Factor XI (FXI) is one of the components of the coagulation cascade, connecting its intrinsic and common pathway. FXI inhibitors have emerged in these few years as interesting therapeutic drugs, with potential advantages over standard anticoagulants in terms of lowering thrombotic risk and limiting bleeding consequences. Better knowledge of the underlying processes of thrombosis led to the design of several clinical trials based on the inhibition of this factor. The discovery of molecules, antibodies, and antisense oligonucleotides capable of binding to this factor is paving the way for new an…

Stroke prevention remains a cornerstone of the management of atrial fibrillation (AF) given the increased risk of thromboembolic events. While direct oral anticoagulants (DOACs) have significantly advanced stroke prophylaxis in AF, bleeding risks remain a challenge and preclude use in some populations. Furthermore, evidence indicates significant undertreatment in AF, with up to 40% of patients receiving incorrect anticoagulant dosing or no anticoagulation at all, further elevating their stroke risk. Novel anticoagulants targeting factor (F)XI are emerging as a promising alternative, offering t

Inhibitors of the protease factor XIa (FXIa) and its zymogen factor XI (FXI) represent a promising new class of anticoagulants that aim to offer safer alternatives to traditional therapies. With their potential to reduce thrombosis without a dose-dependent increase in bleeding, FXIa inhibitors are currently undergoing rigorous phase 3 testing. This article reviews unmet needs in anticoagulation therapy, explains the role of FXI in distinguishing hemostasis from thrombosis, describes FXI/FXIa inhibitors and their effects on coagulation tests, and provides insights into ongoing clinical trials. …

: Congenital factor XI (FXI) deficiency is a mild trauma-related bleeding disorder with estimated worldwide prevalence of one per 1 million. The disorder is less frequent in Iran and a few studies have been performed on Iranian patients. In the current study, we assessed molecular, laboratory and clinical features of two Iranian patients with congenital FXI deficiency and their families. Clinical features and demographic data of the patients were assessed by the physician and a staff member trained specifically to deal with patients with bleeding disorders. FXI activity and antigen assays were…

Factor XIa (FXIa) is a serine protease homodimer that belongs to the intrinsic coagulation pathway. FXIa primarily catalyzes factor IX activation to factor IXa, which subsequently activates factor X to factor Xa in the common coagulation pathway. Growing evidence suggests that FXIa plays an important role in thrombosis with a relatively limited contribution to hemostasis. Therefore, inhibitors targeting factor XI (FXI)/FXIa system have emerged as a paradigm-shifting strategy so as to develop a new generation of anticoagulants to effectively prevent and/or treat thromboembolic diseases without …

Although the current therapeutic armamentarium of venous thrombosis encompasses the use of vitamin K antagonists, heparins, and direct oral anticoagulants, these drugs have several important drawbacks. Antisense oligonucleotides are relatively short single-stranded nucleic acid sequences, which hybridize with a target messenger RNA (mRNA) and suppress protein synthesis. Coagulation factor XI is a key player in blood coagulation, and thus represents a potential target for antisense therapy. The available evidence reviewed in this article suggests that factor XI antisense oligonucleotides may be…

This report describes three patients with a prolonged bleeding time and reduced platelet retention on glass beads, two of the three established criteria for conventional von Willebrand's disease (vWd). However, the third standard, the diminution of factor VIII, was replaced by a decrease of factor IX or XI in the first two instances, respectively, and combined factor VII and IX deficiency was present in the last patient. These associations suggest that the vW (bleeding time/platelet retention) factor is not necessarily homogeneous, may not be dependent on a single genetic determinant, and may

Thrombosis is a major cause of morbidity and mortality. Current antithrombotic drugs are not ideal in that they must balance prevention of thrombosis against bleeding risk. Inhibition of coagulation factor XI (FXI) may offer an improvement over existing antithrombotic strategies by preventing some forms of thrombosis with lower bleeding risk. To permit exploration of this hypothesis in humans, we generated and characterized a series of human immunoglobulin Gs (IgGs) that blocked FXIa active-site function but did not bind FXI zymogen or other coagulation proteases. The most potent of these IgGs…

