Unilateral conjugate gaze palsy due to a lesion of the abducens nucleus. Clinical and neuroradiological correlations.

We report a case of left-sided horizontal gaze palsy, ipsilateral adduction weakness, and left peripheral facial weakness, all of which indicate the lesion in the left median pontine tegmentum. The enhanced MRIs revealed a discrete left median pontine tegmental lesion, involving the abducens nucleus, MLF, and facial nerve knee. This lesion spared the area of the left PPRF. Among these structures, the area of the abducens nucleus seems to be responsible for the unilateral horizontal gaze palsy. We are not aware of any previous precise neuroradiological documentation of unilateral paralysis of c

Magnetic resonance imaging of the facial nerve.

Magnetic resonance imaging (MRI) of the facial nerve was evaluated by studying normal volunteers and patients with diseases of the facial nerve with a 0.3 Tesla permanent-magnet MRI system with special surface coils. The normal MR images were correlated with the anatomy of thin cryosection specimens of fresh cadavers. The seventh nerve was followed from its nucleus in the brainstem through the temporal bone to the parotid gland bed. The entire labyrinth and tympanic portions, as well as the geniculate ganglion, could be shown with appropriate scan planes. Examples of brainstem diseases affecti

Isolated acquired unilateral horizontal gaze paresis from a putative lesion of the abducens nucleus.

In three patients, acute horizontal gaze pareses developed that could not be overcome with the oculocephalic maneuver, indicating a putative lesion of the ipsilateral abducens nerve nucleus. None of the patients had a facial nerve paresis or evidence of a trigeminal sensory neuropathy. Although most lesions that affect the abducens nerve nucleus also damage the ipsilateral fasciculus of the facial nerve, small lesions in this region can produce an isolated horizontal gaze paresis.

Dual Convergence of Facial Nerve Branches Innervating Whisker Pad in Rats.

The precise anatomy of the facial nerve branches innervating rat whisker pad and the distribution of their corresponding motor neurons in facial nucleus area were investigated. The extratemporal facial nerves of 6 rats were anatomically observed under a surgical microscope, and then the nerve specimens of facial nerve branches at 7 anatomical sites were taken and examined for the axons and myelin sheath using Luxol fast blue staining. The distribution of facial motor neurons innervating the facial branches was observed in 12 rats by retrograde labelling. The distal pes, a fusing architecture o…

Trigeminal herpes zoster and Ramsay Hunt syndrome with a lesion in the spinal trigeminal nucleus and tract.

We report the case of a 77-year-old immuno-competent man who developed herpes zoster in the maxillary and mandibular branches of the trigeminal nerve. Within 3 weeks, he developed ipsilateral peripheral facial palsy, hearing loss, vesicles over the external auditory canal, and pain in the face and ear. A T(2)-weighted MRI of the brain revealed a hyper-intense lesion at the right medulla corresponding to the spinal trigeminal nucleus and tract. Gadolinium enhancement was seen over the right facial nerve. These lesions suggest a possibility of transaxonal spread of the varicella zoster virus bet

Presumed metastasis of breast cancer to the abducens nucleus presenting as gaze palsy.

A 51-year-old woman with breast cancer presented with progressive diplopia. Neuro-ophthalmologic examination revealed right gaze palsy and peripheral facial nerve palsy. Brain magnetic resonance imaging (MRI) was normal. However, two months later a repeat brain MRI revealed an enhancing round nodular mass at the right facial colliculus of the lower pons, at the location of the abducens nucleus. Localized metastasis to the abducens nucleus can cause gaze palsy in a patient with breast cancer.

The motor cortex and facial expression: new insights from neuroscience.

BACKGROUND: For more than a century, unusual and complex deficits in facial expression have been known to occur following localized brain da … he anterior cerebral artery. Both preferentially give rise to bilateral projections to parts of the facial nucleus that innervate the upper facial musculature as demonstrated in the monkey. The facial representation of M1, LPMCv, and M4 preferentially give rise to contralateral axonal projections ending in parts of the facial nucleus that innervate the lower fac …

Afferent and efferent components of the facial nerve in the bullfrog (Rana catesbeiana).

