This prospective study was aimed to evaluate the incidence of osteonecrosis in Thai patients with fracture neck of femur Forty-three patients who had sustained fracture were enrolled in the present study. The overall incidence of osteonecrosis was 72.1%. Incidence of osteonecrosis was 53.3% among patients who underwent surgery within 1 week after injury and 82.1% among those who underwent surgery after that time but there was no statistical difference (p-value = 0.07. Regarding displacement of fracture, the present study found the incidence of osteonecrosis was 100% among patients with undispl

AIM: To establish a proinflammatory cytokine profile of the cerebrospinal fluid (CSF) following trauma.
BACKGROUND: Trauma is associated with a postinjury syndrome consisting of loss of weight and nitrogen, pyrexia, anorexia and fatigue. It has been proposed that cytokines are pathophysiologically involved in this syndrome but the site of action of these is unclear. Previous work in head injury models, supported by animal work, has suggested that one important site of action is the central nervous system (CNS).
METHODS: Women who had sustained neck of femur fractures were enrolled (trauma gr

Treatment of congenital dislocation of the hip, Perthes disease, bacterial coxitis, or fractures in childhood may be complicated by vascular insufficiency and subsequent growth disturbance of the proximal femur. The resulting deformity, with a high-standing greater trochanter and a short femoral neck, causes leg length shortening and insufficiency of the hip abductors with a positive Trendelenburg sign and limp. Normal anatomy and biomechanics of the hip joint can be restored by lengthening the femoral neck after two parallel osteotomies of the femur at the the upper and lower border of the fe

Fifty patients with fractured neck of femur that required surgical correction with either a compression screw or pin and plate device were randomly allocated to receive one of two anaesthetic techniques, general anaesthesia combined with either opioid supplementation or triple nerve block (three in one block) with subcostal nerve block. The nerve blocks significantly reduced the quantity of opioid administered after operation; 48% of these patients required no additional analgesia in the first 24 hours. Plasma prilocaine levels in these patients were well below the toxic threshold, and peak ab

Background: Hyponatraemia is the commonest electrolyte disturbance encountered in older adults admitted with neck of femur (NOF) fracture. It is now recognised both as associated with fragility fracture and as an independent prognostic indicator for adverse post-operative outcomes. Methods: Narrative review of the literature, with emphasis on cohort studies, meta-analyses and mechanistic investigations pertinent to hip fracture in adults. Results: Admission hyponatraemia affects approximately 13-20% of NOF patients, twice the prevalence observed in age-matched community-dwelling elders and bro

OBJECTIVES: Pain management for patients with neck of femur fractures is a challenge for out-of-hospital clinicians. This study aimed to describe (1) the characteristics of patients presenting to emergency medical services (EMS) with suspected neck of femur injuries, (2) clinically meaningful pain reduction, and (3) ongoing moderate to severe pain in the out-of-hospital setting.
METHODS: A retrospective cohort study of adult patients with suspected neck of femur injuries attended by EMS in Victoria, Australia, between 01 July 2015 and 30 June 2023. Multivariable logistic regression was used t

BACKGROUND: The incidence of hip fractures is increasing every year worldwide. Even though hemiarthroplasty is one of the most common procedures performed for a fractured neck of the femur, there is very little research comparing the outcomes of monoblock and modular systems, the two most commonly used prostheses in neck of the femur fractures. It was hypothesized that in narrow femoral canals, the single-size monoblock stem would be more difficult to insert and lead to complications like limb lengthening and intra-operative calcar fractures. The objective of our study was to compare the outco

AIMS: Persistent wound leakage following joint arthroplasty is a known risk for periprosthetic joint infection. Little is known of the predictors of wound leakage, particularly in patients with a fractured neck of femur. We aimed to determine patient and surgical risk factors for wound leakage in this cohort.
MATERIALS AND METHODS: All patients undergoing surgery for a fractured neck of femur at Leicester Royal Infirmary between May and August 2017 were included. Patients were identified from a prospective database and placed into two groups: those with wound leakage later than three days pos