Coagulation Factor XI (FXI) and its activated form FXIa have been identified as important regulators of thrombin amplification, contributing substantially to pathological thrombosis while exerting a relatively limited role in physiological hemostasis, making it an attractive target for low-hemorrhagic antithrombotic therapy. In recent years, FXI/FXIa-targeted therapeutics have rapidly advanced in clinical trials and represent a promising strategy for precision anticoagulation. This review systematically summarizes the structural features, biological functions, and mechanistic roles of FXI/FXIa…

A 63-year-old man suddenly presented with right hemiplegia and was taken to our hospital. Computed tomography (CT) scan revealed subcortical hemorrhage of the left parietal lobe. He had no medical history except hypertension; thus, it initially appeared to be a typical hypertensive hemorrhage. However, blood analysis showed an abnormally elevated activated partial thromboplastin time. One hour after admission, his Glasgow Coma Scale fell from 14 to 11. We performed an echo-guided stereotaxic removal of the hematoma. He improved immediately and was diagnosed with congenital factor XI (FXI) defi…

A 42-year-old-women with sigmoid colon adenocarcinoma was found to have isolated prolonged activated partial thromboplastin time (aPTT 102.5 s, normal range 24-36 s) preoperatively. Her medical history included an episode of prolonged postdelivery uterine bleeding 16 years previously. A mixed aPTT test showed immediate correction of the prolonged aPTT, indicating a coagulation factor deficiency in the intrinsic pathway. Factor assays showed factor XI was below 1% of average normal value whereas factor VIII, IX and XII activities were normal. Family screening revealed one sister among the three…

We experienced a successful surgical case, who was a 16-year-old boy suffered from atrial septal defect with congenital factor XI deficiency. This disorder is an inherited disorder of blood coagulation characterized by a defect of the intrinsic pathway of thrombin formation and a mild to moderate bleeding tendency, but it can result in severe bleeding in surgical procedures. Open heart surgery in this disorders is rare. Preoperatively we prepared 4000 cc of fresh frozen plasma, but operated without using it. It would appear that the possibility of open heart surgery with congenital factor XI d…

BACKGROUND: Advanced heart failure (HF) is characterized by progressive cardiac remodeling and recurrent episodes of decompensation, highlighting the need for biomarkers reflecting disease severity. Coagulation factor XI (FXI), beyond its established role in hemostasis, may influence inflammatory and remodeling pathways in the heart, but its relevance in advanced HF remains unclear.
AIMS: This study evaluated the association between FXI activity and clinical, laboratory, and echocardiographic parameters in patients with advanced HF.
METHODS: We prospectively enrolled 91 patients with advance…

Thrombosis is a potentially fatal condition for which various anticoagulant therapies have been used for prevention and treatment. However, bleeding events remain a concern with all anticoagulant drugs. Recent evidence suggests that inhibiting coagulation factor XI (FXI) and activated FXI (FXIa) plays a greater role in the formation of pathological thrombi in thrombosis than in normal hemostatic thrombi, allowing for the potential to address these two events separately. Consequently, FXI/XIa inhibition has become the focus of anticoagulant drug research, leading to the development of numerous …

Therapeutic anticoagulation is essential to prevent and treat venous and arterial thromboembolism. The available agents target coagulation factors involved in thrombus formation but are associated with an increased risk of bleeding. Factor XI plays a minor role in haemostasis but contributes substantially to thrombus expansion, making it an attractive target to mitigate bleeding while maintaining antithrombotic efficacy. Various novel inhibitors, including antisense oligonucleotides, monoclonal antibodies and small molecules, have been developed. Phase II trials in orthopaedic surgery showed d…