The afferent and efferent components of the facial nerve were traced within the brain stem of Rana catesbeiana, using three different neuroanatomical techniques. Primary afferent fibers could be traced to the spinal tract of trigeminal nerve and to fasciculus solitarius as far caudally as the first or second spinal segment, using silver degeneration methods. Cobalt filling of of the entire nerve showed the same distribution of afferent fibers, as well as the filling of the cells within the mesencephalic nucleus of trigeminal, indicating the origin of a proprioceptive component of the facial ne

Gene expression of NMDA and AMPA receptors in different facial motor neurons.

OBJECTIVES/HYPOTHESIS: Facial motor neurons (FMNs) are involved in the remodeling of the facial nucleus in response to peripheral injury. This study aimed to examine the gene expression of alpha-amino-3-hydroxy-5-methylisoxazole-4-propionic acid receptor (AMPAR) and N-methyl-D-aspartate subtype of ionotropic glutamate receptor (NMDAR) in reinnervating dormant FMNs after facial nerve axotomy. STUDY DESIGN: Animal study. METHODS: Rat models of facial-facial anastomosis were set up and raised until the 90th day. By laser capture microdissection (LCM), the reinnervating neurons labeled by Fluoro…

Facial motor nuclei cell loss with intratemporal facial nerve crush injuries in rats.

OBJECTIVES/HYPOTHESIS: Injuries of cranial nerves that are distal to but near the motor nucleus might result in retrograde motoneuron cell death. The hypothesis of this article is that an intratemporal crush injury of the facial nerve in rats can cause facial motor nuclei cell death. STUDY DESIGN: Prospective, randomized, controlled animal study. METHODS: Sprague-Dawley rats were randomly divided into four groups: intratemporal sham, intratemporal crush injury, extratemporal crush injury, and extratemporal sham. The intratemporal (n = 9) and extratemporal crush injury (n = 4) groups underwen

Regenerative process of the facial nerve: rate of regeneration of fibers and their bifurcations.

After the main trunk of the mouse facial nerve was injured by crushing, a fiber tracing method was used to quantify the facial motor neurons … ys after facial nerve injuries. Double-labeled neurons were consistently found in the nerve-crushed facial nucleus (3.2%), and their number increased in the nerve-transected facial nucleus (12.2%). The present study indicates that the regenerating facial nerve consists of heterogeneous nerve fibers with varying growth rates and that excessive axonal branching oc …

Reduction of oligodendrocyte myelin glycoprotein expression following facial nerve transection.

Oligodendrocyte myelin glycoprotein (OMgp) has been thought to be expressed in the oligodendrocytes and inhibit the regeneration of the nerves by binding to the Nogo receptor expressed in neurons in the central nervous system (CNS). However, OMgp is expressed in the CNS in the neurons as well as oligodendrocytes. In order to help understanding the physiological role of neuronal OMgp, we examined the change of OMgp expression in the facial nucleus after the facial nerve transection. Real-time RT-PCR and Western blot analysis showed a down-regulation of OMgp expression in the facial nucleus 5-7 …

Central origins of cranial nerve parasympathetic neurons in the rat.

The location of central neurons that contribute preganglionic parasympathetic axons to cranial nerves VII, IX, and X in rats has been identified using horseradish peroxidase (HRP) tracing methods. Collectively, these neurons form an uninterrupted dorsal column that extends over the entire length of the medulla. The cephalic end of this column turns ventrally with neurons scattered in the parvicellular reticular formation between the rostral pole of the nucleus of the solitary tract (NST) and the facial motor nucleus. Applying HRP crystals to the cut cervical vagus labels neurons in the classic

NG2 expression in microglial cells affects the expression of neurotrophic and proinflammatory factors by regulating FAK phosphorylation.

Neural/glial antigen 2 (NG2), a chondroitin sulfate proteoglycan, is significantly upregulated in a subset of glial cells in the facial motor nucleus (FMN) following CNS injury. NG2 is reported to promote the resulting inflammatory reaction, however, the mechanism by which NG2 mediates these effects is yet to be determined. In this study, we examined the changes in NG2 expressing microglial cells in the FMN in response to facial nerve axotomy (FNA) in mice. Our findings indicated that NG2 expression was progressively induced and upregulated specifically in the ipsilateral facial nucleus follow…