Though uncommon, fracture of neck of femur in children is a devastating injury due to the complications it may cause. Treatment depends on the age of the child, the displacement of the fracture and the type of fracture based on Delbet classification. Surgical treatment is indicated in displaced fractures. We report a case of an impacted fracture of neck of femur in a 12-year-old girl. The girl was managed non-operatively. The fracture united uneventfully. An impacted fracture of neck of femur is common in the adult population. To the best of our knowledge, this fracture pattern has not been re

We present a rare case of a previously healthy 16-year-old boy who sustained simultaneous bilateral femoral neck fractures after a single first-time seizure episode. He was diagnosed to have severe vitamin D deficiency and secondary hyperparathyroidism. Symptomatic hypocalcemia was the cause of seizures. Both fractures were treated surgically and united at 3 months. Bilateral femoral neck fractures after seizures are very rare, especially in children. Severe vitamin D deficiency may cause seizures and also weakening of bone, predisposing to fractures without significant trauma. We recommend th

PURPOSE: There is vast literature supporting valgus osteotomy in fracture neck of femur. However, little or no distinction has ever been made to evaluate the success of the procedure in these two different scenarios-non-unions due to failed osteosynthesis and neglected fractures neck of femur. The aim of our study was to compare the results of valgus osteotomy in neglected neck femur fractures and non-union fractures of neck of femur.
METHODS: This is a single tertiary centre-based retrospective study. The records of all patients aged less than 45 years who underwent valgus osteotomy for neck

This editorial reviews and summarises the current evidence (meta-analyses and Cochrane reviews) relating to the use of hip hemi-arthroplasty for neck of femur fractures. Regarding the optimal surgical approach, two recent meta-analyses have found that posterior approaches are associated with: higher rates of dislocation compared to lateral and anterior approaches; and higher rates of re-operation compared to lateral approaches. Posterior approaches should therefore be avoided when performing hip hemi-arthroplasty procedures. Assessing the optimal prosthesis head component, three recent meta-an

Head preservation of an infected neck of femur fracture appears to be extremely rare with no described cases in literature till date. We present the outcome of head salvage in a young adult with an infected neck of femur nonunion who in addition had chronic osteomyelitic sequelae of his entire femur with reactivation of latent infection in the distal femoral diaphysis. Osteosynthesis was performed by means of cancellous screw fixation augmented with bone substitute following a failed attempt at salvage with a valgus intertrochanteric osteotomy. The patient had an excellent functional outcome w

The fracture in a Factor XIII deficiency patient is being reported for the first time in the literature. We report a displaced fracture neck of femur in a 17-year-old boy with Factor XIII deficiency. Open reduction and internal fixation was done 8 days after the following the initial injury. Two units each of fresh frozen plasma and cryoprecipitate were given perioperatively to prevent excessive bleeding. No perioperative bleeding complications were encountered. At 18-months follow-up, the fracture had united with evidence of avascular necrosis. The fracture neck of femur in a child or young a

BACKGROUND: Neglected fracture of neck of femur in young adults is a major problem in developing countries. Existing treatment options varies from osteotomy to various graft techniques (muscle pedicle, vascularized and nonvascularized fibula etc.). There is paucity of literature regarding the use of double angle barrel plate and hip screw in such cases. We conducted a prospective longitudinal observational study with displaced, neglected fracture neck of femur in young adults and compared the results with literature.
OBJECTIVES: The aim of this study was to determine feasibility and effective

Healing of bone wounds is sensitive to various environmental stimuli. Using knee loading, which has been shown to stimulate bone formation in mouse femora and tibiae, we addressed a question: Does knee loading accelerate a closure of open wounds in a femur neck? A surgical wound (0.5 mm in diameter) was generated at the femur neck in the left and right femora of C57/BL/6 female mice, and knee loading was applied to the left knee for 3 min/day for 3 consecutive days. Surgical holes at the femoral midshaft were used as control. Animals were sacrificed 1, 2, and 3 weeks after surgery for analyses…
