial intelligence (AI), electronic patient-reported outcomes (ePROMs), and digital infrastructures in palliative care is transforming how clinical encounters are organized and how suffering is interpreted. These technological shifts heighten the risk of relational compression and a reduction of dignity to measurable outputs. This paper proposes the DiRePal model (Relational-Temporal Dignity in Palliative Care) as a philosophical framework to re-examine dignity beyond coherent narrative identity or autonomy-centered ethics, emphasizing relational pre

Exploratory randomised trial of face-to-face and mobile phone counselling against usual care for tobacco cessation in Indian primary care: a randomised controlled trial protocol for project CERTAIN.

INTRODUCTION: Despite widespread use of smokeless tobacco products by people within the Indian subcontinent, there is little awareness among Indians of its health hazards when compared with smoked tobacco. We hypothesise that mobile phone counselling will be feasible and effective for smokeless tobacco cessation intervention in India. This paper presents the protocol of the development and conduct of an exploratory trial before progression to a full randomised controlled trial. METHODS AND ANALYSIS: An exploratory randomised controlled trial will be conducted in urban primary health centres i

Filling the Network Gap in Research Ethics: Analyzing Ethical Issues at Scale in Big Team Science.

Scientific research increasingly involves large, multidisciplinary teams networked across multiple institutions to develop new technologies. Despite the rise of complex research networks and big team science, there has been too little analysis to date of the ethical challenges facing these networks. The extensive literature on the ethical issues confronting individual researchers and small teams (the microlevel) and on the larger societal challenges flowing from research and new technology (the macrolevel) leaves a troubling gap in between, at the mesolevel of the research network involved in

A Walker and Avant concept analysis of slow code practice in end-of-life resuscitation.

BACKGROUND: The slow code is a complex and ethically sensitive phenomenon in cardiopulmonary resuscitation (CPR), referring to a deliberately partial or delayed resuscitation attempt in patients with poor prognosis or terminal conditions. Although it is recognized in clinical practice, the concept remains ambiguous and ethically controversial. Clarifying its defining attributes, antecedents, and consequences is essential for ensuring ethical and responsible end-of-life decision-making in healthcare. METHODS: This study applied Walker and Avant's (2019) concept analysis method to clarify the c

Determining the long-term health burden and risk of sequelae for 14 foodborne infections in British Columbia, Canada: protocol for a retrospective population-based cohort study.

INTRODUCTION: Over one in eight Canadians is affected by a foodborne infection annually; however, the long-term consequences, including the risks and costs of sequelae, are unclear. We aim to estimate the health burden and direct costs of 14 infections commonly transmitted by food, considering the acute illness and subsequent sequelae and mortality, for the population of British Columbia, Canada (~4.7 million). METHODS AND ANALYSIS: We will conduct a population-based retrospective cohort study of the British Columbia provincial population, over a 10-year study period (1 January 2005 to 31 Dec

Ethical issues in HIV/AIDS infections.

BACKGROUND: Human relationships are guided by codes of ethics, written or otherwise. The reality and the place of universal standards of ethics may be argued but local variations and interpretations of these universal standards do occur. METHOD: A search of some relevant articles on ethical issues on HIV/AIDS was undertaken through the medline and standard texts. RESULTS: Unlike any other disease HIV/AIDS infection has resulted in large amount of legal/ethical commentaries. For the most parts of the world HIV/AIDS issues have produced metamorphosing legal/ethical issues notably, the concern
