Effectiveness of chest radiography, lung ultrasound and thoracic computed tomography in the diagnosis of congestive heart failure.

Hydrostatic pulmonary edema is as an abnormal increase in extravascular water secondary to elevated pressure in the pulmonary circulation, d … ructures and alveolar air, as it happens in case of thickened interlobular septa due to increase of extravascular lung water. The number, diffusion and intensity of B lines correlates with both the radiologic and invasive estimate of extravascular lung water. The integration of conventional chest radiograph with LUS can be very helpful to obtain the correct diagnosis. Com …

Modeling of Transport Mechanisms in the Respiratory System: Validation via Congestive Heart Failure Patients.

The heart and lungs are intricately related. For congestive heart failure patients, fluid (plasma) backs up into the pulmonary system. As a result, pulmonary capillary pressure increases, causing fluid to seep into the lungs (pulmonary edema) within minutes. This excess fluid induces extra stress during breathing that affects respiratory health. In this paper, we focus on the effect that high pulmonary capillary pressure has on the development of this extravascular lung water (EVLW). A mathematical model of pulmonary fluid and mass transport mechanisms is developed in order to quantitatively a…

Diagnostic value of transpulmonary thermodilution measurements for acute respiratory distress syndrome in a pig model of septic shock.

BACKGROUND: No direct approach assessing pulmonary vascular permeability exists in the current therapeutic strategy for patients with acute respiratory distress syndrome (ARDS). Transpulmonary thermodilution measures hemodynamic parameters such as pulmonary vascular permeability index and extravascular lung water, enabling clinicians to assess ARDS severity. The aim of this study is to explore a precise transpulmonary thermodilution-based criteria for quantifying the severity of lung injury using a clinically relevant septic-ARDS pig model. METHODS: Thirteen female pigs (weight: 31 ± 2 kg) we…

[Effect of various colloidal solutions on pulmonary oxygenizing function in patients with acute lung lesion].

The effects of 6% hydroxyethyl starch and 10% albumin solution on the parameters of central hemodynamics and pulmonary extravascular water were studied in acute lung lesion. The patients were divided into 2 groups that did not significantly differ by the baseline severity of a condition (APACHE II and Murray scales). Groups 1 and 2 patients were transfused 6% hydroxyethyl starch and 10% albumin solution, respectively. The "PICCO PLUS" system was used to determine the parameters of central hemodynamics, pulmonary extravascular water, and oxygenation before and after infusion of the test colloid

Polyethylene glycol-surfactant for lavage lung injury in rats.

Addition of ionic and nonionic water-soluble polymers to pulmonary surfactants in the presence of inactivating substances prevents surfactan … s after intravascular injections of (125)I-labeled albumin confirmed that addition of PEG increased extravascular lung water and that this effect is mitigated by furosemide. We conclude that surfactant + PEG mixtures are less effective in lavage injury than in other forms of lung injury because of increased extravascular lung water.

A century of exercise physiology: lung fluid balance during and following exercise.

PURPOSE: This review recalls the principles developed over a century to describe trans-capillary fluid exchanges concerning in particular the lung during exercise, a specific condition where dyspnea is a leading symptom, the question being whether this symptom simply relates to fatigue or also implies some degree of lung edema. METHOD: Data from experimental models of lung edema are recalled aiming to: (1) describe how extravascular lung water is strictly controlled by "safety factors" in physiological conditions, (2) consider how waning of "safety factors" inevitably leads to development of …

The radiographic detection of acute pulmonary oedema. A comparison of radiographic appearances, densitometry and lung water in dogs.

We have induced acute pulmonary oedema in upright anaesthetized dogs by increasing pulmonary microvascular permeability or by extracellular fluid volume overload in order to determine the sensitivity and specificity of the radiograph to the presence of abnormal lung water. Radiographs were taken before and after development of oedema. At the end of the experiment we removed the inflated lungs and froze them in liquid nitrogen and subsequently examined them macroscopically in the frozen state. The extravascular water/dry lung weight ratio was measured gravimetrically on seven portions of each l

Thermal dye double indicator dilution measurement of lung water in man: comparison with gravimetric measurements.

The thermal dye double indicator dilution technique for estimating lung water was compared with gravimetric analyses in nine human subjects who were organ donors. As observed in animal studies, the thermal dye measurement of extravascular thermal volume (EVTV) consistently overestimated gravimetric extravascular lung water (EVLW), the mean (SEM) difference being 3.43 (0.59) ml/kg. In eight of the nine subjects the EVTV -3.43 ml/kg would yield an estimate of EVLW that would be from 3.23 ml/kg under to 3.37 ml/kg over the actual value EVLW at the 95% confidence limits. Reproducibility, assessed …

Ultrafiltration during cardiopulmonary bypass: laboratory evaluation and initial clinical experience.

