Cemento-ossifying fibroma involving paranasal sinuses and skull base.

INTRODUCTION: Ossifying fibroma is a rare benign fibro-osseous lesion that usually affects mandible and maxillary bone. Their localisation to paranasal sinus and skull base is uncommon. MATERIAL AND METHOD: We report a huge recurrence of ossifying fibroma of the ethmoid paranasal sinus involving the skull base. CASE REPORT: Ten years after the removal of ossifying of the ethmoid 34 year old man presented headache with diplopia. Computed tomography (CT) and magnetic resonance imagery (MRI) showed a recurrent tumour witch extended to the nasal cavity, the sphenoid and the posterior ethmoid sin

[Effectiveness of the treatment of frontal sinus inflammation and its prevention].

The analysis of intraoperative data obtained at correction of ethmoid bone structure in 130 patients with frontal sinusitis including the removed tissue histology shows that the severity of ostiomeatal complex deformation correlates with actual advance of the destructive alterations in the ethmoid labyrinth by the frontal sinus. Removal of necrobiotic tissues adjacent to the frontal sinus is pathogenetically valid and the most effective treatment of frontal sinusitis which has reduced the number of recurrences to 3.5%.

Osteoblastoma of the ethmoid sinus.

An osteoblastoma is a benign bone tumor that most often occurs in the vertebral column and the long bones of the extremities. Paranasal sinus involvement is very rare. We report an osteoblastoma of the ethmoid sinus in a 13-year-old boy who presented with exophthalmos. CT demonstrated an expansile, lesion of the right ethmoid sinus, which caused the orbital contents to be compressed and deviated to the right. A mixture of ground glass opacity and dense bone was present. The less dense portions of the lesion showed intense enhancement on MRI; a signal void was present on all sequences in the de

[Case report: Cemento-ossifying fibroma of the ethmoidal sinus presenting with exophthalmos].

Cemento-ossifying fibroma is a benign fibro-osseous tumor which contains fibrous tissue and varying amounts of calcified tissue resembling bone, cementum or both. The tumor is frequently seen in the mandibula and maxilla, but it may rarely affect the ethmoidal sinus. In this report, CT findings of cemento-ossifying fibroma located in the ethmoidal sinus are presented.

How we do it: using the nasion as a landmark for identifying the anterior ethmoidal artery.

BACKGROUND: In severe refractory epistaxis, the anterior ethmoidal artery may need to be ligated. Previously described endonasal or transorbital approaches are not always effective, or they have suboptimal aesthetic outcomes. This paper describes a safe and effective surgical technique, with a consistent landmark allowing quick identification. TECHNICAL DESCRIPTION: A transcaruncular incision is made, oriented medially in the direction of the medial orbital wall towards the level of the nasion. Once onto bone, a subperiosteal plane is developed and an endoscope is used to dissect posteriorly

Cranial fasciitis of the ethmoid: a rare mimic of pediatric intracranial malignancy.

Cranial fasciitis is a rare pediatric skull tumor with a challenging radiological diagnosis, particularly when it arises in unusual locations or presents with atypical features. We report a case of cranial fasciitis originating from the ethmoid bone in a 17-month-old girl, presenting as isolated facial asymmetry instead of the usual scalp mass. Cross-sectional and metabolic imaging demonstrated an aggressive-appearing ethmoid sinus mass mimicking rhabdomyosarcoma, with extensive bone destruction and intracranial extension. The patient underwent complete surgical resection of the lesion. This r…

Preservation of continence in radical prostatectomy patients: a laparoscopic surgeon's perspective.

BACKGROUND: The knowledge of sinonasal vasculature is inevitable in transnasal neurosurgery. We performed an anatomical study on the sphenop … ony structure bordering the sphenopalatine foramen anteriorly which is built up by the palatine and ethmoid bone as well as the maxilla. According to the radiographic studies, this triangular prominence is surrounded superiorly by a posterior ethmoid cell (57.4%), the sphenoid sinus (41.7%) or the orbit (0.9%) with a varying contribution of the superior nasal …

In-utero Diagnosis of Double Encephalocele - Imaging Features and Review of Literature.

