Sinobronchial Syndrome Patients with Suspected Non-Tuberculous Mycobacterium Infection Exacerbated by Exophiala dermatitidis Infection.

BACKGROUND: Exophiala dermatitidis is an environmental black fungus that rarely causes respiratory infections, yet its pathophysiological features and treatment regimens have not been established. CASE SERIES: Two cases of exacerbations of chronic bronchitis and sinusitis due to E. dermatitidis infection in Japan are presented. Both patients were women, and non-tuberculous Mycobacterium (NTM) infection was suspected based on chest radiological findings, but E. dermatitidis was detected from bronchial lavage fluid and nasal mucus, respectively. Both cases were successfully treated by antifunga…

A novel case of feline Exophiala spinifera disease in Americas: review integrating diagnosis and clinical.

We report the first case of phaeohyphomycosis caused by the dematiaceous dimorphic fungus Exophiala spinifera in a free-roaming domestic cat from the Americas, specifically diagnosed in the Central-West Region of Brazil, which progressed to euthanasia due to severe clinical deterioration. A comprehensive diagnostic approach was employed, integrating epidemiological, clinical, laboratory, mycological, and molecular analyses to confirm the fungal infection. In addition to the fungal infection, the cat tested seropositive for feline immunodeficiency virus (FIV) and was suspected of having seromuc…

In Vitro and In Vivo Evaluation of Synergistic Effects of Everolimus in Combination with Antifungal Agents on Exophiala dermatitidis.

To investigate the combined function of the novel oral mTOR inhibitor, everolimus, with antifungal agents and their potential mechanisms against Exophiala dermatitidis, the CLSI microliquid-based dilution method M38-A2, chequerboard technique, and disk diffusion testing were performed. The efficacy of everolimus was evaluated in combination with itraconazole, voriconazole, posaconazole, and amphotericin B against 16 clinically isolated strains of E. dermatitidis. The synergistic effect was determined by measuring the MIC and fractional inhibitory concentration index. Dihydrorhodamine 123 was u…

A fatal case of Exophiala dermatitidis meningoencephalitis in an immunocompetent host: A case report and literature review.

BACKGROUND: Central nervous system (CNS) infection due to Exophiala dermatitidis is rare and fatal, and primarily reported in immunocompromised patients or those with caspase recruitment domain-containing protein 9 deficiency. Herein, we describe a case of an otherwise healthy person (without underlying disease or gene deficiency) diagnosed with Exophiala dermatitidis meningoencephalitis. The patient achieved clinical remission under high-dose antifungal therapy in the first 14 months but died after 2 years of the therapy. CASE PRESENTATION: A 15-year-old student with headache and fever was a…

A Cautionary Tale of Exophiala spinifera Infection in Two Cats: Case Reports and Literature Review.

This case series reports two independent cases of Exophiala spinifera infection in adult male neutered domestic cats, both referred following misdiagnosis. To date, only six cases associated with this organism have been reported in domestic cats, excluding those described herein. These also represent the first documented cases of E. spinifera infection in cats in the United States. In both cases, a definitive etiologic diagnosis could not be made by cytology, histology, or fungal culture. Moreover, histologic features did not allow for clear classification of the lesions as phaeohyphomycosis o…

Diagnosis and clinical management of Exophiala dermatitidis pneumonia in a patient with anorexia nervosa: A case report.

We report a patient with anorexia nervosa without bronchiectasis and cystic fibrosis who developed acute pneumonia caused by Exophiala dermatitidis (E. dermatitidis). The black fungus found in multiple sputum cultures was determined to be E. dermatitidis using mass spectrometry and identified using genetic analysis. Although the initiation of antifungal therapy was late, the pneumonia gradually improved with long-term treatment. This case highlights the need for early diagnosis and effective long-term treatment of the fungal etiologic agent.

Pulmonary phaeohyphomycosis due to Exophiala dermatitidis in a patient with pulmonary non-tuberculous mycobacterial infection.

A 65-year-old Japanese woman repeatedly withdrew and resumed antibiotics against pulmonary non-tuberculous mycobacterial infection caused by Mycobacterium intracellulare for more than 10 years. Although she continued to take medications, her respiratory symptoms and chest computed tomography indicated an enlarged infiltrative shadow in the lingular segment of the left lung that gradually worsened over the course of a year or more. Bronchoscopy was performed and mycobacterial culture of the bronchial lavage fluid was negative, whereas Exophiala dermatitidis was detected. After administration of…

Peritoneal dialysis-associated peritonitis caused by Exophiala spinifera: A case report and review of literature.