Ultrafiltration during crystalloid hemodilution cardiopulmonary bypass (CPB) was evaluated in two groups of mongrel dogs: in one group during 2 hours of CPB with the heart empty and beating and in the other during 90 minutes of cold cardioplegic arrest followed by 30 minutes of recovery. In both groups, the accumulation of extravascular lung water was less in the dogs undergoing ultrafiltration than in control animals. In 10 patients with clinical evidence of severe fluid overload, ultrafiltration was employed during CPB. The amount of fluid removed ranged from 1,700 to 6,100 ml (mean, 3,240 +…

Cardiopulmonary ultrasound correlates of pleural effusions in patients with congestive heart failure.

BACKGROUND: Pleural effusions are common in patients with congestive heart failure. However, there is a need to assess systematically the correlation between effusion volume, extravascular lung water and echocardiographic parameters. We used combined cardiopulmonary ultrasound to evaluate the relationship between effusion volume, extravascular lung water, and echocardiographic parameters in patients with congestive heart failure. METHODS: Patients who were hospitalized for congestive heart failure underwent combined cardiopulmonary ultrasound. A semiquantitative score of pleural effusions was…

Dispersal kinetics of deuterated water in the lungs and airways following mouth inhalation: real-time breath analysis by flowing afterglow mass spectrometry (FA-MS).

Pulmonary oedema is a medical condition characterized by abnormal accumulation of fluid in the extravascular space in the alveoli. Effective oxygenation is impaired and this leads to significant short- and long-term morbidity and mortality. The detection and monitoring of pulmonary oedema by measuring lung water volume is therefore crucial in the initiation and guidance of therapeutic intervention. The current gold standard bedside measurement of extravascular lung water volume (EVLW) is the dilution method using various indicators, but despite the good correlation of the results with those ob…

Exponential increases in pulmonary intravascular and extravascular fluid volumes by overperfusion with dextran 70 in anesthetized dogs.

In the lung overperfused by dextran 70, it is not known how intravascular blood volume and extravascular water volume increase as a function of pulmonary microvascular pressure. To determine their characteristics, we produced an overperfusion state (high pressure and high blood flow) by infusing dextran 70 (100 ml/kg over 30 min) into 15 anesthetized dogs. Using the thermal and dye dilution technique, we simultaneously estimated the pulmonary blood volume (PBV) and the extravascular lung thermal volume (EVLTV), and correlated mathematically these variables with pulmonary artery end-diastolic p

Role of longitudinal diffusion in the extravascular pulmonary space on parameter estimates derived from data of multiple indicator dilution.

A mathematical model of transcapillary exchange has been developed that considers in detail the role of axial diffusion in the extravascular tissue region on estimates of such physiological parameters as lung water (VE) and pulmonary capillary permeability-surface area products (PS), obtained from multiple indicator dilution studies. The experimental cases considered correspond to two animal models of pulmonary oedema in which the integrity of the pulmonary capillary membrane is disrupted and the effects of extravascular axial diffusion may be important. A novel feature of the computational sc

Nodules resembling arachnoid villi in pulmonary venules in plexogenic pulmonary arteriopathy.

We have found nodules histologically resembling arachnoid villi adjacent to pulmonary venules in a woman of 30 years with primary pulmonary hypertension. Such lesions have been reported previously in cases of mitral stenosis and pulmonary thromboembolism and we believe them to absorb excess water from the interstitial tissues of the alveolar walls, thus protecting the lung from the development of edema. In this way they seem to have a similar function to that of cerebral arachnoid villi and granulations which transport cerebrospinal fluid into the dural sinuses.

Second-order grey-scale texture analysis of pleural ultrasound images to differentiate acute respiratory distress syndrome and cardiogenic pulmonary edema.

Discriminating acute respiratory distress syndrome (ARDS) from acute cardiogenic pulmonary edema (CPE) may be challenging in critically ill patients. Aim of this study was to investigate if gray-level co-occurrence matrix (GLCM) analysis of lung ultrasound (LUS) images can differentiate ARDS from CPE. The study population consisted of critically ill patients admitted to intensive care unit (ICU) with acute respiratory failure and submitted to LUS and extravascular lung water monitoring, and of a healthy control group (HCG). A digital analysis of pleural line and subpleural space, based on the …
