Severe fetomaternal hemorrhage: a review.

The etiology, clinical presentation, obstetrical antecedents, and outcome of pregnancies complicated by large fetomaternal hemorrhage (FMH) were reviewed by doing a MEDLINE search from 1966 to the present and manual search before 1966. One hundred thirty-four infants with FMH > 50 dl were reported in the literature. The primary variables: birth weight, gestational age, presence of sinusoidal fetal heart rate pattern, decrease or absent fetal body movements (FBM) estimated the amount of fetomaternal bleeding and the pretransfusion hemoglobin. Other variables included the condition of the infant

Immunological factors responsible for pathogenetic cell degeneration in pregnancy.

The placenta has an important role as an immunological barrier during pregnancy. When the placental barrier is disrupted, materno-embryonic transfusion takes place. Several clinical reports relate congenital malformations or abortion to intrauterine bleeding or transplacental transfusion. In an earlier experiment, pathogenetic cell degeneration was induced using an in vitro whole rat embryo culture. Transplacental transfusion was simulated by intracardiac injection of an allogeneic rat-antirat serum directed against the blood group antigens. The present study examines the morphological and imm

Rh sensitization after third-trimester fetal death.

Three primigravidas developed Rh sensitization after unexplained third-trimester fetal death. One patient manifested sensitization after the diagnosis of fetal death had been made but before delivery occurred. The other two demonstrated anti-D antibodies early in the next pregnancy, despite having received postpartum Rh immunoglobulin. Unsensitized Rh-negative women with unexplained third-trimester fetal death should be screened routinely for fetomaternal hemorrhage when fetal death is discovered, so that adequate prophylaxis against Rh sensitization can be given.

Chorionic villi sampling changes maternal serum alpha-fetoprotein.

Second trimester amniocentesis has traditionally been utilized for prenatal genetic diagnosis. Chorionic villi sampling (CVS) is presently offered as an alternative. The occurrence of fetomaternal bleed (FMB) during CVS could increase the rate of post sampling abortion and, additionally, be of significance in patients at risk for isoimmunization. Detection and quantitation of FMB can be accomplished by the determination of changes in maternal serum alpha-fetoprotein (MSAFP) before and after CVS.

Diagnosis and treatment of feto-maternal hemorrhage in a fetus with homozygous von Willebrand's disease.

In a fetus with severe type-III von Willebrand's disease, fetal blood sampling by cordocentesis was associated with feto-maternal hemorrhage, fetal hypovolemia, and persistent bradycardia. The fetal condition improved after intracardiac transfusion of blood.

Prenatal treatment of severe fetomaternal alloimmune thrombocytopenia.

Prenatal treatment of fetomaternal alloimmune thrombocytopenia (FMAIT) in previously affected families is of great clinical importance. We report here our experience in the prenatal treatment of 15 severely thrombocytopenic fetuses. Thrombocytopenia was in 13 cases due to immunization to HPA-1a, in one case to HPA-5b, and in one case to HPA-6b. Thirteen fetuses received altogether 34 intrauterine platelet transfusions, seven of them in combination with maternal-administered intravenous gammaglobulin (IVIG) and two in combination with IVIG and prednisone. Six of the 13 fetuses had only one tran

A Five-Year Review of Feto-Maternal Outcome of Antepartum Haemorrhage in a Tertiary Center.

BACKGROUND: Pregnancies complicated with antepartum-haemorrhage is high risk pregnancies associated with adverse maternal, fetal-and-perinatal-outcomes. It contributes significantly to fetal and maternal mortality especially in the developing countries. Proper antenatal care and prompt intervention is necessary to forestall adverse and improve outcome. OBJECTIVE: To determine the prevalence, sociodemographic characteristics, risk factors, fetomaternal outcome of pregnancies with antepartum haemorrhage. METHODS: The case files of the patients were retrieved from the medical records department

Cesarean section in a wedged head.

Cesarean section many a times, has to be done late in labour when the head is deeply wedged in the pelvis. The techniques described in standard text books, usually result in extension of the incision either laterally into the broad ligament or vertically upwards into the upper segment or downwards posterior to the bladder from the centre of the incision line. In this study we have reviewed the Patwardhan's technique for the extraction of baby and fetomaternal outcome was compared with cases where this technique was not used. There was no extension of the incision either laterally into broad li

Treatment options in fetomaternal hemorrhage: four case studies.

BACKGROUND: Significant fetomaternal hemorrhage (FMH) is an uncommon event that places the fetus at risk of severe morbidity and mortality. Symptoms and signs at presentation are subtle and, if promptly recognized, appropriate management may permit the fetus to escape serious injury. CASES: Four cases of significant FMH were diagnosed in the high-risk obstetrical unit at Mount Sinai Hospital, Toronto, during 2003. Three of the women complained of reduced fetal movements and were investigated initially with a non-stress test, a Kleihauer-Betke test, and ultrasound, including Doppler of the mid

Challenges in the Management of Decreased Fetal Movement in Fetomaternal Hemorrhage With Cerebral Palsy: A Nationwide Registry Study.