Encephalocele is protrusion of brain parenchyma through a defect in the cranium. It is classified into various types based on the defect location: sincipital (fronto-ethmoidal), basal (trans-sphenoidal, spheno-ethmoidal, trans-ethmoidal, and spheno-orbital), occipital and parietal. Double encephaloceles are very rare with only a handful of cases reported in the literature and most of these cases involved either occipital or sub-occipital region. All, except one, cases of double encephaloceles were diagnosed postnatally. We present a case of double encephalocele with parietal and occipital comp

Sinolith in the ethmoid sinus.

We report a case of sinolith in the left ethmoid sinus of a 61-year-old man. The patient complained of nasal obstruction. Computed tomography revealed a small, calcified mass associated with a nasal polyp in the left ethmoid sinus. The antrolith and polyp were removed via endoscopic sinus surgery. Histopathological analysis of the antrolith revealed it to be bone-like in formation. The antrolith was about 1 cm in diameter. Infrared spectroscopy revealed that the antrolith contained protein (45 per cent), calcium phosphate (43 per cent) and calcium carbonate (12 per cent).

[Fibrous dysplasia of the frontal and ethmoid sinuses: a case report].

Fibrous dysplasia is a genetic and rare bone disorder affecting the young subject. Its pathophysiology involves genetic mutations leading to defective skeletal development with fibrous and medullary proliferations. Cephalic extremity is involved in one third of cases. We here report a case of surgically treated fibrous dysplasia of the frontal and ethmoid sinuses. The study involved a 31-year-old man presenting with chronic holocranial headaches. Computed tomography (CT) scan showed voluminous bilateral hyperdense expansive process in the frontal and ethmoid sinuses. Surgery was performed on t

Use of titanium mesh for reconstruction of extensive defects in fronto-orbito-ethmoidal fracture.

Traumatic injuries treatment of the fronto-naso-orbito-ethmoidal region has been one of the most challenging treatments within maxillofacial surgery, particularly of extensive orbital defects, very common in this type of pathologic condition. A 48-year-old man involved in a car collision presented an extensive bilateral fracture of the orbit medial wall, nasal bones, the nasal septum, and the frontal anterior table. The clinical and tomographic findings concluded the diagnosis of a maxilla and fronto-naso-orbito-ethmoidal fracture. Among the variety of biomaterials, the titanium mesh was elect

Ethmoid sinus disease: CT evaluation in 400 cases. Part I. Nonsurgical patients.

The unique anatomy of the ethmoid sinuses makes it difficult to interpret "soft-tissue clouding," especially on plain films. The clinical significance of isolated or generalized soft-tissue disease within ethmoid cells is unclear, and the patterns of bone erosion or remodeling must be relied on in establishing a specific differential diagnosis. Of 400 patients studied, 186 had inflammatory disease and 214 had tumors. Squamous cell carcinomas, metastases, and a few aggressive sarcomas were found almost exclusively to have bone destruction, while 94% of the remaining neoplasms were characterized

[Clinico-anatomic study of the spina nasalis ossis frontalis and its role within the scope of frontal sinus surgical approach].

BACKGROUND: During endonasal frontal sinusotomy using the sharp spoon (endonasal frontal sinus surgery type II according to Draf or May and Schaitkin) a solid piece of bone is frequently encountered anterior to the neo-ostium. This bone may be referred to as a "nasal spine". A prominent spine may render a sinusotomy difficult or even impossible. METHODS: A maximum endonasal frontal sinusotomy was performed on 36 anatomical specimens by means of a sharp spoon producing neo-ostia of 7 x 5 mm on average. The dimensions of the remaining nasal spine were measured subsequently together with the dia
