BACKGROUND: To evaluate the safety, etiology and outcomes of patients undergoing bilateral thoracentesis.
METHODS: This is a prospective cohort study of 100 consecutive patients who underwent bilateral thoracenteses in an academic medical center from July 2009 through November 2010. Pleural fluid characteristics and etiologies of the effusions were assessed. Mean differences in levels of fluid characteristics between right and left lungs were tested. Associations between fluid characteristics and occurrence of bilateral malignant effusions were evaluated. The rate of pneumothorax and other co

The authors relate the data on the importance of a biochemical analysis of the pleural fluid for the differential diagnosis of pleurisy and identification of the early signs of pleural fluid suppuration. For the purposes of the differential diagnosis between transudates and exudates measurements are made of the protein content and LDH activity in the pleural fluid. The exudate protein/serum protein and exudate LDH/serum LDH ratios are computed in necessary cases. The high prothrombin index (over 100%) and the level of lysozyme (over 25 (micrograms/ml) in the pleural fluid provide evidence in f

Discharge from the ear can be the result of many disease processes. The ear may discharge blood, pus, cerebrospinal fluid (CSF) or wax. Keratosis obturans, stenosis of the external meatus and benign tumours of the external meatus all lead to wax build-up, which may cause recurrent attacks of otitis externa. Malignant tumours, such as basal cell carcinoma, squamous cell carcinoma and tumours of ceruminous gland origin may also present with discharge. Tumours should be excluded by submitting all material removed from the external canal for histological examination. Single or multiple abscesses (

The case reports of 2 patients with constrictive pericarditis and 1 patient with the Budd-Chiari syndrome are used to show that high-protein ascites may occur in the absence of disease of the peritoneum. Protein content should not be the only factor assessed in differentiating between transudative and exudative ascites.

Pleural effusions can be a manifestation of several nosological entities. Etiologic diagnosis involves a good clinical history, followed by thoracocentesis with pleural biopsy and eventually bronchoscopy or thoracoscopy. The differentiation between transudates and exudates, by the biochemical characteristics of the pleural effusions, can orientate the underlying disorder. It is known that there are more than 35 different etiological entities of exudative pleural effusions. However, pneumonia, malignancies, pulmonary embolism, abdominal disease and tuberculosis are the major causes (around 90%)

BACKGROUND: Pleural tuberculosis (TB) diagnosis is sometimes controversial because the microbiologic confirmation ratio is very low in pleural fluid. There are few pediatric pleural TB case series in the literature.
METHODS: We retrospectively evaluated our TB cases below 18 years of age and extracted pleural TB cases.
RESULTS: Seven cases with pleural TB were identified. About 42.9% of the patients had isolated pleural TB whereas 57.1% of the patients had accompanying pulmonary TB. Lymphocytic pleural effusion and increased adenosine deaminase (ADA) (>40 U/L) level are found in 85.7% of the

Pleural effusion affects more than 1.5 million people in the United States each year and often complicates the management of heart failure, pneumonia, and malignancy. Pleural effusion occurs when fluid collects between the parietal and visceral pleura. Processes causing a distortion in body fluid mechanics, such as in heart failure or nephrotic syndrome, tend to cause transudative effusions, whereas localized inflammatory or malignant processes are often associated with exudative effusions. Patients can be asymptomatic or can present with cough, dyspnea, and pleuritic chest pain. Dullness to p

Hepatic hydrothorax is an uncommon presentation of decompensated liver cirrhosis and usually presents after other complications of portal hypertension such as ascites. We report the case of a 73-year-old female with autoimmune hepatitis (AIH) treated with budesonide, presenting with a right-sided hepatic chylothorax secondary to AIH and subsequent diagnosis of primary biliary cholangitis (PBC). Pleural fluid analysis revealed a transudative chylothorax, whilst serology, liver elastography, hepatic venous pressure gradients and biopsy diagnosed advanced fibrosis with portal hypertension seconda

BACKGROUND: Anthracosis and anthracofibrosis are attributed to the deposition of carbon particles along with fibrosis, adhesion, narrowing, and collapse. There has been no study on the characteristics of the pleural fluid in anthracosis. The present study analyzed the biochemical characteristics of pleural effusion in patients with pulmonary anthracosis.
PATIENTS AND METHODS: The study is a cross-sectional study which included patients who were referred to the Afzalipour Hospital in Kerman, eastern Iran. Between April 2018 and October 2019, patients who had undergone bronchoscopy and were dia

The biochemical characteristics and smears of the fluids in M-OME, S-OME, AOM, POMC, and AMS were studied. The fluids in S-OME and POMC had the character of transudate rather than exudate. On the other hand, M-OME, AOM, and AMS showed a more exudative or secretive nature. The character of the product from the inflammatory process of the specific condition was particularly seen in M-OME, AOM, and AMS. Thus we speculate that the chronic inflammatory process has a close relationship to the pathogenesis of OME, especially in mucoid types.