AIM: To describe how decreased fetal movement (DFM) was managed in patients with fetomaternal hemorrhage (FMH) and to evaluate the diagnostic limitations of this condition in real-world settings. METHODS: This retrospective cohort study included individuals who developed FMH with cerebral palsy from the Japanese nationwide cerebral palsy registry between 2009 and 2022. We investigated whether patients experienced DFM or were prenatally diagnosed with FMH. Furthermore, we assessed how DFM was managed. RESULTS: This study included 57 patients who developed FMH with cerebral palsy, and none wer

Electrical immunosensor for detecting fetal red blood cells with potential diagnosis of fetomaternal hemorrhage.

Fetomaternal hemorrhage, characterized by the passage of fetal blood into maternal circulation, poses significant risks to both mother and fetus. As it may be diagnosed by quantifying fetal red blood cells in maternal circulation, we developed an immunosensor to detect fetal cells made with a layer-by-layer silk fibroin film and a commercial CD71 monoclonal antibody. Detection was achieved through electrical impedance spectroscopy, allowing discrimination between human umbilical cord blood (UCB) cells and adult peripheral blood (APB) cells from patients and volunteers. Positive samples (UCB) e

[Contribution of Doppler in fetal prognosis].

Uteroplacental and cerebral artery velocimetry is a predictive factor in the outcome of many high-risk pregnancies. Positive predictive value is good for intrauterine growth retardation (complications due to preeclampsia, diabetes or twin pregnancy). Velocimetry is also a predictor of adverse outcome in small for gestational age fetuses and for oligohydramnios but not for anemic fetuses of isoimmune pregnancies or post-term pregnancies.

Overestimation of fetomaternal haemorrhage by the acid-elution technique in mothers with beta-thalassaemia minor.

In Rh-negative women, it is important to quantify the magnitude of an Rh-positive fetomaternal haemorrhage (FMH) so that sufficient Rh immune globulin (RhIg) can be administered early in the post-partum period to prevent alloimmunization. The standard post-partum dose of RhIg varies from 100 micrograms in the UK to 300 micrograms in North America. It is therefore important to identify all Rh-negative women who have had an FMH greater than 10 ml in the UK or greater than 30 ml in North America because an FMH greater than these amounts will affect the dose of RhIg that is administered. As acid-e

Chorioangioma placentae and feto-maternal transfusion; a report of two cases.

Chorioangioma (CA) is found in about 1% of all placentas. Here we report cases of CA: one associated with intra-uterine fetal death, and the other with feto-maternal transfusion resulting in severe anaemia of the child. Feto-maternal transfusion through a CA could, in suspected cases, be diagnosed by s-AFP, one-minute alkali-denaturation test and perhaps ultrasound of the placenta. Intra-uterine death of the child could then be prevented by Caesarean section.

Hematologic disorders and nonimmune hydrops fetalis.

Hematologic disorders are implicated in approximately 10% to 27% of cases of nonimmune hydrops fetalis. In almost all of these disorders, anemia leading to heart failure, edema, ascites, and anasarca is the final common denominator. The etiology of the anemia in these cases can be conveniently divided into two categories: (1) excessive erythrocyte loss by hemolysis or hemorrhage, and (2) erythrocyte underproduction. The former include intrinsic erythrocyte abnormalities such as alpha-thalassemia and glucose-6-phosphate dehydrogenase deficiency, and conditions with excessive fetal blood loss su

The influence of fetal loss on the presence of fetal cell microchimerism: a systematic review.

OBJECTIVE: Fetal cells enter the maternal circulation during most pregnancies. Their persistence for years occurs in only some women and has … he engraftment of microchimeric cells in maternal tissue. This may be due to an increased amount of fetomaternal transfusion or to transfer of a cell type that is more likely to engraft. We recommend that investigators in future studies on microchimerism report detailed pregnancy information, since these data are critical for the understanding of factors that influence th …

Fetomaternal hemorrhage: a case highlighting the significance of multifaceted prenatal detection and timely intervention.

INTRODUCTION: Fetomaternal hemorrhage (FMH), a life-threatening obstetric complication with high concealment and poor prognosis, is mostly postnatally diagnosed. CASE REPORT: This case reports the 30 years old primigravida at 32 + 1 weeks, preoperatively identified with FMH via combined indicators: markedly elevated maternal serum alpha-fetoprotein (AFP) (16698.2 μg/L post-dilution), decreased fetal movement, two non-reassuring fetal heart rate tracings, and polyhydramnios. Emergency cesarean section was done. The neonate had severe anemia (Hb 5.9 g/dL) and metabolic acidosis, with Hb rising

Applications of Flow Cytometry in Diagnosis and Evaluation of Red Blood Cell Disorders.

Clinical flow cytometry plays a vital role in the diagnosis and monitoring of various red blood cell disorders. The high throughput, precision, and automation potential of this technique allows for cost-effective and timely analysis compared to older and more manual test methods. Flow cytometric analysis serves as the gold standard diagnostic method for multiple hematological disorders, especially in clinical scenarios where an assay needs to have high sensitivity, high specificity, and a short turnaround time. In this review, we discuss the role of flow cytometric analysis in paroxysmal noctu