Exophiala spinifera is a black ascomycetous yeast and is responsible for phaeohyphomycosis. We provide the first case report of peritoneal dialysis (PD)-associated peritonitis in a female patient with progressive impairment of visual capacity. The infection was caused by a cutaneous infection of her hands. The patient responded well with PD catheter removal and 2-week antifungal medication. This case emphasizes the importance of hand hygiene and regular eye evaluation in preventing environment-bound infection in patients on PD. 2012 Elsevier Ltd. All rights reserved.

Tolerance and antioxidant response of a dark septate endophyte (DSE), Exophiala pisciphila, to cadmium stress.

The growth, oxidative damage and antioxidant response of Exophiala pisciphila ACCC32496, a dark septate endophyte isolated from an abandoned lead-zinc mining area, were measured at cadmium (Cd) concentrations of 0, 25, 50, 100, 200 and 400 mg L(-1). The EC50 values of E. pisciphila ACCC32496 to Cd were 332.2 mg L(-1) after 30 days on solid medium and 111.2 mg L(-1) after 7 days in liquid medium. Cd stress markedly stimulated the production of superoxide anion, H2O2 and malondialdehyde in the fungal mycelia. The activities of superoxide dismutase and catalase reached their maxima at 100 mg L(-1…

[Subcutaneous phaeohyphomycose due to Exophiala jeanselmir. Report of 3 cases in patients with a kidney transplant].

We report three cases of subcutaneous phaeohyphomycosis due to Exophiala jeanselmei (Langeron) McGinnis et Padhye 1977, in kidney transplant patients. Exophiala jeanselmei is a dematious fungus having also ability to rarely procedure eumycetoma (black grains). According to KWON-CHUNG & BENNETT (1992) such fungus is antigenically very heterogeneous, since so far three serotypes have been identified; each serotype including subgroups. Subcutaneous phaeohyphomycosis is becoming more and more frequent in kidney transplant patients submitted to an immunosuppressive treatment. As Exophiala jeanselme…

Clinical Perspective on Exophiala dermatitidis, A Systematic Review of Cases Reported Globally over 36 Years.

Exophiala dermatitidis is an opportunistic pathogen belonging to the so-called black yeasts, a group of fungi capable of causing a wide range of infections. Here we systematically reviewed 77 global cases of E. dermatitidis infections reported between 1989 and 2024. Most cases were reported from Japan (19/77), United States (16/77) and China (10/77), with 64.9% (50/77) cases reported since 2010, indicating an increasing global awareness of this fungus. Our results indicated that reported E. dermatitidis infections occurred in both immunocompromised (54.55%; 42/77) and apparently immunocompeten…

Phaeohyphomycosis due to Exophiala infections in solid organ transplant recipients: Case report and literature review.

This case report and literature review underscores the cutaneous presentations of phaeohyphomycosis in the solid organ transplant population. Increased cognizance with prompt identification is critical. The therapy and clinical outcomes of phaeohyphomycosis, caused by the Exophiala genus, in the solid organ transplant population, is analyzed to examine optimal care. This review highlights the inherent difficulties in providing the appropriate duration of antifungal therapy to avoid relapsing infections in immunosuppressed patients.

Cloning, expression, and characterization of Baeyer-Villiger monooxygenases from eukaryotic Exophiala jeanselmei strain KUFI-6N.

The fungus Exophiala jeanselmei strain KUFI-6N produces a unique cycloalkanone monooxygenase (ExCAMO) that displays an uncommon substrate spectrum of Baeyer-Villiger oxidation of 4-10-membered ring ketones. In this study, we aimed to identify and sequence the gene encoding ExCAMO from KUFI-6N and overexpress the gene in Escherichia coli. We found that the primary structure of ExCAMO is most closely related to the cycloalkanone monooxygenase from Cylindrocarpon radicicola ATCC 11011, with 54.2% amino acid identity. ExCAMO was functionally expressed in E. coli and its substrate spectrum and kine…
