Primary healthcare system readiness for the prevention and management of non-communicable diseases in Nepal.

BACKGROUND: In 2016, Nepal adopted the World Health Organization's Package of Essential Non-Communicable Disease Interventions (PEN) to address the rising burden of non-communicable diseases (NCDs). This study evaluated the primary healthcare system's readiness to prevent and manage major NCDs, including cardiovascular diseases (CVDs), diabetes mellitus (DM), and chronic respiratory diseases (CRDs), and associated factors. METHODOLOGY: We conducted a convergent parallel mixed-methods study between 2020 and 2022. We assessed 105 primary healthcare facilities selected through multistage stratif

Fee cuts for radiology and low-value imaging.

Based on the belief that fee-for-service reimbursement contributes to the overuse of costly medical procedures, policymakers have sought to cut payments for low-value services. Using a difference-in-differences model and Medicare claims linked with cancer registry records, we evaluate the impact of a large payment reduction for an imaging procedure commonly used in prostate cancer patients. We find that the payment cut did not affect imaging use, even in low-risk patients for whom imaging is not recommended. Our results suggest that supply may be relatively insensitive to fee levels when decis

Needs survey on the priority given to periodical medical examination items among occupational physicians in Japan.

OBJECTIVES: To clarify the priority given to periodical medical examination items among occupational physicians in Japan. METHODS: Sixty-two occupational physicians who participated in this study selected statutory and non-statutory items within two types of budget plans: one lower (8,500 yen) than the total fee of statutory medical examinations (9,250 and 11,290 yen), and the other higher (12,000 yen) than the total fee. Medical fee points were used to set the cost of each medical examination item. A three round Delphi method was used from May to July 2016 to clarify the consensus opinion of

Trends in Medicare Work Relative Value Unit Payment Rates Across Physician Specialties: 2018-2024.

BACKGROUND: Recent Medicare Physician Fee Schedule revisions have sought to narrow perceived compensation differences between medical and surgical specialties, but their effects on work relative value unit (wRVU) payment rates are unknown. We estimated contemporary specialty-level wRVU payment rates and trends from 2018 through 2024. STUDY DESIGN: Repeated cross-sectional analysis of Medicare Part B fee-for-service claims linked CMS Physician Time File, Addendum B, and Physician/Supplier Procedure Summary Master File data at the CPT-specialty-year level across 42 specialties (27 medical, 15 s

Contemporary cost-effectiveness analysis comparing sequential bacillus Calmette-Guerin and electromotive mitomycin versus bacillus Calmette-Guerin alone for patients with high-risk non-muscle-invasive bladder cancer.

BACKGROUND: Sequential bacillus Calmette-Guerin (BCG) and electromotive mitomycin (sequential therapy) have been shown in a randomized prospective trial to be superior to therapy with BCG alone in patients with high-risk non-muscle-invasive bladder cancer. The objective of the current study was to compare the costs and benefits of these 2 treatment strategies by performing a 5-year and 10-year cost-effectiveness study. METHODS: A Markov model was developed to estimate the incremental cost-effectiveness ratio over a 5-year and 10-year period. Estimates of disease progression, death, and treatm

Cost comparison between surgical and conservative management for pediatric sinogenic subperiosteal abscesses.

BACKGROUND: There remains variation in management of orbital complications of acute bacterial rhinosinusitis (ABRS); specifically, those subperiosteal abscesses that present without immediate surgical indication. Recent systematic reviews on management and proposed treatment algorithms are helpful but do not consider the financial implications for healthcare systems and patients. METHODS: A retrospective chart review of pediatric patients from a tertiary care children's hospital between 2002 and 2020 was performed, identifying patients via ICD coding corresponding to acute bacterial sinusitis

[Clinical Analysis of Evaluation of the Swallowing Function before Gastrostomy in an Acute-care Hospital for Elderly People].

The Ministry of Health, Labour and Welfare, while defining a significant reduction of the medical fee points for gastrostomy in the medical fee revision of fiscal year 2014, assigned additional fee points for evaluation of the swallowing function by videofluoroscopy (VF) or videoendoscopy (VE) prior to gastrostomy. In addition, for facilities that carried out more than 50 gastrostomy operations, evaluation of the swallowing function was made mandatory in all cases and 35% of oral ingestion recovery rate to require the full amount calculation. Therefore, we evaluated the data on swallowing func

Association Between Capitated Payments and Preventive Care Among U.S. Adults.

INTRODUCTION: There is increasing interest in using capitation rather than fee for service to promote primary care and population health. The goal of this study was to examine the association between practice reimbursement mix (majority fee for service versus majority capitation versus other) and receipt of common preventive screening examinations and health counseling from 2012 to 2018. METHODS: Using the National Ambulatory Medical Care Survey, a retrospective cross-sectional study of 24,864 visits with primary care clinicians among patients aged 18-75 years without a cancer diagnosis was c

Medically Unexplained Oropharyngeal Dysphagia at the University Hospital ENT Outpatient Clinic for Dysphagia: A Cross-Sectional Cohort Study.

Medically unexplained oropharyngeal dysphagia (MUNOD) is a rare condition. It presents without demonstrable abnormalities in the anatomy of the upper aero-digestive tract and/or swallowing physiology. This study investigates whether MUNOD is related to affective or other psychiatric conditions. The study included patients with dysphagic complaints who had no detectible structural or physiological abnormalities upon swallowing examination. Patients with any underlying disease or disorder that could explain the oropharyngeal dysphagia were excluded. All patients underwent a standardized examinat

[The treatment of Dupuytren's contracture. A social medical problem (author's transl)].

After a brief description of the characteristics of Dupuytren's disease the operative and conservative therapeutic measures are examined from both clinical and social medical aspects. It is shown in 154 patients with Dupuytren's contracture of the hands in various stages that an advance of this disease could usually be prevented by treatment with soft X-rays. A comparison of costs shows that this therapy is at least 11 times cheaper than a good operative procedure without follow-up treatment. For the patient it is highly important that only little time is lost in the outpatient compared with i
